HIV Infections- Where are we?

• India has the 3rd largest population of PLHIV in the world (after South Aftica and Nigeria).

We have more PLHIV than North and South Americas and Europe (not including Russia) put together.

• Karnataka, Former Andhra Pradesh, Tamil Nadu and Kerala together has more PLHIV than all other states of India put together.

• Good news! New infections are declining in these states.

HIV Care- Where are we?

• Only 50% of PLHIV are on free ART through the government -distributed through public health care.

Challenges of PHCs – Increased risk of OIs at public health care centers. Stigma and discrimination Bureaucracy

• Government funding for non-ART treatment has virtually stopped since 2013 for lack of funds. Funding for Sneha Care Trust from government sources has also stopped.

For longevity, PLHIV need several other supporting medical/social infrastructure in addition to ART.

– Hygienic environment to reduce OIs.– Pro-active treatment of OIs – Nutritional support to boost immunity.– Counseling to deal with stigma and discrimination.

• Almost two decades in HIV care.

• Involved in HIV Care - ART administration, OI management, Immunity management, Counseling, Training and Rehabilitation and Palliative Care.

SNEHA CARE TRUST Holistic approach to care for PLHIV and terminally ill.

• Dedicated staff and support team with the ability to carry on the challenges of PLHIV management without discrimination or stigma.

ACHEIVEMENTS• 1) Close to 300 children being provided full time residential

care and education starting from 2008.

• 2) Close to 60 adults being provided full time residential care and treatment and they are mostly terminally ill patients and destitute.

• 3) 50 adolecents and young adults provided residential care, educational, vocational training and being readied to return to society when they reach adulthood.

4) Over 10000 adults are given in-patient care at various centres such as Snehadaan, Snehakiran, Snehatheeram and Snehasadan, Mangalore.

5) Over 9500 adult patients given out patient care.

6) Treated as a model HIV care center and training provider from 1997 to 2012 by Karnataka State AIDS control Society and Central Government. (National Aids control society).

Sheha Care Home and Shining Star School, Bangalore -Facility to care for 100 children with residential, educational, socio-psychological and HIV care.

Snehasadan, Mangalore – Residential care facility for 46 children and 14 adults. Scope to expand to accommodate more.

Snehakiran Secunderabad – Residential care facility for 35 Children and adults . Scope to expand to accommodate more.

Focus on regular ART administration, nutritional support, childhood needs.

Shehagram, Krishnagiri Tamil Nadu:Facility to care for 200 adolescents. Currently 46 adolescent children are provided with residential, educational, occupational, socio-psychological and HIV care facilities.

Snehadaan Hospice, Bangalore: 55 bed hospital with ICU and support staff for managing OIs and terminally ill adult PLHIV.

Camilian Task Force, Bangalore. • Established in 2001 to renders disaster response services worldwide. • Offers support to lessen the trauma of sudden loss, injury and disaster. • Mobilises an emergency team of in-house experts and medical volunteers

to provide curative treatment, prevention, pastoral care, rehabilitation and education in remote locations.

• Recent activities include Disaster relief in Uttarakand and Seirria Leone (Africa).

CHALLENGES

• Funding in the absence of government support–day to day operations.

• Demand exceeds capacity at Snehacare. – Scaling infrastructure and support with increasing children.

• Aging infrastructure, especially at the Hospices– 17 years old infrastructure, needs renewal.

• Demand for palliative care – terminal patients.

• Missed opportunities– Number of calls which we cannot attend.
