Health Informa!on Department of Universi! Kebangsaan
Malaysia Medical Centre in its ac!vity plans for 2016 has
proposed to conduct a seminar on Clinical Documenta!on
and Coding (CDC). The aim of this seminar is to enhance the
importance and value of complete documenta!on and coding
to the par!cipants. The target par!cipants are public health
prac!!oners, clinicians, medical prac!!oners, hospital
administrators, medical records and nursing staffs and anyone
responsible for handling records in a healthcare se'ng.
Documen!ng pa!ent care requires much more than just
providing enough informa!on for correct coding and billing.
Clinical documenta!on impacts many areas in all healthcare
organiza!ons including: appropriate pa!ent treatment and
follow-up care, coding, reimbursement, risk management and
compliance. The need to properly document all pa!ent
encounters is, or should be, a concern for everyone in a
healthcare organiza!on. Through this two day’s seminar,
we’ll learn about the need to know and manage our clinical
documenta!on and coding with the accuracy, confiden!ality,
and efficiency that pa!ents and medical staff members have a
right to expect.

1. Ensure that pa!ent care documenta!on is clear, consistent,
accurate, complete, and supports accurate coding.
2. Improve medical record documenta!on and coding to:
• Accurately reflect pa!ents medical status/condi!ons
• Enhance coordina!on of care.
3. Learn, understand and opera!onalize the role of clinical
documenta!on improvement specialists in integra!ng
elements of medical necessity documenta!on into their
daily prac!ce of medical record chart review and query
process.
