VISION

To be recognized as a culturally competent premier lead organization for the provision of sound integrated primary healthcare, nutrition and diabetes care interventions, in the Northern region of New Jersey with emphasis in East Orange and the surrounding service areas of Essex County. This region is made up of large urban cities such as East Orange, Newark, Orange, South Orange and others. Large ethnic/minority populations of African Americans, Blacks, Hispanics/Latinos as well as migrants from many African countries and the Caribbean reside in the northern area of New Jersey who will benefit from the proposed primary healthcare, nutrition and diabetes care interventions.

Consumer primary prevention, diabetes, nutrition and health management intervention strategies will be implemented to address health prevention and disease promotion to combat chronic disease conditions such as cardiovascular and diabetes mellitus. Emphasis will also be placed on the provision of nutrition interventions to halt the epidemic of obesity and overweight both in adults and children. Diabetes Mellitus which is now at an epidemic level is closely linked to obesity which will be a major focus, as well as hyperlipidemias, heart disease, hypertension and other chronic conditions which are present in the target high risk ethnic minority populations of African Americans, Blacks, and Hispanics at higher rates and with current health projections that the incidence of obesity and diabetes mellitus will continue to increase in these ethnic minority population groups.

The chronic care model will be used model with Primary Care Innovations by Nurse Practitioners to promote spread and quality healthcare.

The Chronic Care Model will be the focus of the provision of Primary Family Health care and the DSME & S Program; the use of the Chronic Care Model is effective and relevant for those who have been diagnosed with diabetes mellitus and other chronic disease conditions. The model’s creator Ed Wagner stated that with this approach, “Patients gain the ability and interest in managing their own conditions.” The key to the whole approach is healthcare providers becoming teammates with patients to empower patients to take more control over their illnesses. Wagner defines chronic illness broadly as “Any condition that requires ongoing activities and response from patients and their caregivers, as well as a response from the medical care system”.

OBJECTIVES

1. To provide high quality comprehensive primary care, according to established primary (family) care standards and guidelines to 100% of clients/patients provided with healthcare services.

2. To promote clients/patients adherence to 80% to medical nutrition therapy interventions.

Sivam Diabetes, Nutrition & Family Health Care (SDN & FHC) LLC. is a minority owned registered business in the State of New Jersey, specializing in the provision of primary family healthcare, medical nutrition therapy (MNT) and the provision of diabetes care (DSME & S). The purpose of this company is to establish a new primary healthcare Family Nurse Practitioner (FHNP) practice with a focus on diabetes care, and the provision of medical nutrition therapy in East Orange, New Jersey. Family healthcare is comprehensive healthcare and is a form of primary care that provides continuing healthcare for the individuals and family across all ages, sexes (gender), diseases and all parts of the body. Primary and family healthcare providers are the gatekeepers of the Unites States (U.S) health system. Primary care and family healthcare providers practice health promotion, disease prevention, health maintenance, counseling, patient education and diagnosis and treatment of acute and chronic illnesses in a variety of settings (e.g., office, inpatient, critical care, long-term care, home care and day care). They are the first point of contact for people seeking healthcare by providing care for the whole body

Primary Care, Family Health Care Providers has contributed to the healthcare industry revenue growth in recent years. Most recently the health care reform of the Affordable Care Act (ACA), which aims to provide health insurance to a considerably larger portion of the population has continued to boost the healthcare industry revenue. The boost in healthcare revenue is forecast to increase at an average of 4.1 percent, reaching $204 billion in 2016 (1). According to the Centers for Disease control more than 29 million Americans are living with diabetes, and 86 million are living with prediabetes, a serious health condition that increases a person’s risk of type 2 diabetes and other chronic diseases. People with diabetes either don’t make enough insulin (type 1 diabetes) or can’t use insulin properly (type 2 diabetes). Insulin allows blood sugar (glucose) to enter cells, where it can be used for energy. When the body doesn’t have

Dr. Faulknor is exceptionally prepared to start her own practice, with co-location of Luminous Care LLC. Mental Health Provider. Dr. Faulknor has more than thirty (30) years’ experience in healthcare. She completed her Family Health Nurse Practitioner training in 2012 at the

University of Medicine and Dentistry (Now Rutgers University), is board certified as a Family Health Nurse Practitioner by the American Association of Nurse Practitioner and holds a

In addition, to clinical training and expertise as a Family Health Nurse Practitioner, Dr. Faulknor has extensive experience in program planning, program administration and implementation and served as Women, Infant and Children Coordinator and Director in New Jersey and New York for a total of more than 15 years. Dr. Faulknor is currently employed as an Advanced Practice Nurse (Family Health Nurse Practitioner ) with the County of Hudson, Meadowview Psychiatric Hospital, New Jersey.

Dr. Faulknor has identified an appropriate location for the private practice at 25 Sanford Street, East Orange, New Jersey, 07018, which is located to New Jersey Transit bus lines via Central Avenue and is in close proximity to the public commuter train lines (Brick-Church in East Orange. The property is on the market for sale at a cost of $260,000, is a mixed used building with more than 7700 square feet on the first (1st) floor that will be modified into three (3) examination rooms and allows for co-location with Luminous Cares LLC. a registered Psychiatric Nurse Practitioner Mental health care provider. In the future the plan is to use the second (2nd

floor) to train entry level healthcare providers such as certified home health aides, certified medication aides and certified nurse aides and CPR). Dr. Faulknor possesses the related State of New Jersey certification in these areas. Additionally, the third floor (3 rd) is currently rented for 850 monthly and the plan is to continue to rent the third floor space to provide financial support for some overhead costs. Sivam Diabetes, Nutrition & Family Health Care LLC. will employ one medical assistant and a part –time office manager during the start –up phase of the private practice, until demand reaches more than 24 clients per day. The private practice will offer primary family healthcare, MNT, DSME & S to individuals and families with private insurance, MEDICAID and MEDICARE. Medical record keeping and billing has been streamlined into the select electronic medical record (EMR) software and SDN & FHC) will seek to use EMR for the practice. Dr. Faulknor is computer literate, has used several EMR software programs which can be used to establish billing, practice management and record keeping systems that provides maximum efficiency and reimbursement cycle to optimize healthcare services.

Primary Family Healthcare is a healthcare/medical specialty devoted to the comprehensive health care of people of all ages. Family Health Nurse Practitioners are the principal group of advanced –practice nurses (APNs) delivering primary care in the United States. Gaps in the provision of quality health care is a major problem in U.S and in urban communities such as East Orange; combined with the aging the population. According to national statistics, the projected greatest demand for healthcare practitioners is in primary care. There are increased Medicare payments under the ACA reform for primary care providers in an effort to encourage these healthcare providers to stay in primary care. The rise in demand for primary care providers with the use of technology, and the effective use of medical assistants/health care assistants will be required to make a primary care family practice profitable such as Sivam Diabetes, Nutrition, & Family Health Care LLC. while providing optimal diagnostic and personal health care/management service to patients in East Orange and the surrounding target areas.

The East Orange, Essex County, New Jersey Behavioral Health Risk Factor Surveillance survey (EOBRFSS), indicated that Diabetes Mellitus is the fifth leading cause of death in Essex County which is the location of the East Orange Service Area. Rates among Black residents are significantly higher than for all county residents and are indicated as follows:

The total population for East Orange according to 2011 census data is 64,268; and 41, 216 of the total population is 25 years and older. 41.6% of the total population has poverty status, with 24,270 at 185% of the poverty rate and 26,130 at 200% of the poverty rate. Questions still exist on the cost of diabetes self-management education programs and who will pay for it. FHDC Inc. hopes to fill a portion of the gap by demonstrating that a community-based diabetes self-management education and support program can be cost effective and serve as a model program for people with Type 2 Diabetes Mellitus.

Target Market Analysis:

In East Orange , New Jersey, diabetes is a growing burden. East Orange has a total population of 64,268. Adults 18-64 make up 63% of the population and seniors age 65+ were 12%. A total of 48,715 East Orange residents are above age 18 with thirty-three percent (36%) of that population (16, 075) potentially eligible for DSME/T services. With increases in both the geriatric population and rates of obesity, the incidence of diabetes is also increasing. Diabetes Mellitus is the fifth (5) leading cause of death in Essex County; with blacks 4.1/100,000; Hispanics 106.8/100,000; and whites 167.8/100,000.

Diabetes can lead to serious complications and premature death, but people with diabetes can take steps to control the disease and lower the risk of complications. Type 2 diabetes, which accounts for 90-95% of all diabetes cases, is linked to obesity, lack of physical activity, unhealthy eating and smoking. Furthermore, diabetes is more common in ethnic minorities (African Americans and Latinos) and persons of lower socioeconomic status. (East Orange Community Health Needs Assessment, 2013). Seventeen percent (17%) of East Orange residents do not have a high school diploma.

Pertinent to the health status of East Orange is the socioeconomic condition. Poverty and lack of access to healthcare are two major factors. In East Orange, African Americans comprise 88.5% of the population and Latinos make up 7.9%. (Census data, 2011). In addition, The 2011 Census reported that 41.6% of all East Orange residents most of which are African Americans live in poverty with lack of access health care coverage.

In East Orange in 2011 the median household income was $34,599, which is $19,254 below the median income of Essex County (NJ) residents. Healthcare insurance coverage and diabetes management classes for the targeted groups are inadequate. The defined primary health service area for the DSME & S intervention will include the following zip codes 07017; 07018; 07109; 07050 and 07101 but not limited to these zip codes.

The majority of SDN & FHC LLC. DSME & S clientele will be primarily those who are Medicaid, Medicare, uninsured and/or underinsured and private paid clients. Most service providers in the same business such as hospitals and drug stores target clients who have insurance, can afford co-payments and are minimum payment risks. There are no competitors providing similar mix of services offered by s SDN & FHC LLC are the free

clinics, federally funded community health clinics and local health services within the service area but none currently provides DSME & S. T these services are already overburdened by the increasing number of patient and are potential sources of referrals for DSME/T services their programs and facilities. As SDN & FHC LLC. is able to reduce inpatient hospital stays and decrease emergency room visits, potential competitors such as “for profit” hospitals may in effect become supporters rather than competitors. The four major partners or “strategic alliances” for the successful implementation of this business plan to provide primary care, diabetes self- management education and training to the target population of East Orange and will serve as the enablers for AADE Recognized program for SDN & FHC LLC; enabling Medicare, Medicaid and other insurance reimbursement for eligible clients served. There is a recognized need to provide these services in the target area as currently there is no DSME& S Program in East Orange and this primary care provider of DSME& S program in a high density urban minority population and will facilitate increased access for people with diabetes to self-management education and training and primary health care. Opportunities to engage national, state and local collaboration and partnerships are included in the plan. At the national level, this will include the CDC, the American Medical Association and the American Association of Diabetes Educators (AADE). At the state level, Partnerships with offices within DHHS including Medicare, Medicaid and Minority Health will be sought. Other partners essential to the successful implementation of the plan at the local level are faith-based organizations that have sponsored and will continue to sponsor training for their members, physicians who serve as referral sources, and pharmaceutical

Dr. Mavis C. Faulknor, DNP, MSN, RD., CDE., APN-C, Family Nurse Practitioner/Nurse Educator , previous Director of the Nursing Program at Hudson County Community College and with an earned Doctorate in Nursing from the University of Buffalo, New York. Ms. Faulknor has devoted her career to improving the nutrition, health and education status of vulnerable, medically underserved and minority population groups. She was a member of the first class of Leadership Newark Fellows.

Ms. Faulknor has lectured and taught extensively in nutrition, health and nursing with an emphasis on reducing infant mortality and promoting access to nutrition for women, infants and children. She served as Women, Infants, and Children (WIC) Coordinator in urban communities in New York and New Jersey for more that fifteen years combined and as Health Director for the Leaguers Inc.

Grant Funding Applications Approved:Successful submission of many USDA discretionary grant requests/applications for funding to enhance WIC Program services to the residents of the City of Newark and the surrounding service area, these include:

Federal Fiscal Year (FFY) 1998- Obtained WIC Pediatric Health Mobile secondary to a “WIC Mobile on Wheels” United States Department of Agriculture (USDA) discretionary grant application request for funding. The mobile currently serves residents in all five (5) wards of the City of Newark and has contributed to an increase in the WIC Program caseload.

This project was highlighted in the media (TV) on “Positively Black” by Mr. Noble with recording by Ms. Pat Battle of News Channel 4.

FFY 2000 –Obtained USDA funding in the amount of $176,633 for the implementation of aggressive outreach activities in the city of Newark, WIC Program.

FFY 2002- Obtained USDA WIC Program discretionary funding to promote access to WIC Program benefits and services for the provision of non-traditional WIC Program service hours during weekends and extended service hours on weekdays.

FFY 2003- Obtained funding in the amount of $115.950 for “Extended Hours and Outreach” for the City of Newark, WIC Program.

FFY 2004 – Obtained additional funding for the City of Newark WIC Program to continue the provision of non-traditional service hours to promote access to WIC Program services by low income clients/applicants who work and attend school and experienced difficulty accessing WIC Program benefits and services during traditional service hours.

FFY 2007-Obtained USDA discretionary grant funding in the amount of $136,000.00 for “Obesity prevention and intervention initiative” in the City of Newark WIC Program.
