Hello and Welcome!Hello and Welcome! What are Short Breaks?Short Breaks provide opportunities for disabled children/young person to spend time away from their families, relax, have fun, develop new friendships, take part in new experiences and have fun doing positive activities. They also give families a break from their caring responsibilities, a chance to unwind, rest, or spend to time with their other children.

Short Breaks can vary between a few hours and a few days and may include day, evening, overnight and weekend activities. They can take place in the child’s own home, the home of an approved carer, or a residential or community setting.

Who can access Short Breaks? Eligibility criteria for disabled children are used to make sure that services are allocated fairly. Children and young people across Coventry can access Short Breaks if they:

• Have a disability. • Are aged 0-18 years. • Live in Coventry. • Are in receipt of low, middle or high rate Disability Living Allowance (DLA) or if they are on

a school action plus.• Have a completed Short Breaks Passport.

DLA provides help with the extra costs of bringing up a disabled child. It is paid on top of almost any other income you may have, and also gives you access to other kinds of help. If you are not currently in receipt of DLA and you feel you may be eligible to it, contact 0800 882 200.

How to access Short BreaksAll children and young people accessing the Short Breaks service must have a completed ‘Short Breaks Passport’. This will need to be completed and returned, along with a copy of your DLA/School Action Plus letter to: Broad Park Centre, Logan Road, Coventry, CV2 1AG.

What is a Short Breaks Passport? A Short Breaks Passport is a care plan for your child. It is a document in the form of a passport which holds key information about your child and their disability. Passports are kept at Broad Park Centre in a locked cabinet. Staff use these passports to gain information about the needs of your child when they are accessing the service. We also may use the passports to help inform our planning of future services and to see what support is best for your child.

Service delivery in Coventry Coventry currently provides Short Breaks to approximately 1,500 children and young people aged 0-18 years. This is 2% of Coventry’s child population. The service has grown year on year due to the success of the project.

Our vision in Coventry Our aim is to ensure that families with disabled children have the same human rights as others, including the right to the same quality of life as those who do not live with a disability. However, we recognise that all families are different, so they will need different levels of support and different types of short breaks therefore we ensure that there is a wide range of short break provision to meet those needs. Research demonstrates the value of early intervention and prevention, therefore we ensure that we provide on-going support, not just at points of crisis. We want young people to be healthy, stay safe, enjoy life, achieve success, make a positive contribution and benefit from economic wellbeing.

Short Break Provisionin Coventry

Short Breaks are offered on the basis of an assessed need. All Short Breaks are outcome focused to ensure young people get something out of accessing the service. The services below are offered at three levels that reflect the extent of the individuals needs:

Universal services

For children and young people receiving low, middle or high rate DLA or receive extra help at school such as school action plus. However, all disabled children should have access to most universal and mainstream services. Universal services are really important in ensuring disabled children and young people have fun and enjoy themselves with friends, just the same as other children and young people. Universal services aim to be inclusive and open to disabled children and young people wherever possible. Disabled children will not need an assessment to access these services but must meet the eligibility criteria stated on page 3.

Example of Universal Short Breaks include:Short Break activities, these include term time and school holiday activities. Please refer to page 11 to view the range of activities that are on offer.

Targeted services

Targeted short breaks enable disabled children and young people with severe or complex needs to get short breaks. These services can be accessed via a common assessment framework. Children can also access universal services.

Examples of Targeted Short Breaks include: • 1:1 Support: This may take place in the child’s own home or within the community. If there

is an assessed need, a support worker may take your child to the universal short break activities, with the outcome of them to then access the universal activities on their own.

• Directpayments:ADirectPaymentisthesumofmoneythatCoventryCityCouncilwillpaydirectly to the family to help them buy and arrange their own care and support. Direct Payments are part of the Government’s Personalisation agenda which puts the child central to the support they receive, helping them to live as independently as possible and giving them more choice and control. Specialist services

Specialist short breaks are for disabled children and families assessed as having the highest level of need. For children and young people who are assessed via a social worker as needing a specialist service from the Children’s Disability Team. They are provided to children and families with the most complex needs.

Example of Specialist Short Breaks include:• Overnightresidentialcare:Thisserviceassistschildrenwithhighsupportneedstosocialise

with others and develop some independence from their families. The quantity of overnight care will depend on the individual family’s level of need.

• Homecaresupportinthehome:Thismayinvolveanindividualworkerwhocomesintoyour home to support the family – this might be to assist with personal care or to help with domestic tasks. Some children and young people may need two support workers, depending on their individual level of need.

How do we decide who is eligible for specialist short breaks?The Short Breaks Panel takes place on the last Wednesday of every month in Coventry. It aims to ensure allocation of resources to service users and their families is done fairly and openly. The process follows that set out in the ‘Short Breaks Provision Matrix’ (please refer to page 10 for the matrix), recognising that those in the greatest need will receive the highest allocation of resources. The panel ensures that services are needs led, that services fit need, and that the provision is routinely monitored and reviewed. The panel involves professionals from the service to discuss new referrals for specialist short breaks and review children and young people’s current allocation of short breaks. Each month there are seven 15 minute slots. Social Workers and other professionals book a slot at the panel then they attend to present the case, a decision is then made by the panel.

What will happen after I have booked a Universal Short Break? School holiday short breaks:Universal short breaks can be booked during the times and dates as stated on page 7. When you have booked, allocations will be made taking into account your previous attendance and the currently level of support the family receives. A confirmation letter will then be sent to you in the post outlining what activities your child has been booked onto. If you cannot attend the sessions you have booked, please ensure you contact 02476786087 to enable us to offer the space to another child.

Term-time short breaks:Once the term-time activity is booked and you have submitted a Short Breaks Passport, your child can then attend if there are spaces. If your child is booked on a weekly term time activity, please ensure you contact 024 7678 6087 if they are unable to attend each week to enable us to ensure we have appropriate staffing in place.

Keeping up-to-date with Short BreaksEmailIf you would like to be kept up-to-date with information about what we have on offer on a regular basis via email, please email [email protected]

Text messagingA suggestion was made by parents/carers to have a regular update about services via text message. Short Breaks Youth Club (Every Monday) A place for young people to come and chill out with new friends. The group aims to give young people independence, a chance to learn new skills and have fun doing positive activities. The group will involve lots of different activities depending on what the young people would like to do, i.e cooking, watching films, playing games etc. Drinks and toast will be provided.

Inclusive Football & Multi sports (Every Tuesday) Aims to encourage children to socialise and make new friends whilst improving their motor skills including balance, agility, coordination and fitness. The group will take part in a wide variety of activities every week depending on what they would like to do.

Martial Arts (Every Wednesday) This group aims to improve the body and mind. It teaches children and YP how to focus, be disciplined, concentrate, improve confidence, set goals and build their self esteem. It also gives them an opportunity to learn new skills, express themselves and meet new friends.

Sensory Fun & Messy Play (Every Thursday) A stimulating session involving messy play, sensory toys and relaxing music. Children can look, listen, touch, make noise and smell using a range of interesting materials. A time for you and your child to enjoy getting messy and artistic.

Short Break Saturday Club (Every Saturday) A place for young people to relax, unwind, learn new skills and make new friends. The group will involve games, arts & crafts, cooking, computer games, sensory toys, music, outdoor games and most importantly children will have lots of fun! Drinks and snacks will be provided. Family Disco (1st Saturday of every month) A place for families to come along, have fun together, meet new friends and have a dance. The disco will take place in the bar area. Free soft drinks and snacks will be available.

Unfortunately we are unable to provide 1:1 support for children and young people on the day of activities, unless this has already been agreed. Therefore if your child requires 1:1 support, please ensure this is arranged with your social worker or lead professional. If this is not arranged, a parent/carer will need to stay with the child at the activity.

CHILDREN UNDER 5 YEARS OLD - All children under 5 years must be accompanied by an adultat all times during all Short Break activities.

Age appropriate activities For activities that are advertised for ages 5-18 years, where possible, we will split the groups into age groups to ensure the children and young people are socialising with others of a similar age group.

Broad Park Centre

Broad Park Centre is Coventry’s integrated Children’s Centre for children and young people with disabilities. It is purpose built and fully accessible.

We have an extensive range of play/leisure equipment and we organise activities and projects which promote the personal development and life skills of our service users. Many of the term-time and school holiday Short Breaks take place here.

Broad Park Centre also combines a purpose-built residential Short Breaks unit (known as Broad Park House) for children aged 5 to 17 years with learning and/or physical disabilities. The home is registered by Ofsted. A recent Ofsted inspection praised the involvement of children and young people with disabilities in the decision making and design process of the rebuild.

If you would like to arrange a time to come and view Broad Park Centre, this can be done by contacting Michelle Roberts on 024 7678 6078. If you are travelling by bus, you will need to catch the number 10 bus from Coventry city centre.

Meet the TeamAll Short Break support workers are trained in working with disabled children and continue to access training through our workforce development programme. Mandatory training includes: personal care, first aid, risk assessment, safeguarding, manual handling, epi pen, safeguarding children, autism awareness and medication. All staff have undergone a Criminal Records Bureau check.

Hi my name is Andy; I am currently working in a school and looking forward to working with Short Breaks in the evenings and weekends. I am a very sporty person and enjoy a challenge. In my spare time I enjoy watching and playing football, taking part in swimming competitions and seeing friends.

Hello my name is David, I am a drama graduate. I love going to the cinema, I am a movie and TV nerd and proud of it. I enjoy listening to and playing music, I dabble on the guitar, piano and drums. I enjoy most kinds of sports, playing and watching except cricket, cricket’s a little too boring for me. Apart from going to the cinema and watching bands in my free time I also enjoy writing plays and getting involved with friends to make work for fun.

I am 23 years old and a nursery nurse currently studying my NVQ level 3. I enjoy going out with my friends shopping and going on holidays. I love zumba!

Hello, my name is Laura, I am currently in my second year of university studying to be a Primary School teacher. I enjoy going out for dinner, the gym and catching up with friends. My favourite Short Breaks activity was definitely the Ice-Cream tasting during the Summer ‘12 despite being forced to try lots of horrible flavours, the delicious ones certainly made up for it. I look forward to seeing you all at Short Breaks!

Lynsey Williamson (Support Worker)

Hi everyone! My name is Lynsey and I have recently completed a level 3 BTEC Extended Diploma in health and social care, my dream is to become a Social Worker. I enjoy socialising with friends, I laugh a lot, I like to read and I love meeting new people. I can also juggle as when I was little I did want to become a clown!! But I think a social worker is wiser decision! I dislike bad manners and mayonnaise… yuk!

Hi my name is Molly and I have recently completed a Btec in health & social care level 3. I am also currently a part-time waitress and I volunteer working with adults who have learning disabilities. I like the seaside, shopping and cooking (except when it comes to cleaning it up!). I dislike sprouts, the rain and cleaning my room. In my spare time I enjoy eating out with friends - any excuse really!

Mohammed Patel (Support Worker)

Hi my name is Mohammed; I currently work at Hereward College supporting young people with special needs. In my spare time I enjoy sports such as football, however, I am a long suffering Sky Blues fan. I also enjoy travelling and have been fortunate to have visited some amazing countries.

Hi my name is Pio I am an Honours graduate with a support work background, having worked with both children and adults who have differing levels of learning needs and disabilities. I have experience supporting a wide range of young people as a youth support worker and as a personal advisor in youth related organisations. Hi my name is Sam, I have been working with Short Breaks for over two years now and love every minute of it! I also work full time as an Allocation Coordinator. I love being involved in different activities and meeting new people suits my personality as I am a bubbly person. In my spare time I enjoy spending quality time with my family, going to the park and days out.

Sharon Gudgeon (Support Worker)

Hi my name is Sharon and I am a student social worker. I mainly work for Short Breaks during the school holidays. I also work in a primary school and a community care agency. I love music (I try very hard to sing well), seeing my friends, eating and laughing. I dislike baked beans and being too hot or cold!

Hi there. I love working with Short Breaks and the experience I’ve had has been fantastic. I also teach at a college and I value young people as a start for a new future. I enjoy being creative and going crazy with new ideas to create a media product. I love attending gigs, computer gaming and films (watching and making) are my big favourite past times. I also enjoy art galleries and cooking.

Disabled Blue Badge SchemeThe Blue Badge scheme is a European scheme which gives parking concessions in the UK and Europe to people with severe mobility problems. It allows badge holders to park closer than other members of the public to their destinations. If you would like more information about this, please contact 0500 834 333.

Independent Travel TrainingTrain people how to travel safely around the city, promoting their independence. They work in partnership with individuals to meet their goals and outcomes.• Offering training in the practical skills required to travel independently.• Assessing and managing risk by teaching safe practice, personal safety and road safety.• Agreeing routes that will be specific and tailored to meet the individual’s needs.• Supporting people to develop their skills and confidence.• Working at the pace of the individual. One Voice ParentsOne Voice is a group of parents who have come together to help shape services for disabled children in Coventry. We are not associated with any particular disability, group or organisation and any parent is welcome to join us. We know how difficult it is for parents of disabled children to get involved but even if you can spare just a couple of hours a month you would be very welcome.

The aims of the group are to:• Raise awareness of the impact of a wide range of disabilities• Influence decision makers• Provide views based on personal experiences• Share information and provide support to other parents and carers

Children’s CentresChildren’s Centres act as a ‘service hub’ within the community, offering not only a base for childminder networks, but also a link to other day care, out-of-school clubs, and extended schools. Centres may also offer other services such as: • training for parents (e.g. parenting classes, basic skills, English as an additional language) • benefits advice • childcare services for older children and; • and toy libraries. Please check with individual centres about what they have on offer. Most centres change their timetables and courses termly. Visit your local Children’s Centre! Family Information ServiceThe Family Information Service can help with a range of information, advice and support for families with children and young people. The Family Information Service is totally free, impartial and confidential. VIBES Vibes is a children and young people’s service provided by Coventry & Warwickshire Mind to meet the emotional and mental health needs of children and young people between the ages of 7-19. Services include short courses which help young people explore issues affecting their behaviour, confidence, self-esteem and emotional resilience, a one to one buddy service and support groups for young people with Autistic Spectrum Disorder. Coventry Parent Partnership ServiceCoventry Parent Partnership Service offers information, support and advice to the parents and carers of children with special educational needs. The service provides accurate and impartial information on the full range of options available for the child’s education. They explain parents and carers rights and responsibilities and help them make informed decisions about their child’s education.

Domestic Violence Helpline The helpline provides support and information to women experiencing (or who have experienced) domestic abuse and to those seeking help on a women’s behalf. Helpline staff will confidentially discuss the available options on 0808 2000 247.

Changing Places toilet for city centre Coventry city centre will soon have its first Changing Places toilet after the Council helped to fund and build a much needed facility in Central Library. Work is now virtually complete on the new facility and it is likely to be open in mid-April 2013.Changing Places toilets have plenty of space and include a changing bench, a hoist and other specialist equipment for people with disabilities. A nationwide campaign has been running to raise awareness of the need for such facilities so that disabled people and their careers can have the same opportunities to take part in the shopping experience that everyone takes for granted.Coventry City Council and Coventry Mencap have been working closely on the project. Jill Hindi, from Coventry Mencap said it will mean that hundreds of people will be able to spend more quality time in the city centre.
