    1 Slight - superficial injuries,minor cuts,bruises 1 Very low - would not expect to happen 12-16 Intolerable - immediate improvement to control measures

    2 Moderate - open wounds,minor fratures,sprains,burns 2 Low - unlikely to occur (once each year) 8-9 Substantial - planned improvement to control measures

    3 Major - major fractures, amputations 3 Moderate - likely to occur sometime (2/3 times each year) 4-6 Moderate - improvement when possible

    1. Fall of persons 4 Severe 1 Very low 4 Moderate Only trained and authorised persons to use MEWP. All operator to be screened for fitness.

    2. Fall of materials 3 Major 2 Low 6 Moderate Working surface to be lavel and firm.If there is need to operate

    on soft ground outriggers must be used,with sutaible metel/

    timer spreaders(minimum ground support requirements to be

    followed).

    3. Trapping of Persons in mechanism 2 Moderate 2 Low 4 Moderate MEWP must not be moved until they are clear of loose materials

    or tools, and the way ahead is clear.

    4. Overturning 4 Severe 1 Very low 4 Moderate Operators to use a harness and be attached to an anchorage

    in the platform of the MEWP. Operator should not lean out over

    guard rail.

    5. Inadvertent movement 2 Moderate 3 Moderate 6 Moderate Electric,hydraulic,compressed air supplies to be connected to

    chassis never taken direct to platform.

    6. Failure of MEWP in raised position 1 slight 1 Very low 1 Tolerable For dual control MEWP only one station operable at a time

    7. Contact with fixed structure or Fitted with emergency lowering control or inertia reel safety

    parts to be guarded.
