wide range of orthoses. Our functional classification assists in selection of the correct orthosis type, and all our products are weight classified to

ensure they meet the necessary demands.

Overview Hip Joints Reciprocating hip joints are generally indicated for use in patients with lesions at levels TH-5 to L-3. Here we distinguish between two different

variants of reciprocating joints: one or two dimensional (1-D or 2-D). Conventional 1-D joints provide movement in flexion / extension, while 2-D hip

joints add a pelvic rotation component for improved, smoother walking. Stabilization and motion transmission take place via a rocker-bar

mechanism, which we can custom fabricate according to your specifications (model “AG”).

Buran Special features of the reciprocating Buran-2-D hip joints include two separate movement axes, an extended step

length and improved alignment adjustments. The walking axis, when viewed from the front, is tilted 20 degrees

from the vertical in order to permit pelvic rotation. This rotation represents movement of the trunk in relation to the

lower extremity, which allows the legs to remain aligned in the chosen walking direction. The second axis,

separated from the walking axis, is for sitting and allows a maximum angle of up to 155 degrees. The Buran-2-D

makes it possible to change the alignment at any time and adapt to flexion contractures up to 30 degrees.

Revo A special feature of the Revo 2-D hip joints is the 7+7 degree positioning of the movement axis. The first joint

angle, seen from the front, is slanted 7 degrees from the vertical in order to permit pelvic rotation. This rotation

represents movement of the trunk in relation to the lower extremity, which allows the legs to remain aligned in the

chosen walking direction. The second 7-degree angle, viewed from above, is slanted inwardly and anteriorly. It

allows the joint axis to be anteriorly offset and prevents pinching during sitting. Revo 1-D hip joints, without pelvic

rotation, are available for use in patients with limited range of motion. 1-D joints offer a wider adjustment range

than 2-D joints, for alignments to patients with significant contractures.

Partially restricted hip joints for HKAFOs are indicated for use in patients with lesions at levels L-3 to L-5, or for other diagnoses which necessitate

hip stabilization. Here we distinguish between three different variants: 1-D, 2-D and 3-D. 1-D joints permit motion in flexion / extension only; 2-D

joints add a pelvic rotation component for increased step length; 3-D hip joints include abduction. Stabilization of the pelvis takes place via a pelvic

band, which we can custom fabricate according to your specifications (model “AG”).

Fokus A special feature of the Fokus hip joint is that adduction can be completely blocked and flexion partly restricted (to

a 30 degree step length). For the Fokus-3-D / -2-D this occurs without affecting free rotation, as the joint axis

permits 12° of internal and 12° of pelvic rotation independent of flexion / extension.

This rotation refers to the maximum movement of the trunk in relation to the lower extremity (the legs remain

aligned in the previously chosen walking direction). The proximal joint section provides the flexion / extension stop

while the distal section permits rotation. Fokus-1-D hip joints restrict motion only in flexion / extension, for use in

patients with limited ranges of motion.

Absence of hip adductors is compensated by the rigid pelvic connection.

When the weight is shifted laterally and posteriorly, the opposite swing leg is lifted from the floor and, by a reciprocal rocking

mechanism, is transferred forwards. With Buran-2-D hip joints, an external rotation takes place in the swing leg. The hip follows this

rotary movement and simultaneously guides the inner rotation of the supporting leg. This combination of movements enables partial hip

rotation while walking, and allows a longer stride length. A detailed explanation of the gait is available from us.

Sitting Function The sitting function is a further demand on the hip joint for the lesion area L 3 - Th 5. Both hip joints must be capable of flexing at the

same time. This requires releasing the legs from the reciprocal movement mechanism. The patient turns with his back towards a sitting surface. He holds firmly to this with, for example, his left hand and releases the lock at

the right side with his right hand, and vice versa. If the locks were to release the joints immediately after opening, this would cause the

patient to fall, since he is not supporting himself with both hands in order to sit securely. To avoid falling, the sitting function lock is, so to

speak, pre-selected. The patient holds tight exactly as described above and opens the joints in the stressed state. That means that the

center of gravity lies anterior to the hip joints, and the locks are under tension. Once the sides have been unlocked, both hands are

available for support and a hip extension movement relieves the locks, allowing them to spring open. To allow the patient to stand up,

the sitting function is locked while he is still sitting. He stands up supported by both arms. Once erect, the hips are extended completely,

allowing the locks to engage by a snap mechanism.

The step is initiated by shifting the weight onto the supporting leg with simultaneous extension of the hip. Lifting of the swing leg is permitted

Sitting Function Sitting is a further demand on the hip joint in this lesion area. It must be possible to flex both hip joints at the same time. This requires

releasing the legs from the locked flexion restriction. Sitting down with an applied orthosis is usually carried out as follows: the patient turns with his back towards a seat. He holds onto the seat

with, for example, his left hand and releases the lock at the right-hand side and vice versa. The patient is now standing in maximum flexion.

The lever mechanism allows the joints to be opened with little effort. The force required (friction forces of the flexion lock) can be adjusted to

suit the patient by using a different handle length. In addition, the patient can reduce the opening resistance through a hyperlordosis

movement (posteriorly shifting the center of gravity). To re-engage the locks, the patient straightens up after standing up, the hips are fully

extended and the locks automatically snap into place. The joint axis remains locked while standing or lying by means of a pre-compressed

locking spring.

Your outline should be taken over top of any trunk padding. We strive for exact duplication of your measurements within a +/- 2 mm tolerance.

Please indicate the right and left sides of the outline. As seen from the patient’s perspective.

The outline is taken 2.5 cm above the hip joint placement. The distal edge of the pelvic band sits here.

Please inform us if any hip contractures are present. We can adjust the pelvic band accordingly.

Locked Knee Joints These self-locking knee joints remain locked during the swing and stance phases, but can be opened for sitting by lifting the drop lock. A special

feature of the Konso knee joint is the option to increase or decrease the locked flexion angles from 0 to 40 degrees in 5 degree intervals. Blocker

knee joints are available, depending on size, in 0, 10, 20 and 30 degrees of flexion.

Free Knee Joints (modular) Weak knee extensors are stabilized by the Liber knee joint’s posterior offset axis, and flexion / extension is aligned and guided in the walking

direction. The extension stop dampens noisy stopping actions caused by muscle imbalances. Medial / lateral knee stability is ensured by the medial

condyle and lateral supracondylar support points of the orthosis. A flexion angle of up to 140° is possible. The modular Extensor-N option can be

attached to the Liber knee joint to assist knee extension.

New Possibilities with Modular Knee Joints Just adding extension force to an orthosis is not enough. The additional force must also be properly adjusted and accepted by the patient.

How much force should be provided? At what point during flexion should maximum force take place? How much flexion angle should be

Overview Ankle Joints Simply put, grinding joint stops is unnecessary. All ORTHO-SYSTEMS ankle joints use the same interchangeable movement stops to limit plantar-

and dorsiflexion (except the Basic joint with threaded pins). With joints using a cartridge system, this is even possible without disassembling the

joint (excl. joints with attached power unit). All our joints facilitate stumble-free walking by resisting foot drop during swing phase, either using power

units (e.g. Shuttle-Turbo), tension bands (e.g. Ela-Dor) or sustained joint friction (e.g. Jumper). All upper joint sections are CNC-machined from one

piece and assembled with maintenance free washers calibrated to 1/100th of a millimeter to ensure quality and durability. Assistance in narrowing

joint selection according to diagnosis, orthosis type and body weight can be found on our website.

History For the longest time, orthotic ankle joints limited motion (plantar- and dorsiflexion) using movement stops without any, or only insufficient, resistance

to force. Orthoses with these joints always permitted the maximum allowed dorsiflexion during standing, even though the movement was only

necessary for walking. This inefficient standing position required additional muscle force at the knee and hip joints. The first solution to this loss of

energy was provided in 1999 with the development of the L carbon spring. While the carbon spring restricted forward movement to taking place

during walking only, it unfortunately prevented any plantarflexion. In 2011, the V-joint with power units made, for the first time, an erect standing

position possible while allowing plantar- and dorsiflexion during gait. Further information can be found under "Comparison of Ankle Joints with

Power Units" on pages 15-16.

An orthotic system today shouldn’t just support and guide the skeleton. The system also needs to compliment remaining musculature and provide

correction if necessary. All forces applied by orthotic knee and ankle joints must be specifically configured to the needs of the patient and then fine-

tuned. The Shuttle system was designed to accommodate all patients.

The Shuttle ankle joint provides the framework to support and guide the skeleton. The adjustable stirrup

enables alignment modifications of up to 15° to accommodate

any plantar- or dorsiflexion contractures.

Interchangeable movement stops with a range of motion up to

15° plantar- and dorsiflexion adapt movement to ankle joint

conditions.

Please note: We strive to construct our joints as small as possible. However, we don’t make any compromises when it comes to durability and function. The

more functions an orthotic joint possesses (ensured throughout its lifespan), the larger it will be. The modular design of the Shuttle system opens up

more possibilities for the patient and increases acceptance through the ability to change or remove options.

Example The force must be matched to the weight of the patient! One should not be tempted to use a smaller joint, with the argument that the patient moves

very little or has only a low degree of activity. A patient with a body weight of 46-50 kg cannot be supplied with an ankle joint and force unit which is

designated for 36-40 kg. The below example illustrates this. This 46-50 kg patient is fitted with forces light (blue) and strong (yellow). If the same

patient is supplied with a smaller 36-40 kg ankle joint, in order to achieve the equivalent force he could be fitted with the moderate (green) unit, but

the strong (yellow) force is not obtainable.

Ankle Joints with Power Units Compared

The following table compares currently available orthotic ankle joints which make use of a so-called power unit to produce movement

through strong tension or counter force. Compared were two fixed shape L-carbon springs with movement in dorsiflexion, and three

adjustable alignment ankle joints which produce plantar- and dorsiflexion forces that can be dynamically adjusted to gait. Orthotic joints not

possessing similar characteristics, forces, or applications were left out.

Alignment An orthosis using a power unit provides an energy saving and harmonious gait while enabling sturdy and upright standing. The power unit

The Adaptable Shuttle- Ankle Joint A special feature of the Shuttle joint is the ability to

independently adjust the ankle angle according to

patient circumstances at any time.

This variable, double-action ankle joint uses an

Functions of the Leicht-Walker and Foot-Walker Control

Custom adjustments permit unrestricted walking and standing.

Any undesired curving gait can be prevented by increasing the force of the damper unit. Gait: When the body’s center of gravity is shifted, the Walker tilts laterally, producing a forward step. The swing leg stops when reaching the stride length stop. As the weight is shifted onto the swing leg, it becomes the supporting leg and the step cycle starts again. To change direction, the user sustains the weight shift on the supporting leg. Now the damper unit causes the swing leg and the entire Walker construction to turn inwards towards the supporting leg. The user then sets down the free leg and shifts the center of gravity onto the new supporting leg in order to simultaneously relieve the weight from the former supporting leg. The new free leg now turns in the new walking direction. By repeating this sequence, it is possible to change direction or turn around.

Fitting When fittings become complicated and time consuming, it can lead to compromises being made at the cost of the patient. With this in mind, our Leicht-Walker and Foot-Walker podium were designed (and patented) so that all adjustments can be made on-site without having to take the patient out of the orthosis.

Vertical axis adjustment in four directions without dismantling unit.

Primary or secondary deformities can bring about shifts in the body´s center of gravity, which can be compensated for by repositioning the alignment plate. After loosening 4 lock nuts, the alignment plate can slide on the base plate.

Individual adjustment of stride length without dismantling the unit.

Some patients experience unwanted differences in stride length, which can be overcome using the individual stride length adjustment. After loosening the clamp screws on the guide assembly, the stride length can be adjusted using the set screws.

Simple step width adjustment without dismantling the unit.

To optimize walking and stability it is imperative that the correct distance between the footplates be adjusted to your patient. Lesser distance between the footplates eases initiation of the step. Greater distance increases stance stability, however, reduces step length. The foot plates are easily moved closer together or further apart via the threaded spindle on the guide assembly. Subsequent adjustment of the damper unit keeps the feet parallel.

Versatile

Body shell is easily detached to ease donning while lying down.

The ability to separate the body shell from the Leicht-Walker or Foot-Walker makes donning of the orthosis much easier and allows the shell to be used separately if necessary. After opening grips on the connecting plate and superstructure, the body shell can be removed.

Connecting plate is compatible with the Stehbrett Standing Platform.

E.g. A Stehbrett Standing Platform can be used to stand the patient during the period preceding walking, or as an alternating option. After opening the grip on the connecting plate and the upright screw on the superstructure, the body shell together with the superstructure can be transferred to the Stehbrett Standing Platform.

Can be adapted to the next biggest standing and walking unit.

With patients who are still growing, it can be necessary to change to a larger walking or standing unit. After opening grips on the connecting plate and superstructure, the body shell can be adapted to the next size walking unit.

Silicone Frame Socket This ischial ramus containment socket fastens the pelvis diagonally on both sides between the iliac crests and the ischial tuberosity. This technique

has been realized by Udo Danske. A silicone liner in the shape of a swimsuit is fashioned in order to spread out the forces and protect the skin.

Then a two-piece rigid frame fastening both ilia is fabricated. The iliac crest and spine are left uncovered and an opening is provided at the level of

the acetabulum. Posteriorly, the socket is trimmed so that the frame lies on the sacrum but lumbar vertebrae are left free. The Gleiter at the sacral

level maintains the stability of the rigid two piece frame while the Mammut, positioned on the side, holds the socket together and enables donning

of the prosthesis.

The configurator was developed in an interdisciplinary team; a collaboration of design engineer, programmer and orthotist. Each orthosis type is

assigned a so-called rainbow table which uses existing forces of different body types, ages, height and weight ratios (overweight to very thin) to

calculate an indicator, which then selects the appropriate orthotic joint.

Joint selection with just a few clicks

The carousels in the left column offer you a choice of orthotic joints at each level (ankle, knee, hip). All relevant information about each joint is displayed simultaneously on the right.

After making your selection, the total price is shown on the right where you can move the items to a shopping cart.

Milestones

ORTHO-SYSTEMS is a small and relatively new to the industry company, specializing in high quality orthotic joints. Of utmost importance to us is

the production of both durable and functional products. Our company foundations go back to the university days of the owner. While fabricating an

HKAFO, no joints available on the market met the required functions while providing sufficient stability. Together with a precision-engineer friend,

the first unilateral hip, knee and ankle joints were born. Today, more than 20 years later, we are an engineering company led by orthotists. All our

products are designed in CAD, milled exclusively in CNC, and assembled and distributed solely by ourselves. Made in Germany. From the very

beginning, we chose to focus on research and development and thus forgo a sales force. Our model is to meet new customers through trade fairs,

The Unilateral Joint System

To ensure full stability while the orthosis is being worn, it is necessary to ensure proper resistance to the occurring forces.

This is only possible if the stresses are determined in

advance using weight and functional classifications.

It is important to apply the predetermined uprights over the

entire orthosis length in order to stabilize all contact points.

Thanks to this dependable stability, rotation can be

considerably prevented, while a defined lateral flexibility is

permitted to prevent breakage.

Example: The evolution of double-winged aircraft to

today’s passenger planes clearly demonstrates the

unilateral principle. Wings have a defined flexibility to

prevent breakage, while their rotational stability

simultaneously avoids any unwanted movements.
