Unrestricted Gift or Bequest I give, devise and bequeath to the Lung Cancer Initiative of North Carolina (FEIN- 26-2300885), or its successor organization, the sum of ________________ for uses and purposes of the Lung Cancer Initiative of NC as its Board of Directors may determine. Bequest of Residue I give, devise and bequeath to Lung Cancer Initiative of North Carolina (FEIN- 26-2300885), or its successor organization, the residue (or share of residue) of my estate for the uses and purposes the Lung Cancer Initiative of North Carolina and/or Board of Directors may determine. (Alternatively, the residue may be directed to any specific purpose). Bequest for a Specific Purpose I give, devise and bequeath to Lung Cancer Initiative of North Carolina(FEIN- 26-2300885), or its successor organization, the sum of ________________ to be used for _______________________________ (e.g., equipment, grants, etc).
