Location: St Cadoc’s Hospital, Newport[with requirement for travel throughout Gofal areas of operation]

Salary: £15,150 per annum pro rata – actual salary £9,924.00

Hours: 1261 annualised hours contract (18.5hrs per week core contract plus 299hrs to be worked when required)

The ServiceThe current vacancy is within our Newport Hospital to Home project and will link with other Gofal services in Gwent. The temporary post is for a Hospital Intervention Support Worker providing outreach support to in-patients at St Cadocs. The service aims to establish and maintain links between health and homelessness and housing services in order to meet housing and ongoing support needs.

Reporting to: Senior Case Worker/Project Co-ordinator

Office Base: St. Cadoc’s Hospital [linking with other Gofal services in Gwent]

Date: March 2016

Job Purpose To provide outreach support to psychiatric in-patients.

To assess individual housing need and co-ordinate ongoing support to facilitate access to housing.

To broker services and maintain links between health and homeless/housing services.

To comply with and maintain all necessary administrative and reporting systems.

Role Expectations:Support Work1. To provide outreach support to psychiatric in-patients.2. To broker services between health and homelessness/housing services on

behalf of, and with individuals.3. To identify on-going support needs with individuals and appropriate support to

enable them to access relevant and appropriate housing services.4. To work alongside colleagues in other organisations sharing knowledge and

information about mental health issues, promoting the role that stable housing has in aiding recovery from mental health problems.
