Reviewing New Applications

Click to see applications you are responsible for reviewing

Right-click on the Name of the application you would like to review and open in a new window

Right-click and open the Application (Review/Print) in a new window

The condensed view allows you to print the application as well as view the entire body of the application in one window

Clicking on the “View Form” link located next to each page heading will take you into that particular page of the full application form as you would see if you clicked on “Application – Full” in the Application Homepage

Click to record your review comments

Enter your comments in this text box

Click “Ok” when complete

Your comments will be filed under the Correspondence tab and viewed by other committee members and the REBA

Reviewing New Applications with Provisos

Identify that Provisos were issued by the Committee/Board previously

Click on the Provisos tab to view the list of provisos issued by the REB

Click to view changes made by the PI/Study Team in response to provisos issued

List of provisos – Click on the title for more details

Click to view changes made in the body of the application

Understanding Versioning:

Each time changes are made to the application, the version number increases.

In this example:Version 0.1 – The Original Application submitted by the Principal InvestigatorVersion 0.2 – This version includes changes submitted in response to the request of the REBAVersion 0.3 – This version includes changes submitted in response to provisos issued by the REB

Selecting version 0.2 will show applications made in response to provisos issued between current version (0.3) and 0.2 only.

Selecting version 0.1 will show all application changes made from the time the application was initially submitted for review to the present, including proviso changes (in v0.3) and changes submitted in response to REBA request (in v0.2).
