WELCOME TO UTAH STATE UNIVERSITY, Our Orientation staff and the A-Team are excited to welcome you into the Aggie Family.

For over 30 years, A-Team members have been helping parents and students transition to USU, get involved, and succeed. These student staff members are hired not only for their personality and skills but also for their knowledge about university policies and procedures.

While the threat of spreading COVID-19 may require some of the events or services listed in this handbook to be modified or cancelled, the university remains committed to supporting student learning, discovery, and engagement. REPORTING AGGIES TAKE CARE OF EACH OTHER

Utah State University uses online reporting for many student conduct and Student Code violations. In the event of an emergency, please call 9-1-1. View the reporting forms at studentconduct.usu.edu/reporting. * Report Sexual Assault/Misconduct - Use the Sexual Misconduct Form

to report an incident involving an alleged violation by an active Utah State University student of the USU Sexual Misconduct Policy (sexual harassment, sexual assault, gender-based harassment, intimate partner violence, domestic violence, and stalking). This report form can be utilized by students wishing to file a report on their own behalf or by anyone wishing to file a report on behalf of any person aggrieved by a current USU student.

* Report Student of Concern - Use the Student of Concern Reporting to report a student’s behavior of concern that may lead to disruption of classroom or University activity, or that causes concern of threat to oneself or others. Remember, in an emergency, please dial 9-1-1.

* Report Discrimination/Harassment - Use the Discrimination/Harassment Reporting Form to report discrimination or harassment based on protected categories such as race, color, religion, sex/gender, national origin, age, disability, veteran status, sexual misconduct (including harassment), pregnancy, sexual orientation or gender identity/expression, genetic information, or retaliation.

ADDITIONAL WELLNESS RESOURCES Sexual Assault and Anti-Violence Information (SAAVI)

Why to visit:

ADDITIONAL CAMPUS CULTUREINFORMATIONKEEP CAMPUS SAFE My.usu.edu is your home portal for everything Utah State. It also provides easy access to report threats to campus safety with convenient reporting options on the right side of the homepage. The Office of Equity is also a safe place to report sexual misconduct and discrimination in all forms. You can easily access their website at equity.usu.edu.

DRY CAMPUS Dry campus is a term used for the banning of alcohol and illegal drugs on campus, regardless of the owner’s age. The Student Code at USU states that possession, consumption, sale, purchase, distribution, manufacture, and/or storage of any alcoholic beverage and/or illegal substance anywhere on campus is prohibited.

AMNESTYUSU offers amnesty for drug and alcohol student code violations when reporting sexual misconduct, either on behalf of yourself or another, to the Office of Equity.

USU ALERTS USU issues campus alerts when there is a significant emergency, an ongoing threat, crimes on campus, campus closures, or a safety concern from weather or off-campus incidents. Alerts are sent as text and email through USU's alert system and as push

notifications through the Utah State Safe app. All members of the campus community should keep their USU Alert settings in myid.usu.edu updated. Learn how to download the Utah State Safe app at safeapp.usu.edu.

The goal of the Student Health and Wellness Center is to diagnose and treat illness, and to educate students, staff, and faculty in the development of skills necessary to enhance their personal wellness and ability to be successful in the Five Areas of Life (Spiritual, Mental/Emotional, Physical, Social, and Financial).

USU is required by HIPAA to ensure the privacy and security of all "Protected Health Information" or "PHI" created, received, maintained, stored, or transmitted by or for its Health Care Components and self-insured health plans that are subject to HIPAA.

INSURANCE NOT REQUIREDStudents do not need health insurance to use the student health and wellness center.

HOW TO SUBMIT VACCINESProof of immunization against the following conditions may be provided to the university through the secure server at aggiehealth.usu.edu. You will need your A-Number, USU password, and date of birth to enter the site.

CAPS provides confidential mental health services to students. By offering students a full range of counseling services in a friendly environment, we strive to help them achieve their personal, relational, and academic goals while at USU.

ELIGIBILITY Any student, regardless of credits, is eligible for a consultation. Undergraduates must be enrolled in a minimum of nine credits and graduate students must enroll in 6 credits to participate in individual therapy.

CONFIDENTIALITYStudent confidentiality is respected in accordance with the guidelines established by the American Psychological Association. Counseling records are NOT part of academic or university records and are available to no one unless a student provides a written consent. However, therapists are required to disclose confidential information when there is a danger to the student or others (including child abuse), or under court order.
