SECTION 1: INTRODUCTION The AWAIR Act (Minnesota Rule 5205.1500) was passed in effort to reduce occupational injuries and illnesses. The intent of the legislation is to help make safety an important part of each workplace by making employers and employees more safety conscious. The health and well-being of all SMG - US BANK STADIUM’s employees, suppliers, and customers is important. It is vital that we take control of the situations that are presented to eliminate any possible hazards. We will actively participate in the program providing a safe work environment and facilities for everyone. The company’s goal is to reduce or eliminate the number of safety and health related accidents, injuries, illnesses, and losses. To accomplish this, SMG - US BANK STADIUM will comply with all current occupational health, safety, and environmental laws. We will also develop and implement operations, procedures, and policies to provide and maintain a safe workplace. All accidents, injuries, and illnesses must be reported immediately to supervisors. The occupational health clinic is our primary clinic when looking for care. Each incident must be followed up with a First Report of Injury and Illness packet. Company property must be operated in a safe manner following the proper procedures. The company property includes tools, machinery, vehicles, and any other property that is owned by SMG - US BANK STADIUM and given to the employee for use on the job. If there are any malfunctions or damage to the company property, it should be brought to the supervisor’s attention immediately. We will provide our employees with the proper protective ware that they need to be around hazards. This will consist of any items that are listed on our chemicals or required by OSHA. PPE will be supplied to every employee who needs it and must be worn when working around hazards. SMG - US BANK STADIUM will provide employees with the means to perform and complete their work safely. Any malfunctions in equipment, procedures, and personal protective equipment will need to be addressed immediately. Supervisors are given the necessary tools and resources needed to assist their employees during working hours.

SECTION 2: GOALS AND OBJECTIVES

Safety Vision

It is SMG-US Bank Stadiums goal to establish and maintain a stadium culture that is committed to workplace safety and health in effort to prevent accidents, illnesses or lost time. SMG-US Bank Stadium will work toward this objective by providing thorough safety training, monthly safety committee meetings, analysis of injuries and incidents with a goal to prevent repetitive occurrences and a general desire to build an overall awareness of employee safety and health. All safety rules will be enforced throughout the stadium. SECTION 3: ROLES AND RESPONSIBILITIES Everyone is responsible for safety. We can practice safe work habits through our everyday duties. Understanding how to properly perform our responsibilities will help us determine what we improve on. To support our safety and health we have directors, managers/supervisors, and the employees. All Everyone needs to practice safety by following the rules and procedures that are put in place. These are put in place for you and help guide us to safer work habits. It is important to acknowledge when someone is doing something wrong and show them how to perform the task properly. Employees Be accountable for your safety and the safety of others. Employees need to understand and follow all safe practices, policies and procedures. It is important to perform your job safely. If you are unsure of how to perform it safely, please discuss with your manager/supervisor. Wear the appropriate PPE for the job that you are performing and maintain the equipment in a safe and sanitary manner. Promptly report any safety and health hazards you observe to your manager/supervisor or safety committee representative. Employees must take on the responsibility of responding to an accident immediately. It is important to observe our surroundings to prevent accidents and maintain a readiness in case something does happen. Once an accident is witnessed, employees must notify their supervisor right away. We can work on getting medical assistance and filing a Workman’s Compensation Claim.

Managers/Supervisors Managers/Supervisors must keep employees informed and up-to-date with any safety information. They must also address any safety concerns immediately to prevent future accidents. Once there is an accident, managers/supervisors must perform a thorough investigation and obtain as much information as possible. To set an example for their subordinates, managers/supervisors must maintain a level of commitment to a safe work environment through leading by example and constant follow through. Safety Committee The safety committee will conduct monthly meetings and area inspections to review accident reports, identify hazards and address any and all safety concerns raised by employees, managers/supervisors or the safety manager. Safety committee members each represent their particular work area and, therefore, should address all safety concerns brought to them by their coworkers. These concerns should be handled by initially investigating the issue with the area manager/supervisor to determine if the concern is valid and then, as necessary and appropriate, bring the issue to the safety manager or the full safety committee. Reviewing the accidents can help develop a strategic plan for the future. The plan can consist of errors that need correction and ways to prevent reoccurrence. Open communication is key to safe working habits. To ensure that we maintain a safe work environment we must work together. Every person needs to take responsibility for their actions and practice safety. It is important to keep communication open and constantly review accidents. Planning for the future is based off of what we do today. Working safely will make our future brighter.

SECTION 4: HAZARD IDENTIFICATION, ANALYSIS AND CONTROL

Workplace hazards at SMG - US BANK STADIUM – US Bank Stadium will be identified through review of accident records, preventive maintenance reports, First Report of Injury, OSHA 300 log, safety inspections and employee hazard reports. Periodic safety inspections of working areas will be done for safety and health committee review and follow-up.

Hazards will be controlled by correcting hazards found through evaluating information from the safety inspections and job safety analysis, providing PPE, enforcing safety policies, and training programs. New equipment, materials and processes will be evaluated for safety and health impacts before they are purchased or implemented. Job safety analysis will be prioritized according to potential for causing severe injury. Engineering and maintenance departments will work in conjunction with the safety management to institute engineering and mechanical safeguards. JOB SAFETY ANALYSIS PROGRAM APPLICATION The Job Safety Analysis Program will encompass the following steps:

1. Job Selection Criteria

• A job that has a high frequency of accidents

• Jobs that may produce a high frequency of disabling injuries

• Jobs with potential to cause severe injuries or loss

• New and/or revised jobs

2. Job Broken Down Into Steps

• Select the right person to observe

• Each step completely described with employee verification

• Note possible deviations from regular procedures

3. Identification of Hazards and Potential Accidents

• Electrical exposure

• Overexertion

• Environmental (spills) and Physical hazards (burns chemical and thermal)

• Possible slip, trip, fall, etc.

• Danger of striking against, being struck by, or making harmful contact with an object

4. Solution Development

• Find a new way to do the job

• Change the physical conditions that create the hazard

• Change the work procedure and identify additional training for employees

• Implement engineering controls

• Use of PPE

• Use of proper equipment guards

SECTION 5: COMMUNICATION Safety education and training is a fundamental part of the safety program at SMG - US BANK STADIUM – US Bank Stadium and an import factor in accident prevention. Manager/Supervisor Training Managers/Supervisors need to be adequately educated and trained to accept the responsibilities in relation to safety. The objectives of safety training and education will be based on the following:

• To establish the Manager/Supervisor as the key person in preventing accidents.

• To provide Managers/Supervisors with information to causes of accidents and occupational health hazards and methods of prevention.

• To get Managers/Supervisors to understand their safety responsibilities which include reinforcing safety training and enforcing safety.

• To help Managers/Supervisors gain skill in accident prevention.

• To involve Managers/Supervisors an opportunity to consider current problems of accident prevention and to develop solutions based on their own and other’s experience.

• To help Managers/Supervisors keep their own departments/areas safe.

• To help establish monthly “tool box talks” safety meetings for specialized training needs.

Employee Training Conducting intensive safety training for all employees makes the greatest progress toward accident prevention. Safety information will be communicated through one-to-one safety contacts, safety meetings, bulletin boards, and safety orientation. Employee safety training and education objectives:

• The company philosophy of safety.

• New employee orientation and introduction to the safety program.

• Explanation of specific safety rules that apply in the new employee department/area.

• The importance of properly reporting all injuries to supervisors.

• Reporting unsafe conditions and practices to managers/supervisors.

• Required attendance at the periodic “tool box talks” safety meetings.

• Training requirements are noted in each safety program (i.e. Right-To-Know, Lockout/Tag out, etc.) as it pertains to the work area.

• Provide training on how to properly and safely use equipment and machines required by the employee’s position.

• Training to question processes and operations the employee is not certain of. All training sessions will be documented. The date of the meeting name of instructor, subject discussed, and names of employees attending the meeting will be documented on the safety meeting attendance form. EMPLOYEE COMMUNCIATION BACK TO MANAGEMENT Employees are encouraged to communicate safety related issues back to management for review and/or action. Employees can do this by submitting a safety concern to a member of the safety committee or management staff . An employee can verbally talk with a member of the safety committee to have them ring up the issue at the next meeting and possibly be invited to talk about the issue at the next meeting. Employees are encouraged to fill out a Job Hazard Analysis form and/or a Near Miss report and submit that to the safety committee at any time. NEW EMPLOYEE SAFETY TRAINING SMG – US Bank Stadium will provide safety training to all new employees prior to starting work. It will include but not be limited to the following:

1. AWAIR 2. Right-To-Know 3. Globally Harmonized System of Labeling 4. Personal Protective Equipment 5. BBP/Exposure Control Plan 6. Lock Out/Tag Out 7. Forklift/Material Handling 8. Ergonomics 9. Stadium Evacuation 10. Severe Weather Procedures 11. Hearing Conservation

SECTION 6: ACCIDENT INVESTIGATION An accident is defined as “an unplanned event which may or may not have caused an injury.” Accident investigations are created to help identify and prevent hazards. It is created not to blame employees, but to determine how to prevent future accidents from happening. The accident investigation will look at the details of any injuries, illnesses, or damages to people or property. It will investigate the events that create near miss opportunities for

hazards. This includes situations that aren’t hazardous but may become so. These investigations will also include the evaluation of the First Report of Injury. There are two primary levels that are investigated, direct and indirect causes of injury, illness, or damage. Direct causes include the person or property that was affected by the hazard. The indirect cause is the chemical or machinery that caused the accident. Everyone must assist in the investigation. It is important to analyze the situation from every angle and address every possibility that has influenced the hazard. No matter how insignificant an influence may seem, it has the possibility of turning in to a much larger accident. The following questions should be answered when investigating an accident:

➢ Who was involved?

➢ Were there any witnesses?

➢ What happened (in detail)?

➢ What was different about the situation that may have influenced the accident?

➢ When did the events occur in the accident?

➢ Where was the first occurrence of the hazard?

➢ How and why did the event take place?

Please see Accident Investigation and Record Keeping Program for specific details. ACCIDENT PROCEDURE: Step 1 Assist the employee to a safe area and take to the clinic or call 911 based on severity of injuries. Step 2 Inform the supervisor immediately. Step 3 Complete SMG - US BANK STADIUM First Report of Injury/Illness Packet Step 4 Turn in packet to HR Manager. HR Manager will share a copy to any applicable personnel to complete the investigation. See Accident Investigation and Record Keeping Program.

RETURN TO WORK PROGRAM SMG – US Bank Stadium’s Return to Work Program will let employees know that they are valued productive members of the company even though they are not 100% well. It keeps the employees in the work setting earning money for “working”, rather than for being injured. This will decrease the chance that an employee will have a delayed recovery and/or develop Chronic Pain Syndrome.

1. The doctor should clearly outline any activity restrictions on the Report of Workability.

2. Prior to the employee performing work duties, the manager/supervisor should discuss the restriction with the employee to make sure that the employee understands her/his restrictions and responsibilities while in transitional/light duty.

3. The employee has the responsibility to inform the manager/supervisor of any progress or problems.

• The employee must provide a Report of Workability updating her/his progress EACH time s/he sees the treating doctor.

• The employee must inform the supervisor if s/he feels that s/he is unable to continue working due to pain, if the job is aggravating her/his injury, or if the job activities exceed her/his restrictions.

o If the employee is unable to do the job and/or is unable to come into work, the employee must go and see the doctor on the same day to get evaluated and possibly to update the restrictions.

4. The manager/supervisor should discuss the specific work that is to be done by the employee and assures that the employee understands the job.

5. The supervisor should express ongoing interest in the employee during the employee’s recovery. Ask the employee, “How are things going?” The overall

6. work environment should support the employee’s return to work and should not be adversarial.

7. Notify Human Resources or the workers compensation carrier if difficulties arise.

• The employee does not return to work.

• Unsure of what the restrictions are.

• Difficulty in identifying a job for the employee.

• General questions on return to work program. SECTION 7: ENFORCEMENT OF SAFETY AND HEALTH PROGRAMS The purpose of SMG – US Bank Stadium’s disciplinary program is to provide a method for ensuring compliance with rules concerning operation, personnel, safety, security, and other regulations. All employees are subject to this program. Personnel authorized

to enforce or administer the disciplinary program are determined by management. Fair and impartial enforcement requires that the circumstances of each case be determined and that disciplinary action, when appropriate, be taken and documented. SMG-US Bank Stadium reserves the right to enforce the following steps with regard to disciplinary action:

First violation: Verbal warning and instructions regarding the violation, proper procedures, and the hazards. Possible termination of employment.

Second Violation: Written warning with suspension. Possible termination of

employment. Third Violation: Written warning with suspension. Possible termination of

employment. Fourth Violation: Termination of Employment.

SECTION 8: PROGRAM REVIEW SMG – US Bank Stadium will review all documents, policies, procedures, near miss, accident and injury reports and any other relevant documentation created during the past year on workplace safety and health. This will be done to determine if the prior years’ safety goals were met, what hazards were identified and corrected, and to make sure that all policies and procedures represent the current actions and activities at SMG – US Bank Stadium. 1. Review of OSHA 300 log

The log and the accompanying First Report of Injury forms should be examined for trends, such as similar injuries to those workers with similar job duties, similar causes of injuries and illnesses, or departments with higher than average injury rates.

2. Review of Accident Investigation Reports and Near Miss Reports Determine if similar accidents or near misses continue to occur in the facility, perhaps the root causes of the events are not being determined during the investigation or corrective action is not being implemented.

3. Review reports from safety committee or manager/supervisor inspections or walk rounds

If hazards causing injury or illness in the workplace are not being identified through these inspections, then efforts should be made to assist the individuals to improve the process.

4. Review current policies/procedures Identify the current status of health and safety programs, OSHA compliance and need for training. Review progress of safety program and written plan.

5. Review of Hazard Communication Program

Identify last date of training for Right-To-Know, location of SDS, type of chemical labeling system and date of respirator fit tests (if applicable).

6. Supervisor Training

Review AWAIR program requirement such as disciplinary policy for safety violations, documentation forms, training, accident investigations, return to work policies and safety committee implementation.

7. Review AWAIR Program SECTION 9: SAFETY COMMITTEES SMG - US BANK STADIUM has gathered a group of employees from various departments to take part in the evaluation and implementation of safe work practices. The committee will address concerns and create a plan for prevention. It is our goal to maintain a safe and clean work environment for everyone. PURPOSE OF SMG - US BANK STADIUM’S SAFETY COMMITTEE

1. Review accident investigation records. This will identify areas where rules/procedures may need to be revised or institute new job specific training.

2. Review safety inspections. The inspections are designed to identify areas that

are not in compliance with safety regulations.

3. Review internal inspections. Inspection identify workplace hazards.

4. Review employee hazard reports. Hazard reports identify employee concerns about their workplace. Every hazard report must be addressed. The results of the corrective actions should be communicated to all employees.

5. To represent employees on safety issues.

• Find new ways to promote safety

• Increase employee safety awareness

• Participate in walk around inspections (audits) to analyze the work areas

• Train new employees in proper work habits

• Show management concern for employees’ well-being

• Gather safety-related suggestions and other information from employees

• Improve work environment and activities

6. To address work-related injuries. Assist in accident investigations provide input for future plans of prevention and address and collaborate ideas to prevent future accidents.

7. To monitor compliance issues. Reduce risk of OSHA citations

Monthly meetings will be conducted. Meeting agenda’s and notes are posted for all employees to review.
