Summary FeedbackHALFWAY EVALUATION:

Feedback from Katherine Miles, OT at Box Hill CYMHS where Alice spent 2 weeks:

"Alice has been doing a great job over the past 2 weeks.

Due to her not having any prior mental health placements, she has mainly been observing various aspects of the case management role, however she has beencontributing well to discussions and reflections, and demonstrates good OT clinical reasoning.

The main things we have covered have included:

- Introduction to IC model, including being able to observe several ICs- Overview CYMHS 4 stage ax model, through discussions and also observing start of current DPC case and observing consultant demonstration case- Introduction to MSE and starting to practice documenting MSE based on observations - Scoring and interpreting sensory profiles- Introduction to the concept of formulation

It’s been difficult to assess if this has been a lot of information to process, however she has not indicated this and seems very keen to learn. I have been impressed withher ability to ask questions and clarify issues.

Alice is still getting her head around all of the above concepts and will need more opportunities to clarify and practice. She is working hard to understand the role of OTin a generic mental health clinical role, and would likely benefit from further discussion around this."

From the last two weeks, I have been impressed with Alice's ability to fit into a new team following adjusting to a new team just two weeks earlier. Alice has taken thischange in her stride and with no complaints.

Alice demonstrates a keenness to learn and observe about CYMHS, the clients, their diagnoses and their treatment plans. Alice will benefit further in this area in thesecond half of placement where she will observe more and also take on a more active role. Alice may also benefit from doing some research in this area.

Alice has demonstrated just in the past two weeks that she has excellent documentation skills and a keenness to practice such skills. Alice is developing her skills inwriting MSE's which is a difficult concept to grab. I would like to see Alice attempt a formulation and assessment report of a client in order to push her documentationand writing skills to the next level.

Alice has only observed clients and their families during assessment and treatment sessions so for this reason she has not been assessed on any direct serviceprovision per se. Having said this, Alice will engage in more direct service provision in the second half of placement.

Alice's communication skills with families is appropriate, engaging in appropriate humour and gentle conversation with families in order to support and develop rapport.

Alice is on the way to developing the skills, experience and knowledge to be a great OT and I look forward to seeing her grow and develop even further in the second halfof this placement.

FINAL EVALUATION:

Over the past 6 - 8 weeks, Alice has been a welcome part of the Wundeela CYMHS team. She has fitted in well with the team and it's culture, engaging with otherclinicians and providing support to them through note taking and documentation.

Alice has engaged in more service provision and direct client contact over the last half of her placement. Alice has sound skills with regards to working with youngpeople around developmental needs. Alice has been able to engage young people and their families through her communication skills. Alice has a great knowledge ofdevelopment with regards to fine motor, gross motor, visual motor and overall coordination skills. Alice developed strong OT session plans for a young client andimplemented them independently.

Alice has good organisation skills, completing work requested of her (notes, reports etc.) in timely manners. I have never had to follow Alice up on anything, rather shehas sent through work in a timely manner. This shows that Alice has the ability to prioritise her work in order to get things completed on time.

This has been Alice's first mental health placement and she has engaged in it appropriately. As mental health is broad, and this type of work is also extremely broad,Alice has begun to show an understanding of mental health concepts, such as diagnoses and treatment. Alice would benefit from further research and experience in amental health context in order to further develop her skills.

Alice has good reflective skills, able to reflect on her own service provision and on families. Alice would benefit from working on her reflection skills which will support herbeing able to use her supervision time more effectively.

Alice has been a welcome addition to the Wundeela team and is on the way to becoming a fantastic OT.

Comments (Final)Alice has demonstrated over the past 6-8 weeks that she has a sound knowledge of confidentiality and respects our client's rights and values in this sense. Alice took onsome constructive feedback regarding confidentiality and has since been able to apply this feedback to her contact with clients.

Comments (Halfway)Appropriate note writing, summarises sessions well. Notes are succinct and appropriate for client files. Respects confidentiality of clients.

Comments (Final)Alice has shown that she has been able to develop her own style of documentation which takes into account the audience (either clients or professionals). Alice adheres toprivacy policies and codes of conduct.

Alice has so far demonstrated appropriate communication skills with clients and their families, both verbal and nonverbal. The task for Alice in the second half ofplacement will be to take on a more active role with communication and engagement.

Final Feedback/Recommendations: (current behaviours/desired behaviours)

Alice has sound verbal communication skills with clients and with parents as she has demonstrated in OT sessions with a young client. Alice was able to communicate theneed for completing certain activities and strategies clearly and also communicated her observations of the young person in their school environment.

Will be helpful for Alice to think about non verbal communication with clients, i.e. note taking during sessions, and the impact this may have on the rapport building andengagement with clients.

Comments (Halfway)Alice is practicing her notes everyday and is demonstrating increased awareness of purpose of notes in addition to developing her own style of note writing. Alice'sdocumentation is fantastic and she should continue with practice in order to support her learning.

Comments (Final)Alice has demonstrated sound skills in report writing, attempting an assessment report on a client. She demonstrated that her skills are improving in summarizing factsand presented a great first attempt at a formulation.

Comments (Halfway)Some confusion over subjective and objective interpretations initially yet Alice has quickly demonstrated an ability to provide both subjective and objective impressions inher MSE's of clients. Alice takes good notes which aide her understanding and documentation.

Comments (Final)Alice's report included factual observations and evidence as informed by the client during assessment.

Alice's documentation skills are excellent which is due partly to her keenness to practice note writing and documentation. Alice has developed good observationskills which she is able to document on paper. Alice has demonstrated good skills with MSE's and further practice in second half of placement will help to improvethese skills. Would like to see Alice complete a report in the second half of placement with the aim of having a family read the report.

Alice's written skills have improved significantly over the past 6-8 weeks, demonstrating that she can cut down on written information in order to provide moresuccinct reports and note writing. Alice presented an excellent first attempt at an assessment report and formulation.

Summary Feedback at Halfway Evaluation :Feedback from Katherine Miles, OT at Box Hill CYMHS where Alice spent 2 weeks:

"Alice has been doing a great job over the past 2 weeks.

Due to her not having any prior mental health placements, she has mainly been observing various aspects of the case management role, however she has been contributingwell to discussions and reflections, and demonstrates good OT clinical reasoning.

The main things we have covered have included:

- Introduction to IC model, including being able to observe several ICs- Overview CYMHS 4 stage ax model, through discussions and also observing start of current DPC case and observing consultant demonstration case- Introduction to MSE and starting to practice documenting MSE based on observations - Scoring and interpreting sensory profiles- Introduction to the concept of formulation

It’s been difficult to assess if this has been a lot of information to process, however she has not indicated this and seems very keen to learn. I have been impressed with herability to ask questions and clarify issues.

Alice is still getting her head around all of the above concepts and will need more opportunities to clarify and practice. She is working hard to understand the role of OT in ageneric mental health clinical role, and would likely benefit from further discussion around this."

From the last two weeks, I have been impressed with Alice's ability to fit into a new team following adjusting to a new team just two weeks earlier. Alice has taken this change inher stride and with no complaints.

Alice demonstrates a keenness to learn and observe about CYMHS, the clients, their diagnoses and their treatment plans. Alice will benefit further in this area in the secondhalf of placement where she will observe more and also take on a more active role. Alice may also benefit from doing some research in this area.

Alice has demonstrated just in the past two weeks that she has excellent documentation skills and a keenness to practice such skills. Alice is developing her skills in writingMSE's which is a difficult concept to grab. I would like to see Alice attempt a formulation and assessment report of a client in order to push her documentation and writing skillsto the next level.

Alice has only observed clients and their families during assessment and treatment sessions so for this reason she has not been assessed on any direct service provision perse. Having said this, Alice will engage in more direct service provision in the second half of placement.

Alice's communication skills with families is appropriate, engaging in appropriate humour and gentle conversation with families in order to support and develop rapport.

Alice is on the way to developing the skills, experience and knowledge to be a great OT and I look forward to seeing her grow and develop even further in the second half of thisplacement.

Summary Feedback at Final Evaluation :Over the past 6 - 8 weeks, Alice has been a welcome part of the Wundeela CYMHS team. She has fitted in well with the team and it's culture, engaging with other clinicians andproviding support to them through note taking and documentation.

Alice has engaged in more service provision and direct client contact over the last half of her placement. Alice has sound skills with regards to working with young peoplearound developmental needs. Alice has been able to engage young people and their families through her communication skills. Alice has a great knowledge of developmentwith regards to fine motor, gross motor, visual motor and overall coordination skills. Alice developed strong OT session plans for a young client and implemented themindependently.

Alice has good organisation skills, completing work requested of her (notes, reports etc.) in timely manners. I have never had to follow Alice up on anything, rather she has sentthrough work in a timely manner. This shows that Alice has the ability to prioritise her work in order to get things completed on time.

This has been Alice's first mental health placement and she has engaged in it appropriately. As mental health is broad, and this type of work is also extremely broad, Alice hasbegun to show an understanding of mental health concepts, such as diagnoses and treatment. Alice would benefit from further research and experience in a mental healthcontext in order to further develop her skills.

Alice has good reflective skills, able to reflect on her own service provision and on families. Alice would benefit from working on her reflection skills which will support her beingable to use her supervision time more effectively.

Alice has been a welcome addition to the Wundeela team and is on the way to becoming a fantastic OT.
