• Outcome based assessment – what an individual can do as well as what they need support with

• Individual owns their support plan– Support planning options include help from, friends,

family, care manager and independent brokerage services.

• Care managers will work with the individual to agree their plan, ensuring that what is being proposed is safe and meets their outcomes.

• Individual can choose how they want to manage their personal budget- Direct Payment- Care commissioned by HCC- Combination of both of the above

• Individual works with Care Manager to review how well their support plan is working and to identify any necessary changes

• The implementation of Self-directed Support throughout Hampshire as the basis of our service delivery for persons with eligible needs.

• To embed SDS as part of the wider personalisation programme, and ensure that its processes and philosophy are built in to the total change.

• SDS has now been implemented across Hampshire county within the Older Persons, Physical Disabilities and Learning Disabilities care management teams

• Since April 2010, 1705 (313 LD) people have received an SDS Assessment, with 866 (54 LD) now living their lives with their own support plan in place.
