Terms & Conditions
The Smile for Life Plan is available to all adult patients over the age of 18 years of age.
The Smile for Life Plan is not transferable, this is HDC’s Dental Plan with no alternatives. Complimentary whitening cannot be transferred to another person, this is only available to the named policy holder.
All patients must have had a full examination prior to joining the Smile for Life Plan. Payment is monthly by direct debit.
If the direct debit is cancelled within the first 12 months of the agreement, then full treatment fees will need to be paid retrospectively for benefits already received.
One set of custom made home whitening trays will be provided, with an at home whitening kit
Should additional tubes of whitening gel be required, or a new set of whitening trays, these will need to be purchased by the policyholder.
A one off £10 registration fee is payable per customer on the plan.
The plan can be terminated in writing, CODEplan will require written notice of termination of your contract, providing 3 month’s notice of termination.
^5 year guarantee only applies where a patient attends all examinations and hygienist appointments covered in the Smile for Life Plan they have signed up to. Any written requirements by their dentist for upkeep of their veneers and crowns must clearly have been adhered to. Should any of these points not have been adhered to the guarantee will be invalid.
^^10% discount excludes implants, orthodontic and all other specialist fees
^^10% discount includes additional all hygiene appointments not covered in your plan including routine hygiene or hygiene treatment appointments
**Each patient is individually recommended their plan depending on their health and needs. Example shown is for individuals with very healthy teeth and good oral hygiene.
Should original whitening trays be lost, replacement trays are replaced at patient’s cost
Spread your costs and SAVEAs a member of our Smile for Life Plan, you will also benefit from a 10% discount on all dental treatments over and above your routine examinations covered in this plan.^^ What’s more, our 10% discount even extends to any facial treatments provided by HDC. Plus we will also offer 0% finance over 12 months for treatments over £1000, with a 20% deposit.

Join our plan today and start enjoying the benefitsSimply fill out the attached application form, then hand it into our receptionist who will post it for you, or simply post it to the address at the base of the application form. There is a one-off joining fee of £10 for each patient.
InsuranceHDC’s Smile for Life Plan includes world-wide dental accident cover, emergency and out of hours insurance. For details please refer to the insurance Terms and Conditions.
Your registrationYou may withdraw from your plan at any time by simply giving 3 month’s notice in writing to CODEplan, our scheme administrators.
Changing dental needsShould your dental requirements change resulting in you needing fewer or more routine appointments covered in your policy, this can be amended accordingly by advising CODEplan direct.

Our Dentist and Dental Hygienist at HDC will examine and assess the health of your teeth and gums. As a result of these examinations they will recommend how many examinations and hygiene therapy treatments you will require each year. From this prescription they will then recommend the level of Smile for Life Plan that will best suit you.

The Smile for Life Plan is not transferable, this is HDC’s Dental Plan with no alternatives. Complimentary whitening cannot be transferred to another person, this is only available to the named policy holder.
All patients must have had a full examination prior to joining the Smile for Life Plan. Payment is monthly by direct debit.
If the direct debit is cancelled within the first 12 months of the agreement, then full treatment fees will need to be paid retrospectively for benefits already received.
One set of custom made home whitening trays will be provided, with an at home whitening kit
Should additional tubes of whitening gel be required, or a new set of whitening trays, these will need to be purchased by the policyholder.
A one off £10 registration fee is payable per customer on the plan.
The plan can be terminated in writing, CODEplan will require written notice of termination of your contract, providing 3 month’s notice of termination.
^5 year guarantee only applies where a patient attends all examinations and hygienist appointments covered in the Smile for Life Plan they have signed up to. Any written requirements by their dentist for upkeep of their veneers and crowns must clearly have been adhered to. Should any of these points not have been adhered to the guarantee will be invalid.
^^10% discount excludes implants, orthodontic and all other specialist fees
