Acknowledgment and acceptance of Terms of Use These Terms of Use govern your use of the Black Dog Institute ‘Shift app’ via your mobile device. Shift is owned and operated by Black Dog Institute, ABN 12 115 954 197. Access to and use of Shift is subject to your acceptance of these Terms of Use.

By downloading Shift you accept and consent to, without limitation or qualification, all of the Shift Terms of Use and the Black Dog Institute Privacy Policy (available at: https://www.blackdoginstitute.org.au/privacy-policy). Before downloading Shift, please review these Terms of Use and our Privacy Policy carefully.

Black Dog Institute reserves the right to amend these Terms of Use or Shift, however any updates will be presented to you via the relevant app store should you opt to install any updates to Shift on your device. You will be deemed to have accepted and agreed to the revised Terms of Use if you opt to install app updates to your device.

1. What does Shift offer? Shift is a free, easy-to-use app designed to help Junior Medical Officers (JMOs) working in NSW to monitor, improve and maintain their mental health and wellbeing while managing the demands of their profession.

Developed in collaboration with JMOs and leading clinicians, Shift delivers essential mental health and wellbeing skills, therapy and help seeking direction that is accessible, easy to use and personalised.

1.1 What health conditions does Shift propose to help people with? Shift primarily seeks to address symptoms of depression and general anxiety by delivering a variety of mental health related content for building and maintaining good mental health and wellbeing.

1.2 What kind of assistance does Shift offer? Shift delivers psychoeducational content and strategies contextualised for the specific needs of JMOs. The app uses principles of cognitive behavioural therapy (CBT), acceptance and commitment therapy (ACT), mindfulness, and stress-management. The app activities cover topics organised under the broader terms of mental health, getting help, lifestyle and work with an additional free-standing module in response to the COVID-

19 pandemic. Other components of the Shift app include a tracking function for monitoring mood, physical activity, work-life balance, and sleep.

1.3 Who is the target audience? Shift has been designed for JMOs working in the NSW health system. It is relevant for Medical Students, Interns, Resident Medical Officers, Registrars and Trainees.

1.4 How is Shift delivered? Shift is a smartphone application that delivers content largely on an informational basis. Content is presented via video, audio, graphical representation, and text displays.

1.5 Are health care professionals involved in delivering Shift? Shift is available as a self-guided tool. Health professionals are not involved in the delivery of Shift.

1.6 How much does it cost? Shift is free to download and use but does require a working internet connection and a small amount of mobile data for app download, login, and the recording of app usage.

1.7 Who is Shift accessible to? To access Shift, users will need:

• A smartphone (iOS 11+ or Android 6+), however Shift may work on older devices as well.

• A stable internet connection or data for downloading and using the app.

Shift has been developed specifically for JMOs working in NSW. Some content may not apply to JMOs in other Australian States and Territories.

Shift is not optimised for tablet devices and will only present as a mobile phone sized display.

2. Privacy collection notice: Who will have access to user data?

2.1 What information is collected about Shift users? The purpose of collecting information (data) about our users is to help us monitor how well the app is engaging people, to help us make decisions about what parts of the app we can improve to ensure it remains of the highest quality, and so our users can receive the full benefits of using the app. Only de-identified, aggregated data is used for these purposes. User data is collected when the user accesses the app and is online (i.e. connected to the internet).

There are three main ways we collect user information:

1. At registration and download of the app

Depending on which app store (either the Apple App Store or Google Play) the user downloaded Shift from, some de-identified information is collected by those stores such as when users install and delete the app (date and time), device model, device operating system and country. Additional device-specific information is captured during crash reports if users opt in to send ‘usage and diagnostics data’, such as mobile provider, battery level and phone storage available.

The following data is collected via our servers so users can receive the full benefits of the app.

• Demographic information including gender, birth year, state and career stage.

• User responses to several validated mental health screening tools including the PHQ2 - 9, GAD2 - 7, COVID, and support seeking

2. While using the app

The following data is collected by our servers to support user interactions with the app:

• My Tracking: My Tracking is a feature that allows users to track their mental health changes over time. The information collected via this assessment is based on the user's personal, subjective assessment of their mental health (mood, sleep, work/life balance and physical activity).

• Activities (Topics): Activity start and end times are collected as well as responses to questionnaires applied in certain activities; PHQ 2 (‘Depression’ activity), GAD 2 (‘Anxiety’ activity), IAS (‘Health Anxiety’), PCL5 (‘Posttraumatic Stress Disorder (PTSD)’), AUDITC (‘Alcohol and Drugs’), Exercise (‘Exercise’), the other activity inputs are not collected.

Users are required to provide inputs to several Shift activities, including: Your Values, Worry Decision Tree, Unhelpful Thoughts, Evaluating Thoughts, Exercise, Gratitude, Wellbeing Plan, Alcohol and Drugs, PTSD, Health Anxiety, Anxiety and Depression however these inputs are not collected by our servers and only remain within your phone operating system.

• User feedback survey: A notification for this survey is actioned @ 30-days after app download. This survey provides helpful Insights Into how we can continue to ensure the app is engaging and worthwhile for our users.

3. Information for research purposes

Shift is currently available under a Human Research Ethics Committee (HREC) approval that allows us to collect certain user data once downloaded. Users may however opt out of this data collection component and still use the app. Where a user selects to provide their data, their data is never identifiable but may be analysed in the future in an aggregated way to help interpret the effectiveness of the app. It may also be used to help inform future improvements of the app and its ongoing safety and quality. This particular data is collected via two mental health related questionnaires at registration – the PHQ-9 and the GAD-7. These same questionnaires are made available from within the app 30 days after first downloading the app.

2.2 Who is that information shared with? We do not routinely disclose personal information (data) with external parties and at no time will Black Dog Institute share a user’s personal information without their consent unless:

a) We believe there is a serious and imminent threat to their or someone else’s life, health or safety.

b) We need to investigate or report on activity which we believe on reasonable grounds to be unlawful.

c) Disclosure is required or authorised by law (for example where a regulatory authority has the power to request the provision of certain records or information).

d) Disclosure is reasonably necessary to enable an enforcement body to perform its functions, for example:

i. The prevention, detection, investigation, prosecution or punishment of criminal offences.

ii. The preparation for, or conduct of, proceedings before any court or tribunal, or implementation of the orders of a court or tribunal.

Black Dog Institute may disclose de-identified data to trusted third parties engaged to assist us with services including data processing, data analysis, information technology services and support, website maintenance/development, record archiving and medical consultants who provide expert advice on the program’s clinical content.

De-identified means we will not share any data that links a user’s personal details (i.e., such as name or email address).

Any research undertaken by Black Dog Institute using Shift data must be approved by an external Human Research Ethics Committee. All research is undertaken in accordance with the terms and conditions specified by the relevant Human Research Ethics Committee, and the principles specified in the National Health and Medical Research Council and the Australian Research Council’s Australian Code for the Responsible Conduct of Research 2007 (the Australian Code). Research participants enrolled in trials or research will be given further information detailing how their personal information (including health information) will be handled by Black Dog Institute, including circumstances where personal information may be disclosed. At no point in time will your data be identifiable when used for research purposes.

2.3 What is the reason for sharing a Shift user’s information with others? Black Dog Institute may disclose a user’s de-identified data to trusted third parties engaged to assist us to provide the Shift program. Sharing of any de-identified data assists is to:

a) Undertake research. Shift is currently available under a Human Research Ethics Committee (HREC) approval that allows us to collect certain user data once downloaded. Users may however opt out of this data collection component and still use the app. Where a user selects to provide their data, their data is never identifiable but may be analysed in the future in an aggregated way to help interpret the effectiveness of the app. It may also be used to help inform future improvements of the app and its ongoing safety and quality. Please see Section 2.1 for further information.

b) Carry out our Safety and Quality Assurance processes (i.e. to help us maintain the program and make it better).

c) Inform the development of Shift marketing and promotion strategies.

2.4 Where is user information stored? Any information we collect is stored on secure servers that are protected in controlled facilities.

The security of your personal information is important to us. We use Government Data Centres (GovDC) commissioned by the University of New South Wales (UNSW) for data storage and backups. The data remains controlled by us. UNSW IT follows industry standards to protect personal information and uses secure socket layer (SSL) technology and key based encryption techniques. Any data stored on your phone as a result of using Shift is protected by the Advanced Encryption Standards (AES) algorithms for additional security. However, the internet and any method of electronic storage is not 100% secure.

Therefore, while we strive to use commercially acceptable means to protect your personal information, we cannot guarantee its absolute security. We also encourage you to take responsibility for the security of your own computer system.

Furthermore, our employees and the contractors who provide services related to our information systems are obliged to respect the confidentiality of any personal information held by us. However, we will not be held responsible for events arising from unauthorised access to your personal information.

2.5 Can a Shift user review and/or delete data that has been collected about them? Yes. Users simply need to contact the Shift team with a request to withdraw and delete their data.

2.6 Where is the Black Dog Institute data-sharing (privacy) policy? More detailed information about the way Black Dog Institute uses, discloses and otherwise handles personal information is set out in our comprehensive Privacy Policy at https://www.blackdoginstitute.org.au/privacy-policy

Our Privacy Policy includes information about how individuals can opt-out of receiving marketing communications from us, access their personal information and/or seek correction of their personal Information. Our Privacy Policy also includes information about how individuals may make a complaint about how their personal information has been handled.

2.7 What security measures are used when collecting and storing user data? We take reasonable steps to prevent the personal information we hold about you from misuse, interference or loss, and from unauthorised access, modification or disclosure. Our security measures meet industry standards and you can feel confident in our processes. These may include access control procedures, network firewalls, encryption

and physical security. Although we will endeavour to protect your personal information, we are unable to guarantee that any information you transmit to us over the internet is 100% secure.

In terms of personal protection, users are responsible for the personal protection of the device in which Shift is stored.

3. What is the likelihood of benefit or harm if used as recommended or otherwise?

3.1 Is there any independent scientific evidence of benefit from using Shift? The Shift app's conceptual design was modelled on Black Dog Institute's HeadGear app that produced significant improvements in symptoms of depression, anxiety and stress in work and social functioning.

An acceptability and feasibility evaluation has been conducted on Shift in 2020 involving over 200 JMOs. An element of this evaluation involved participants completing several validated mental health assessments prior to using the app and again at one-month after downloading the app. Positively, results have indicated reductions in depressive symptoms and full results are expected for release in 2021.

3.2 Is Shift endorsed by a government or professional body? Shift is promoted via several credible, government and non-government bodies and services such as:

• The NSW Ministry of Health

• Shift was identified for development via the NSW Health JMO Wellbeing & Support Plan (November 2017)

3.3 Are there any health precautions or safety warnings? • Shift should not replace the advice of a doctor or mental health professional. If you’re

having thoughts and feelings that are getting on top of you, please reach out to your

GP or a mental health professional. If you’d like to talk to someone now, please call Lifeline on 13 11 14.

• Exposure to mental health related topics may cause psychological distress and/or discomfort. It is possible that some participants may find the contents challenging and/or stressful.

3.4 Have there been any adverse health events or consumer concerns reported about Shift? No, Black Dog Institute is not aware of any reports of adverse health events or consumer concerns about the Shift app.

4. Is Shift easy to use and are users likely to keep using it?

4.1 Is there any independent research on how easy it is to use Shift? Black Dog Institute conducted a pilot trial involving 22 JMOs to assess usability and acceptability of the initial prototype. Results were positive in relation to the overall usability. Learnings from the initial prototype evaluation were incorporated into the current version of Shift that was subsequently tested by over 200 JMOs in 2020. Positively, results have indicated reductions in depressive symptoms and a high sense of user satisfaction. Full results are expected for release in 2021.

4.2 Were people from the target audience involved in developing Shift? A four-phased iterative development process was undertaken to create the contents and features of Shift. Each phase involved JMOs as follows:

1. Interviews with 12 JMOs to assess needs.

2. Prototype development with user experience feedback from two JMOs.

3. Pilot trial with 22 JMOs to assess usability and acceptability of the initial prototype.

4. Redesign, including user experience workshops with 51 JMOs.

An evaluation study has subsequently been conducted on the current version of Shift involving over 200 JMOs. Black Dog Institute is in the process of analysing the data relating to this subsequent study are expected for release in 2021.

4.3 How much time will it take an individual to use Shift as suggested? Shift has been designed in a way that allows users to select how much they would like to interact with the app, meaning users can select how many activities they would like to complete each week. Each of the app’s activities will take around two to five minutes to complete and it is recommended that users complete between three and five activities per week.

4.4 Does Shift fit in with other health services? Shift and Black Dog Institute’s Health Professional Training Workshops

Shift is promoted via Black Dog Institute’s Health Professional Education and Training program as a recommended mental health and wellbeing app for trainee doctors in NSW.

4.5 What do other users think of Shift? As indicated in Section 3.1 and 4.2, a significant number of JMOs have been involved in the development and testing of the Shift app. It can be reported that feedback and results are positive and are expected for release in 2021.

5. How is Shift funded and what is the business model?

5.1 Who is offering Shift? Black Dog Institute is offering this service.

5.2 What is Black Dog Institute's business model? Black Dog Institute is an independent not-for-profit organisation located within the Prince of Wales Hospital Campus and affiliated with UNSW Sydney, Australia (the University of New South Wales).

5.3 Was a health professional involved in developing Shift? Several leading health professionals and experts in the digital mental health space were involved in the design, development and testing of the Shift program. They include:

5.5 How does Shift make money? Shift is available at no cost to consumers and does not currently produce any income.
