Our Get Benefit Relief (GBR) program offers discounts in all areas of healthcare, from dental and vision to doctors by phone. If you already have insurance, GBR is an excellent supplement to your existing coverage, saving you money on your deductibles and copays and saving you time away from work. With plans starting at just $5.95 a month for the entire family, there is room in any budget for the convenience and money-saving benefits you will find with GBR. Whether you are an individual, a parent or an employer, we have plans that will fill the gaps in your existing coverage. If you are uninsured, we provide affordable access to quality medical care for your entire family at just $12.95 per month!

GBR Dental - $5.95 per family/per month In most instances, members receive discounts of 15% to 50% per visit* on dental services at over 132,000** available dental practice locations nationwide. Members simply show their card with the Aetna Dental Access® logo and pay the discounted price at the time of service. Members can use their card and enjoy the savings time and time again.

The CTC provider network includes ophthalmologists, optometrists, independent optical centers and national chain locations. Highlights Save 20% to 60% on prescription eyewear. Most frames, lenses and specialty items such as tints, coatings and UV protection are available. Save 10% to 20% on contact lenses (excluding disposables) at participating retail locations. Prescription Savings Card The RxCut® prescription savings program offers a full network of retail pharmacies nationwide designed to lower your out-of pocket prescription drug costs and increase your savings.

Dental Care Save 15% to 50%* on general dentistry, X-rays, fillings, and cleanings, plus root canals, crowns, orthodontia and periodontics at over 132,000** locations. *Actual costs and savings vary by provider, service and geographical area. (** As of June 2013.)

Our most popular plan, GBR Plus gives members everything included in GBR Select, (dental, vision, prescription discount card) as well as the services listed below.

TeleHealth (Doctors by Phone) - A national network of board certified physicians providing consultations via telephone or online, with no consultation fee, 24-hours a day. Physicians discuss symptoms, recommend treatment options, diagnose many common conditions, prescribe and phone in medications to your local pharmacy when appropriate. Doctors by Phone services save time and money by avoiding unnecessary doctor's office or ER visits. Provided by Teladoc.

Health Advocate

Medical Bill Saver™- Benefit can lower out-of-pocket costs on medical bills not covered by insurance. Advocates will work with healthcare providers and attempt to lower the balance on any uncovered medical or dental bill over $400.

Prescription Savings Card Whether you are insured, uninsured or underinsured, our Prescription Savings card can save you money on your prescriptions. Below are 2 examples:

• You are uninsured, and are diagnosed with bronchitis. Your doctor prescribes Azithromycin Tablets, 250 mg. The retail price for 6

pills is $35.00. You visit and enter your zip code and Azithromycin Tablets, 250 mg. You find that the lowest price in your area is $19.08* at the Target Pharmacy. By presenting your Prescription Savings Card to the pharmacist, you will receive a savings of $15.92- and pay only $19.08 for a prescription that would have cost you $35!

• You are insured with a $20 copay for generic prescriptions. You see your doctor and learn that you have high cholesterol. He
