The electric stimulation dates back to far ages and several have been the means used
throughout history to achieve it: minerals, fish able to generate an electric discharge and,
more recently, electric stimulators that, in time and thank to technology advances, have been
subjected to a substantial and fundamental evolution.
Nowadays, there are lots of experimental data that emphasize its great properties in chemical
and biological fields. It is widespread in several medical specializations such as Rehabilitation
Medicine, Orthopedics, Neurology, Urology, Gynecology and Pain Management.
The basis of modern electrostimulation were introduced at the beginning of ‘900 from Weiss
and then Lapique, who developed theoretical elaborations that lead to the theory he set out.
In this way, it was demonstrated the relationship between intensity and application time of
an electric current in order to achieve a muscle stimulation (curve intensity-time), defining
the parameters that characterize the excitability of nervous and muscular tissues: rheobase,
that is an intensity constant and, above all, the chronaxie, that defines the electric pulse
duration.
However, many other factors arise to make optimal the pulse and it is already widely accepted
that the optimal electric stimulus must have particular features so summarized:
- it has to respect the chronaxie of the structure to stimulate;
- and it must have a perfectly balanced biphasic rectangular shape.
The rectangular shape has to be the most pure possible. The symmetry of the rectangles
must be correct, so that it is
achieved an electrical average equals to zero and it is avoided any risk of ionization (burns,
etc.).
This is important because the electric pulse is the mean of the Muscle Electrostimulation
(ESM) and, therefore, from its quality depends that of the ESM.
Hence, it is important to have the best electric pulse in order to have the best ESM.
To make it possible, the electric pulse has to be produced by a constant oscillator and not by
a voltage generator.
The oscillator is able to keep constant the features of the pulse required (stimulation
condition) at the peripheral motorial innervation that is transmitted through the motor end
plate to the muscle fibers making them flutter.
Any time that the pulse performs again, all these events are repeated.
Another important feature for the electrostimulation is the frequency, which is the number
of pulses delivered in a second (Hertz).
If the pulses are quickly repeated in the unit time, it is not possible to identify the single
contractions anymore, but they combine to reach a tetanic contraction (temporary sum of
the contractions).
More the phenomenon is highlighted, stronger is the strength developed and elevated is the
work for unit of time, but is even quicker the develop of the muscle fatigue.

What is MFT?
There are three types of muscle fibers and each one has a range of optimal work frequencies (temporary collecting of motor unit):
• Type I slow fibers
• Type IIa intermediate fibers
• Type IIb fast fibers
The tetanic contraction for the slow fibers starts with 10 Hz and is maximum at 35 Hz; for the intermediate fibers it starts from 20 Hz to maximum around 50 Hz; for the fast fibers it starts at 30 Hz and the highest is 70-75 Hz.
At more elevated frequencies, between 90 and 120 Hz, there is an increase of the speed in the strength development that occurs due to Explosive - Reactive - Ballistic movements and it is indicated as explosive strength.
The above-mentioned frequencies subdivision has an indicative value and there are strong differences among the patients above all if high-level athletes are considered.
Only with specific tests, it is possible to identify the peculiar muscle features of the patient and optimize the work through a personalized program.
As it is not always possible to do similar tests, it has been introduced the concept of Multi Frequency Training (MFT) with programs that use a frequency scanning in the range of specific myoneural properties.
The MFT programs go beyond the above-mentioned concept, considering that it includes other fundamental parameters to carry out a great work:
• rise time of the physiologic electric pulse;
• length of the work frequencies and recovery according to the target to be reached;

• modulability of the workload in line with the patient’s muscle condition.
Therefore, with focused programs, MFT system may be proposed in medical and rehabilitation field, in the sport and fitness one and in aesthetic medicine.
