USAREUR License and valid U.S. Driver License

A Memorandum for Record signed by Co/Btry Commander or equivalent supervisor stating Name, Rank and SSN was screened in accordance with AR 600-55 and is required to operate TMP/NTV vehicles. Request issuance of TMP license for mentioned personnel. Signature and signature block of commander or supervisor.
