Age Morphology of Primary TuberclesHENRY C. SwEANY, M.D., medical director of research,Chicago Tuberculosis Sanitarium. London: Bailliere,Tindall and Cox. Pp. 265. 27s. 6d.IN the acute exanthemata, lobar pneumonia and many
other diseases, evolution of the lesion follows a timesequence. Dr. Sweany attempts to correlate in a similarway the age of the lesion with its morphology in theprimary focus of tuberculosis. He suggests, amongother analogies, that just as a geologist estimates theage of rocks by studying their strata, so the morbidanatomist should be able to reconstruct the sequence ofevents from the minute details of the pathological lesion.Even in the days of Galen lung stones were recognisedas old lesions. Laennec related some of them to tubercu-losis and distinguished between " bony " and " chalky "concretions ; bony tissue forms in old lesions between thecapsule and the calcified core. Dr. Sweany describesthree stages through which a primary focus passes if itheals : an evolutionary phase ; a period of maturation ;and finally a stage of retrogression. The first, a
pneumonia in miniature, begins with a monocyte reac-tion and ends with caseation ; during the second, thelimiting capsule is formed of collagenous fibres andcalcium deposition begins ; in the third stage fibro-osteoid tissue, bone and marrow are laid down. Withthese " geological strata " for his material Dr. Sweanyhas made a careful and elaborate microscopical study ofprimary foci, identified by post-mortem radiograms, andhas expressed his results by a numerical assessmentwhich may be likened to a system of marks. Though heclaims that a numerical method of this kind helps toeliminate the personal factor, its workings are not alwayseasy to follow. Using it, however, he has investigated27 fatal cases infected before the age of seven, most of thepatients having shown clear-cut infections in infancy.Arranged in age sequence, beginning with the shortestage interval between infection and death, and going onto the longest, the records range from that of a child of7 to a man of 51. Having applied his numerical methodto fifteen different qualities of the tubercle Dr. Sweanyuses it for various other lesions including a calcifiedgroup of haematogenous
" reinfects " in the spleen andliver. He estimates that these had been depositeddurinsr the haematosenous nhase of the nrimarv infections.most of them following the primary lesions within a fewweeks or months and having approximately the sameage appearance. There was, however, one notabledifference in that typical bone was rarely found in spleentubercles. The book is largely composed of case-records,and there are photographs of primary foci and post-mortem radiograms which must be almost unique. Hehas made a monumental study of more than 900 suchradiograms.Jameson and Parkinson’s Synopsis of Hygiene

in 1920 and there is nothing else just like it. In thisedition Professor Parkinson has not been able to drawon the collaboration of Sir Wilson Jameson (whoseenergies must be fully occupied in other directions)though it is still easy to detect his influence and inspira-tion. It is difficult to write a successful synopsisavowedly for students but to be read by many whosetrials are no longer vested in examinations. Today, asthe preface says, a new chapter is being written in thepublic health history of these islands ; and a fascinatingchapter it is, filled with experiments, many of which willinfluence postwar policy, though some must be ephemeral.Since- it is as yet difficult to estimate how much willsurvive, the aim in this new edition has been to describethe outlook and technique of the immediate prewarperiod, except where these have been modified by advancein basic scientific knowledge. There is, however, a
brief account of administrative measures arising fromemergency, and references to fumigation of aircraft,protection against vermin, and disinfestation ; there is a
note, too, on recent regulations governing the lightingof factories. The statistical material has been broughtup to date ; immunisation against diphtheria and whoop-ing-cough are discussed, and there are references totrichiniasis, leptospirosis, and the repercussions on publichealth administration of the effect of sulphonamides incerebrospinal fever, gonorrhoea and puerperal infections.The approach which the book makes to the conceptionof social medicine as distinct from preventive medicinegives the student a new and salutary appreciation ofproblems which will confront him in his later work. ,

AT a meeting of the section of Neurology held onFeb. 19 a discussion on
Cerebral CEdemawas opened by Dr. J. G. GREENFIELD, who emphasisedthe difficulty of determining the criteria from whichthe presence of oedema can be established. - There are,he considers, no real differences between cerebral oedemaand the brain swelling of German literature. Thechemical method by which the water content oi brainis analysed provides an accurate method of determiningoedema, provided that the amount of associated conges-tion is allowed for by determining the iron content ofthe tissue. The histological changes include swellingof astrocytes, irregular swelling of myelin sheaths,beading and end-bulb formation of axons. (Edema islimited to white matter, but there is remarkable sparingof long commissural fibres such as-those in the internalcapsule. Important causes of cerebral oedema are
cerebral ischsemia, cerebral abscess, cerebral tumourand cerebral contusion. With regard to fatal cases ofhead injury there is little evidence of generalised cerebraloedema, and recent studies have failed to provideevidence that it is ever a cause of death. Nor is thereany obvious sound reason for the much used dehydrationtreatment. Dr. Greenfield also referred to less commoncauses of oedema, and to the passage of fluid from theventricles into the cerebral white matter in cases ofcerebral oedema.Major W. from the clinical point of view. The slight generalswelling of the brain which may follow head injuryprobably, he said, causes an increase of intracranialpressure, but there is no satisfactory evidence to supportthe common belief that this is a cause of disorders ofconsciousness. The bradycardia during recovery fromsevere head injury may be associated with normalintracranial pressure and full consciousness. In condi-tions such as cerebral abscess or obstruction to a cerebralvein there is evidence to support the view that associ-ated oedema causes paralysis of local cerebral function.Intravenous hypertonic solutions reduce intracranialpressure for a short time but they are likely to causemore shrinkage of relatively normal brain than of theoedematous area. There is a danger lest the diagnosisof cerebral oedema should be used to conceal our ignoranceof the cause of certain cerebral stages.
Dr. J. HAMILTON PATERSON reported the effects onthe lumbar cerebrospinal fluid pressure of administering50 to 100 c.cm. of 5’0% sucrose by the intravenous route.He made use of a method by which the spinal fluidpressure after lumbar puncture was recorded continu-ously for several hours. In eight subjects with normalpressure a reduction of pressure was demonstrated inonly four subjects and the maximum reduction was30 mm. of water which continued for only one hour. Ineight subjects with raised pressure the intravenoussucrose caused no change in pressure in three cases.
The maximum reduction observed was a fall of 50 mm.which lasted for less than 50 minutes, and thereafterthe pressure rose above the original figure.
During the discussion which followed membersemphasised the difficulty of interpreting the intracranialstate from the spinal fluid pressure. Speakers alsoagreed that prolonged post-traumatic bradycardia isnot due to increased intracranial pressure. The difficul-ties of recognising oedema were also emphasised and theneed for further research was stressed.
