Sample Test Patient Care Plan CPESN Pharmacies

1. Log into PrescribeWellness.com

2. Search for the test patient by entering “sample” or “test” in the search field and select a patient.

3. Select the patient.

4. Click on PrescribeCare to start an eCare plan for this patient.

5. Click on Add Encounter on the right hand side of the screen and select eCare Plan in the dropdown list.

6. The name of the eCare plan can be edited so the pharmacy staff can easily determine the reason for the intervention. This is not a requirement but is often helpful for staff. In this case I named it “Sample eCare Plan 05/15/2019”.

7. Enter the encounter details as well as the Encounter Reason (Synchronization of Repeat Medication).

8. Insurance is a required tab with PrescribeWellness. In this case you can select the test insurance listed for this patient.

9. In order to add the medications in question to the eCare plan click on Import Medications or Add Drug if the medication is not on the profile of the patient.

10. Click on Edit to add an MTP to a medication.

11. Click on the MTP (Medication Therapy Problem) tab to enter the MTP for the non-compliance identified with the Lisinopril for this patient.

15. Add MTP and/or Intervention Note under Recommendations:

16. Click the orange Add Goal button

17. Search for an appropriate patient goal description within each goal category.

18. Once goals are entered the eCare plan is ready to be completed. Click on the Complete button

below.

19. Once you Agree & Complete the encounter it will be sent to CPESN USA and your eCare plan has been submitted.

20. If you need to edit part of the encounter you can Re-Open the claim by clicking on the orange Re-Open button below.
