• https://form.jotform.com/201614240957149 on file. We will be using last year’s physical per district and UIL instructions. THIS NEEDS TO BE DONE ASAP SO WE HAVE TIME TO CORRELATE DATA WITH LAST YEAR’S PHYSICAL!
• If you answered yes to any of questions 1-22 on the medical history form and it was documented on last year’s physical you will more than likely not need to have an updated physical. But we will check to be sure.
• If you answered yes to any of the questions 1-22 on the medical history form and it was not documented on last year’s physical you will need written clearance from a physician, physician assistant, chiropractor, or nurse practitioner before any participation in UIL practices, games, or matches.
Basically if you had a medical issue happen since your last physical, specifically pertaining to questions 1-22, you will need written clearance from a medical professional before you can participate.

Health & Safety: Personal Responsibility & Property
Cinco Ranch Cougar Band
Social Distancing - Must be practiced at all times – a minimum of 6 feet
Face Coverings – Staff & Students are expected to have and wear face coverings daily and should be worn at all times except during music practice. Face coverings should cover the mouth and nose completely.
Personal Hygiene• Staff & Students should wash their hands frequently throughout the day’s activity (suggested to bring
hand sanitizer)• Staff & Students will have access to a restroom throughout the day’s activity but it will be controlled to
minimize crowding.• Staff & Students should wash clothes EVERY day after practice.
Student & Staff Private Property• Staff & Students may not share food, beverages, clothing, water bottles, towels, masks, or other
personal items.• Staff & Students should clearly label their water bottle and bring enough water to get them through the
day’s activity. Students who do not bring adequate water for the day’s activity will be sent home by the event staff.
• Students should take all belongings home that they arrive with every day – including instrument if possible.
Health & Safety: Facility Access & Upkeep
Cinco Ranch Cougar Band
Restricted Areas• Staff & Students should arrive at the day’s activities wearing the prescribed practice clothing needed for that particular activity.• Use of Locker Rooms is completely prohibited. (Restrooms should not be used as a changing area for any reason)
Cleaning of Facilities• All indoor facilities will be cleaned and disinfected thoroughly each evening, paying close attention to high traffic and high touch areas• Band & Custodial Staff will insure cleanliness of Band Hall shared resources daily.
Health & Safety: Cleaning Student’s Personal Effects
Cinco Ranch Cougar Band
• Proper disinfecting requires “dwell” time, or time allowed for the disinfectant to be on a surface. The longer it stays on the surface, the more it kills.
• The manufacturer of the district’s cleaning products recommends a minimum 10 minute dwell time, although it has a 1 second contact time for Human Coronavirus.
• Best practice is to spray the surface from 6-8 inches away and allow to dwell.• After dwell time, it can be wiped/rinsed away or left to air dry.
Health & Safety: Symptomatic Persons
Cinco Ranch Cougar Band
If Staff or Students are Observed with COVID Symptoms On-Campus• Anyone who experiences any symptoms, place a mask on the individual, isolate the individual, and arrangements will need to be made for the individual
to be picked up.• The individual should contact their medical provider and report findings to Band Director (who will guide it up the KISD chain of command)
If the Individual has a lab-confirmed case of COVID-19
The individual may return when ALL THREE of the following criteria are met:
1. 72 hours of recovery (no fever/no fever reducing medication) AND
2. Individual has improvement in symptoms AND
3. 10 days have passed since symptoms first appeared
If the individual does not get tested, but has symptoms that
could be COVID-19
It is assumed that the individual has COVID-19, and the individual may not return until the three-
step criteria listed prior has been met OR
If the individual wants to return prior to completing the above self-isolation period, the individual must obtain a medical professional’s note clearing the individual for return based on
an alternative diagnosis.
Anyone living with someone who experiences any of the symptoms of
COVID-19, whether they have a positive test for COVID-19 or not
Should self-isolate for two weeks.
If they do not have any symptoms during/after that period, they may return for participation.
ORIf they do develop symptoms, they must self-
isolate until the criteria listed above have been met.
Staff & Students have a confirmed case of COVID-19Information should be reported to appropriate chain of command: Band Director, school principal, etc. (who will then take it up the Administrative chain) and
then immediately institute the district protocol to determine notification of exposed students and staff, as well as considerations for building sanitization and/or closure/partial closure.
