We discussed the progress and plans for construction of the Swen's Canyon ventilation shaft. Vegetation has established well on the topsoil pile which was seeded last Fall (September 2016). Growth on the subsoil pile is not as well advanced. Refer to Dwg 3.2.4-4A Swens Canyon Shaft Pad Layout and 2016 Annual Report Swen's Canyon Seed mix.

Topsoil and subsoil piles were seeded in September 2016. The first year vegetation on the topsoil pile was very good. All seeded species were present. Some volunteer species were also noted. The subsoil pile vegetation is sparse, but coming along. The subsoil pile is too steep sided and the pocks have already filled in. The clay subsoil is very erosive and at the same time very hardpacked. The Pernittee plans to seed bare, cut slopes around the Swen's facility this Fall and will reseed the subsoil pile at that time. I suggested they include species adapted to clay soils and increase the wood fiber/tackifier application.

The Swen's Canyon pad berm is a non-erosive construction of concrete filled steel drums. It is a novel use of surplus underground supplies.
4.c Hydrologic Balance: Other Sediment Control Measures
Substation is in place.
18. Support Facilities, Utility Installations
The contractor was on site to begin excavation and surface pour of concrete for the ventilation/escapeway shaft. Much of the construction will be raise-bore.

The permittee has one amendment under consideration/review by the Division. This is Skyline PMLU & Bond Release due to be reviewed by September 29, 2017.

The identification sign was inspected and it meets the requirements of the R645 Coal Rules. See Photo
2. Signs and Markers
I inspected the topsoil piles and I did not see any signs of wind or water erosion. The topsoil piles appear to be stable and protected by a berm.
3. Topsoil
The diversions and culverts were open and functioning as designed. Sediment has started to fill within these structures. The permittee had cleaned out several diversions since the last inspection. See Photos
4.a Hydrologic Balance: Diversions
The mine and the rail loadout sediment pond contain water. The embankments appear to be stable. Inlets and outlets would function as designed. I did not see any hazardous conditions to these structures.
4.b Hydrologic Balance: Sediment Ponds and Impoundments
Silt fences and wattles were inspected and comply with the requirement of the R645 Coal Rules.
4.c Hydrologic Balance: Other Sediment Control Measures
Water monitoring information is taken by the permittee and is sent electronically to the Division in Salt Lake City. This information is reviewed and analyzed by the Division staff. A report is written.
4.d Hydrologic Balance: Water Monitoring
I inspected the refuse pile and it appears to be stable. I did not notice any hazardous conditions to the embankments. The refuse pile is active. No hazardous conditions were reported. See Photos

The property was clear of non-coal waste materials such as oil, trash and grease cans.

I inspected the road diversions and culverts throughout the property. They were functioning as designed. Some of the diversions have some sediment build-up within. Diversions will need cleaning out in the near future.

The site has been idle, the road going through the property was covered with vegetation.
The landowner of the Blazon reclamation site has changed as of December 2012.The new owner is Jacob Lake Hills, LC.; 349 East 200 South; Pleasant Grove UT 84062
It is my opinion that this site should be released from DOGM jurisdiction. The site has been reclaimed for more than 10 years. This reclaim site is well vegetated and the design stream is stable. There have been high water flow and the designed stream channel has held up. The high wall area is stable with no signs of erosion or ground cracks. I cannot see any reason for this property to be inspected by DOGM.

I inspected the topsoil piles and I did not see any signs of wind or water erosion. The topsoil piles appear to be stable and protected by a berm. See Photo

I inspected diversions and culverts throughout the property. They were open and would function as designed during a storm event. The structures were recently re-graded and cleaned out of sediment. See photos
4.a Hydrologic Balance: Diversions
I inspected the sediment ponds throughout the property. They contained some water but were not discharging. It is unlikely these structures will discharge in the near future. The embankments appear to be stable. Inlets and outlets would function as designed. I did not see any hazardous conditions to these structures.
4.b Hydrologic Balance: Sediment Ponds and Impoundments
Silt fences and wattles were inspected and comply with the requirement of the R645 Coal Rules.

I inspected the refuse pile and it appears to be stable. I did not notice any hazardous conditions to the embankments. The refuse pile has been inactive for several years. See Photos

The identification sign was inspected and it meets the requirements of the R645 Coal Rules. See Photo
2. Signs and Markers
I inspected the topsoil piles and I did not see any signs of wind or water erosion. The topsoil piles appear to be stable and protected by a berm.
3. Topsoil
I inspected diversions and culverts throughout the property. They were open and would function as designed during a storm event. The structures were recently re-graded and cleaned out of sediment. See photos
4.a Hydrologic Balance: Diversions
I inspected a couple of sediment ponds. They contain water but were not discharging and unlikely to discharge. The embankments and inlets appear to be stable. No hazardous conditions were noticed. No water was entering the sediment pond.

I inspected the refuse pile and it appears to be stable. I did not notice any hazardous conditions to the embankments. The refuse pile is active.

I inspected diversions and culverts throughout the property. They were open and would function as designed during a storm event. See photos

The sediment at the coal storage yard needs some maintenance at the inlet to the pond. Some additional rip rap is needed to prevent erosion on inter- slope of the pond. Pond does not contain water.

Wattles were working as design along the road to the fan facility.

The refuse pile was re-graded (see photos). I did not see any hazardous conditions to the refuse pile. Slopes appear to be stable.

There are (3) three permitting actions with the Division. These are: 1) Bond Sequence, (Sept 28th), 2) Phase I Bond Release, (October 2nd), and 3) Phase II Bond Release, (October 2nd).

The identification sign was inspected and it meets the requirements of the R645 Coal Rules. See Photo
2. Signs and Markers
Sub-soil and topsoil piles were inspected. They all had identification signs on or next to the pile. I did not see any signs of wind or water erosion.
3. Topsoil
I inspected diversions and culverts throughout the property. They were open and would function as designed during a storm event. The structures were recently re-graded and cleaned out of sediment. See photos
4.a Hydrologic Balance: Diversions
I inspected the sediment ponds throughout the property. Some contained some water and others did not. Sediment ponds were not discharging. Ponds 1, 1b and 3 are schedule to be cleaned out this year. I inspected the embankments and they appear to be stable. Inlets and outlets would function as designed. I did not see any hazardous conditions to these structures.
4.b Hydrologic Balance: Sediment Ponds and Impoundments
Silt fences and wattles were inspected and comply with the requirement of the R645 Coal Rules.
4.c Hydrologic Balance: Other Sediment Control Measures
Blasting records are submitted to the Division.
5. Explosives
Borrow area #1 is completly covered with sub-soil material. It is anticipated that this material will become topsoil in the future. See Photos

The property was clear of non-coal waste materials such as oil, trash and grease cans.
