Online Software for entry of for Base Line Survey is published on the URL: http://ssp-up.gov.in

The Data entry for Base Line Survey is to be done of respective districts. For this purpose they have to collect Password from their District officers.

In the first step, Type the url and then open Default.aspx page

Now Login Panel is open…then select district after that select Area….in Area…we can select Rural or Urban…if we select Rural then select Block otherwise select town

After that we type Password , after selection of block/town

And now after that….input captcha code in the text box of Verify…and click on Log In button..

Now will be open default.aspx page ……name will show with district and block/town

Now you go on menu bar and you can see “entry for baseline servey” on mouse over of this menu three sub menus will be open…1. New records entry (for new entry)…click on it

After click, new form will be open & you select your gram panchayat according your town/block

After selection , all recods will be shown on down, according your selction match, with searching option, you can search your record according your ID or Account Number…

Now , if you want to search….you can enter your ID or account number and click on search button.

your record will show on down..

If you want to new entry of which record….click on it ID…

After click on ID, you go on the first step of the entry form….now …fill the all essential fields in step1 & then click….”save step1” ..button

now …fill the all essential fields of step2 & then click….”save step2” ..button

now …fill the all essential fields of step3 & then click….”save step3” ..button

now …fill the all essential fields of step4 & then click….”save step4” ..button

now …fill the all essential fields of step5 & then click….”save step5” ..button

now …fill the all essential fields of step6 & then click….”save step6” ..button

now …fill the all essential fields of step7 & then click….”save step7” ..button

and now …fill the all essential fields of step8, it is the last step of entry form & then click….”save complete form” ..button

and now entry form is completed…..

if any cause of you can not complete the full entry form of all steps then you can go on the 2nd step of menu bar on “entry for baseline servey” …….on “Partial Filled Forms”

After that, you select your grampanchayat according your area selection….

How many record are uncompleted filled (atleast 1 step filled but not the full step) according this selection of ID

And now you can restart entry form in which step you left…like that entry form…

And the 3rd submenu of menu bar is completed filled form….click on it

And now you can select the grampanchayat according your selection of area selection. in which you want to see the completed forms…

And now click on any of record the message will be show that...registration completed for this ID.

And now you can sign out..on the sub menu of health department and click
