Consultations are for a duration of 10 minutes. If you need a longer appointment, please advise theReceptionist at the time of booking. Clinics run every morning and afternoon 5 days a week atChurch Avenue and Burnt Oak Lane. Always arrive on time for your appointment. Please remembera separate appointment is necessary for each member of the family wishing to see a Doctor or Nurse.Children under 16 years should be accompanied by an adult.
If you are unable to keep your appointment, please telephone the Surgery as soon as possible toallow the appointment to be used for another patient.

The Reception Staff are available between 8.00 am and 6.30 pm to make your appointment bytelephone or in person. (The Main Surgery is closed between 1 pm and 2 pm and the Branch Surgeryis closed between 12.30 pm and 2 pm every day).

SEEING A DOCTOR
You can request an appointment with a Doctor of your choice but you may have to wait slightly longerif that General Practitioner is already fully booked. We also have 3 Nurse Practitioners who are able todeal with most problems that necessitate a visit to see a doctor. If you need to be seen that day for amedically urgent problem make this clear to the Reception Staff, but be prepared to see someoneother than your usual Doctor or Nurse Practitioner.

EMERGENCIES
Medical emergencies will be dealt with as soon as possible. Always use 020 8302 1114/020 8300 2747 numbers, 24 hours a day. If you telephone when the Surgery is closed you will beadvised how to access the most appropriate healthcare facility.The Telephone Number for NHS Direct is 0845 4647. The Web Site is www.nhsdirect.nhs.uk.
OUT OF HOURS DOCTORS SERVICE
This service is provided by Greenwich and Bexley Association of Doctors On Call (GRABADOC). Youmay be requested to attend the Out of hours Doctors’ clinic in Woolwich.
DENTAL PROBLEMS
Please note the Doctors are not allowed to treat dental problems. The Telephone Number for theemergency dental service is 020 8303 4077, 9 a m – 5 p m and 020 8316 7257 out of Hours
HOME VISITS
Home Visits are for the housebound or for those whose illness itself prevents them from attendingthe Surgery. Home Visits will be carried out only when the Doctor considers the medical conditionof the patient makes this necessary. Generally speaking, this would be when to travel to the Surgeryby car or taxi would endanger the patient’s health. Where possible, try to telephone between 8.30 –10.30 a m if you think you may need a Home Visit.Please remember that the Doctor can see 4 patients in the surgery in the time it takes to do oneHome Visit.
RECEPTION AND SECRETARIAL STAFF
We have a competent team of fully trained Reception Staff who are here to help you. They havea difficult job to do with telephone calls and enquiries from every direction. When telephoning formedical attention the Receptionist may ask for some details. They have been trained to make theseenquiries so that we can help you in the most appropriate way.
PRACTICE MANAGER
Mrs Fiona Bedus is the Practice Manager, and she may be able to help you with any administrativeor non-medical aspects of the Practice. She is also available to discuss any suggestions orcomplaints.
PRACTICE NURSES
Our practice nurses can give advice on many matters, including health promotion, childimmunisations, well man, well woman, family planning, cervical smears, coronary heart diseaseprevention and blood pressure.
DISTRICT NURSES
Our team of District Nurses are responsible for nursing care in the community. If you need to see aDistrict Nurse, please speak to reception who will convey your Visit request to the District NursingTeam.

Claire Hunter and Jackie Masters are the Health Visitors attached to our practice. They can becontacted at North Cray Childrens Centre, Bedens Road, Sidcup Kent DA14 5JQ and on telephonenumber 020 8308 3886.
REPEAT PRESCRIPTIONS
Patients on regular medication will require repeat prescriptions and for a safer and speedier service,repeat prescriptions are computerised. Your Doctor will have entered onto the computer whatmedication you are taking, and each time a new prescription is written on the computer a newrequest form is also generated for you to make your next request. The Electronic Prescription Serviceis being introduced across the country. This service gives you the option to ‘nominate’ the chemistyou choose to get your medication from so that they can receive your prescriptions electronically.Please speak to your local chemist to see if they have signed up for this service. Requests for repeatprescriptions are not taken over the telephone for medicolegal reasons, as errors can occur in verbalcommunication.
1 Allow 24-48 hours from requesting a prescription to it being available for collection(excluding weekends and bank and local holidays).
2 If you have more than 1 repeat prescription, please try and order all your items together.3 Tick which items you require4 You can collect your prescriptions from the Surgery or we can send it to your home
address, providing you enclose a stamped-addressed envelope.5 Please DO NOT telephone the Surgery for your prescription requests as it blocks the
switchboard and could potentially lead to mistakes being made.6 Please DO NOT add unauthorised prescriptions to your repeat slip.7 Either deliver your request to the Surgery and put it in the box provided or post it.
SICKNESS CERTIFICATES
You should not require a Doctor’s Certificate lasting 7 days or less. Your Employer may, however,require you to complete a Self Certificate (SC2) which is available from your Employer, the Post Officeor the DSS. For any illness lasting longer than 7 days you may need to see the Doctor for him/her toissue a Fitness for Work Certificate and sometimes for subsequent renewals. Should your Employerrequire a Doctor’s Note for a work absence or less than 7 calendar days, the Doctors may agree to dothis but a fee will be charged.
ACCESS FOR DISABLED
Disabled access is available at the Surgeries.
PRACTICE COMPLAINT PROCEDURE
If you have a complaint or concern about the service you have received from the Doctors or any otherStaff working in this Practice, please let us know. We operate a Complaints Procedure as part of theNHS System for dealing with complaints. Our Complaints System meets national criteria. Complaintsshould be addressed to the Practice Manager, Mrs Fiona Bedus. Alternatively, you may ask for anappointment to discuss your concerns.
CLINICS AND SERVICES
The following Clinics are held at the Practice by the Practice Nurses, Midwife, Health Visitors andDoctors.
FAMILY PLANNING - All forms of family planning are offered by the Doctors and Nurses.Dr Karkare fits IUCDs (coils).

CERVICAL SMEARS - are recommended at 3 – 5 yearly intervals between the age of 25 and 65years and the Surgery has a recall system. If you are due for a smear, please make an appointmentwith one of the Practice Nurses.
ANTENATAL - these are held on Thursdays between 9 a m and 6 pm at our Branch Surgery.
POSTNATAL - please make an appointment to see the Doctor for your post-natal check, togetherwith baby check.
CHILDHOOD IMMUNISATIONS. These are done by our Nurse. Please arrange an appointment withReception. Please remember to bring your child’s red book with you. The Nurse cannot givechildhood immunisations without it . Please do not bring children with infectious diseases to thisclinic, e g chickenpox, measles etc.
NEW PATIENT MEDICAL - On Registration with the Practice you will be offered an appointment fora basic medical review with the Practice Nurse.
MINOR SURGERY - this service is offered by the Practice. Please make an ordinary appointmentfor initial assessment with the Doctor.
CORONARY HEART DISEASE PREVENTION CLINIC - we offer this service for men and womenwho would like a general check-up, cholesterol testing, blood pressure and advice on lifestyle. Pleasemake an appointment with our Practice Nurse.
ASTHMA CLINIC – this is open to adults and children who use inhalers or who have been diagnosedwith asthma. Please make an appointment with our Practice Nurse.
DIABETIC CLINIC - we offer an annual diabetic check to all patients with diabetes. Appointmentscan also be made for monitoring of diabetes throughout the year with the Practice Nurse.
SMOKING CESSATION CLINIC - If you wish to give up smoking, please ask the Doctor or Nursewho will refer you to the Bexley Stop Smoking Clinic.
TRAVEL VACCINATIONS - if you are planning a holiday abroad, please check with us at least 6weeks before your departure date. Our Practice Nurses can advise you on your vaccinationrequirements and arrange suitable appointments for you. The initial assessment is a telephoneconsultation. Please speak to the Receptionist to arrange a telephone consultation with the Nurse.
WELL PERSON CLINIC - we offer this service to men and women who would like a general check-up, cholesterol testing if necessary, blood pressure testing and advice on lifestyle. We can also offeradvice on problems related to women’s health, e.g. menopause, contraception and pre-conception.Please make an appointment with one of the Practice Nurses.
VIOLENT OR ABUSIVE PATIENTS
We operate a “Zero Tolerance Policy”. If a patient is violent, abusive or rude to a Doctor, Staff orother persons on the Practice premises, he/she will be removed from our Practice List. In certaincases of violence the Police will be called.
ACCESS TO PATIENT INFORMATION
All our records are kept on computer together with the manual records and are kept safe and we canassure patients of complete confidentiality. Your rights are protected under the Data Protection Act1998. Patients may request access to their Medical Records under this Act. Please make a requestin writing to the Practice Manager

NON NHS MEDICAL SERVICES
Should you require a special examination or report for insurance, driving, employment or for otherreasons, this can be arranged through Reception. This does not form part of the routine NHS and acharge will be made. A list of these services and our fees are available from Reception.

This Practice is involved in teaching of Nurses for the Nurse Practitioner Degree.

HISTORY OF THE PRACTICE
Church Avenue
Sidcup Medical Centre has been on this site for many years. It was extensively refurbishedduring 1989 to provide a spacious and pleasant Surgery offering more facilities to patients.
Burnt Oak Lane
This building has been a Surgery since before the start of the NHS. In 1990 it wascompletely refurbished providing facilities on both floors. Further extension took place in1995.

If you live within the Practice area and would like to Register, please fill in the New PatientRegistration form available at Reception. The Practice area is shown on the map on thenext page. Please telephone the surgery after 3 working days to check whether you havebeen accepted by the Practice.Once registered, you will be asked to make an appointment to see the Practice Nurse for anew patient check.
