Corresponding environmental document or a categorical exclusion certification, as provided in the Rules of Environmental Documents of the Environmental Quality Board. 2. Digital polygon plan of the territorial expansion of the project geo-referenced to State Plane Coordinate System, showing the exact geographical location of the proposed action. 3. Compelling evidence that the proponent is the owner or lessee duly authorized by the owner of the property. 4. Names, contact information and certification of the licensed professionals involved in the proposed action, showing that they are duly licensed and / or registered in Puerto Rico, if any. 5. Names and contact information of all property owners abutting the property subject of the application, preferably including all e-mails. 6. Recommendation of any and all who apply between the Municipality and the involved agencies in Building, Infrastructure, Health and Safety, Environment, Usage, Archaeology and Historic Preservation, as required. 7. Information on the impacted area, the method and the proposed operational sequence for the duration of the permit, the reserve available for mining area and the equipment or machinery to be used, in addition to the description of the areas of storage, processing and distribution of terrestrial material, among others. Certification for the Deposit of Surplus Material, when the surplus material is deposited in an outside land lot (PDF) Cost The cost to file a simple extraction permit is $75. Deadline for filing The service request will remain open for a maximum of thirty (30) days for the applicant to complete the application. The following are requirements to apply for a Simple Permit for Terrestrial Material Extraction:

1. Corresponding environmental document or a categorical exclusion certification, as provided in the Rules of Environmental Documents of theEnvironmental Quality Board.

2. Digital polygon plan of the territorial expansion of the project geo-referenced to State Plane Coordinate System, showing the exact geographical locationof the proposed action.

3. Compelling evidence that the proponent is the owner or lessee duly authorized by the owner of the property.

4. Names, contact information and certification of the licensed professionals involved in the proposed action, showing that they are duly licensed and / orregistered in Puerto Rico, if any.

5. Names and contact information of all property owners abutting the property subject of the application, preferably including all e-mails.

6. Recommendation of any and all who apply between the Municipality and the involved agencies in Building, Infrastructure, Health and Safety,Environment, Usage, Archaeology and Historic Preservation, as required.
