WhAt is A dementiA cARe sPeciAlist?

The mission of the Dementia Care Specialist Program is to support people with dementia and their caregivers in order to ensure the highest quality of life possible while living at home. In order to accomplish this mission, the dementia care specialists (DCSs) have four goals:

�Provide free information and assistance to adults with memory or cognitive concerns, or who have been given a dementia diagnosis. They can help with:

independence. Dementia care specialists are local experts on Alzheimer’s disease and other dementias. They can help connect you with all of the opportunities available in your community to keep you active and involved.

memory screening. Dementia care specialists and other staff members at the aging and disability resource center (ADRC) or tribal agency perform memory screens. A memory screen is a tool that helps identify possible memory and cognitive changes. Like a blood pressure screening, this tool is only an indicator. The dementia care specialist may advise a follow-up with a doctor about memory and cognition.

Research opportunities. Dementia care specialists can connect you to research studies that attempt to further understand the causes and possible treatments for Alzheimer’s disease and other dementia.

enrichment opportunities. Some dementia care specialists provide specific activities and programs that connect people with dementia to music, art, nature, and social activities.

�Provide information and support to family members and friends who are caregivers, such as:

Assistance with care planning. Dementia care

specialists can help to address your current situation and plan for the future, including advance care plans and connections to legal and financial planning experts.

connections to support groups and respite. Dementia care specialists know about all of the opportunities and resources available in your community and can help you find information and make connections.

support through programming. Dementia care specialists run support programs to assist family caregivers in providing care for their loved ones at home while maintaining their own health and well-being.

�Help develop dementia-friendly communities where people with dementia can remain active and safe, and caregivers can feel supported. Dementia-friendly communities include:

memory cafes. Dementia care specialists create and support memory cafes. A memory cafe is a meeting place for anyone who has begun to experience changes in memory. It can be a fun way to connect with others who are dealing with the same issues. The intent is for people with memory or cognitive issues and their family or friends to come together in a relaxed and social environment to connect with one another. Each memory cafe is unique, as the participants are encouraged to make it their own.

dementia-friendly businesses. Dementia care specialists can provide training to a business, organization, or other group to help them understand dementia better and be equipped to respond warmly and effectively when serving people living with dementia and their caregivers.

Public presentations and meetings. Dementia

The Alzheimer’s Association Greater Wisconsin Chapter is located in Green Bay. They offer a 24/7 helpline that anyone can call into for services and support for you and your family. This toll free phone number is 1-800-272-3900.

The ADRC offers free classes throughout the year about topics related to Alzheimer’s disease and other dementias. Topics for the free classes can include: Know the 10 Signs of Alzheimer’s Disease, The Basics of Alzheimer’s Disease, Effective Communication, Caregiver Stress and many more!

Dementia care specialists work with veteran service officers, police and sheriff departments, public health departments, transportation departments, and other agencies to ensure that staff members are equipped to respond warmly and effectively when serving people living with dementia and their caregivers.

The behavioral and psychological symptoms of dementia can include showing anger, refusing needed care and wandering. There are few treatment options for behavioral and psychological symptoms of dementia and caregiver supports are often left to deal with

these stressful behaviors on their own.

DICE (Describe, Investigate, Create, Evaluate) is an approach to use in assessing and managing behavioral and psychological symptoms of dementia,

long teRm cARe PRogRAmsfAmily cAReFamily care is a public program that offers individuals a wide variety of long-term care services, making it possible for seniors and adults with physical and developmental disabilities to live at home and stay independent as long as possible.

Family Care members receive support from a care management team made up of a care manager and nurse. They perform a comprehensive assessment of your needs, abilities, preferences and values to develop a personalized plan to meet your outcomes and goals.

Members continue to choose their own physicians while Family Care provides the added long-term care support you need. Your team may make suggestions and assist

long teRm cARe PRogRAms cAn helP if you:

Have a long term care need expected to last more than 90 days; and

Are at least 18 with a physical or developmental disability; or

Are at least 65 years of age; and

Meet functional eligibility criteria; and

Are Medicaid eligible (ADRC can assist with application process)

in coordination, but you and your doctor will continue to be responsible for managing your health and medical care decisions.

iRis (include, ResPect, i self-diRect)IRIS allows for self directed services and puts the individual in charge of his/her own long-term care. An individual creates a plan outlining long-term care supports and services with an assigned monthly budget. Individuals are able to hire their own service workers directly or may utilize an agency or community provider. Individuals enrolled in IRIS work with an IRIS

Consultant and the IRIS Financial Service Agency which pays for the services participants have authorized.

Persons enrolled in IRIS manage goods and services to meet their long-term care needs and use IRIS-funded, waiver eligible supports and services to remain in the community and avoid institutional care. IRIS program participants are supported in self-direction by IRIS Consultants and Fiscal Employer Agents.

Dr. Kales recognized it was not enough to simply tell providers, staff and family caregiver supports to avoid the use of antipsychotic medications. Instead, it is imperative to help caregiver supports better understand, identify and treat the underlying causes of behavioral and psychological symptoms of dementia rather than resorting to the knee-jerk use of medication.

The DICE Approach walks caregiver supports through a process to give context to and identify possible causes of behavioral and psychological symptoms of

dementia. As behavioral and psychological symptoms of dementia management is inherently caregiver supports and patient centered, the DICE Approach assists caregiver supports in using evidence-based behavioral and environmental strategies to assess and manage the behaviors/symptoms.

Crooks use clever schemes to defraud millions of people every year. They often combine new technology with old tricks to get people to send money or give out personal information. Here are some practical tips to help you stay a step ahead. �spot imposters. Scammers often pretend to

be someone you trust, like a government official, a family member, a charity, or a company you do business with. Don’t send money or give out personal information in response to an unexpected request — whether it comes as a text, a phone call, or an email.

�do online searches. Type a company or product name into your favorite search engine with words like “review,” “complaint” or “scam.” Or search for a phrase that describes your situation, like “IRS call.” You can even search for phone numbers to see if other people have reported them as scams.

�don’t believe your caller id. Technology makes it easy for scammers to fake caller ID information, so the name and number you see aren’t always real. If someone calls asking for money or personal information, hang up. If you think the caller might be telling the truth, call back to a number you know is genuine.

�don’t pay upfront for a promise. Someone might ask you to pay in advance for things like debt relief, credit and loan offers, mortgage assistance, or a job. They might even say you’ve won a prize, but first you have to pay taxes or fees. If you do, they will probably take the money and disappear.

�consider how you pay. Credit cards have significant fraud protection built in, but some payment methods don’t. Wiring money through services like Western Union or MoneyGram is

risky because it’s nearly impossible to get your money back. That’s also true for reloadable cards (like MoneyPak or Reloadit) and gift cards (like iTunes or Google Play). Government offices and honest companies won’t require you to use these payment methods.

�talk to someone. Before you give up your money or personal information, talk to someone you trust. Con artists want you to make decisions in a hurry. They might even threaten you. Slow down, check out the story, do an online search, consult an expert — or just tell a friend.

�hang up on robocalls. If you answer the phone and hear a recorded sales pitch, hang up and report it to the FTC. These calls are illegal, and often the products are bogus. Don’t press 1 to speak to a person or to be taken off the list. That could lead to more calls.

�be skeptical about free trial offers. Some companies use free trials to sign you up for products and bill you every month until you cancel. Before you agree to a free trial, research the company and read the cancellation policy. And always review your monthly statements for charges you don’t recognize.

�don’t deposit a check and wire money back. By law, banks must make funds from deposited checks available within days, but uncovering a fake check can take weeks. If a check you deposit turns out to be a fake, you’re responsible for repaying the bank.

�sign up for free scam alerts from the ftc at ftc.gov/scams. Get the latest tips and advice about scams sent right to your inbox.

If you spot a scam, report it at ftc.gov/complaint. Your reports help the FTC and other law enforcement investigate scams and bring crooks to justice.

Manage your personal information wisely; make sure you know why your personal information is required and how it will be used.

Medicare has expanded its coverage to include prescription drugs as of January 1, 2006. Any person who receives benefits through Medicare (Part A and/or B) is eligible for the Medicare Part D drug benefit. However, Medicare Part D is a voluntary program and if you want the benefit you will need to enroll in a Medicare Part D plan. Please be aware that if you do not enroll in Medicare Part D or have other “creditable coverage” you will be subjecting yourself to a premium penalty if you decide to enroll at a later time. Unlike traditional Medicare coverage, private insurance companies whose plans have been approved by the federal

Medicare Savings Programs can help Medicare recipients with limited income and assets pay for their Medicare Part B premium. Some Medicare receipients may also qualify for Prescription Extra Help to pay for the costs of the monthly premiums, annual deductibles, and prescription co-payments that are related to a Medicare Part D prescription drug plan. Many people qualify for these potentially big savings and don’t even know it, so it is worth asking the question.

To find out if you qualify for Prescription Extra Help for the Medicare Savings Programs, contact the ADRC and ask about these Lowincome Medicare programs. We can assist with a financial assessment to see if you qualify and help with the application if you meet the guidlines

Healthcare and advance planning allows you to make choices regarding your medical care and/or finances in the event that you become incapacitated. This is your opportunity to express your wishes rather than potentially having a court appoint a guardian to do this for you. Advance Planning may be made by any competent individual 18 years and older.

The Wisconsin Department of Health Services (DHS) has healthcare and advance planning forms available online. These forms are designed to be completed without the assistance of an attorney. Given your particular circumstances or concerns, however, you

may want to seek legal advice from an attorney. The forms available online are:

Declaration to Physicians (Wisconsin Living Will)

Power of Attorney for Health Care

Power of Attorney for Finance and Property

If printing these forms from the Internet, please assure that you print and complete all pages of the forms you are using. To be valid, the forms must be complete and signed. The Declaration to Physicians (Living Will) and the Power of Attorney for Health Care have accompanying letters that are not part of the legal forms but contain information that should be read prior to completing the forms.

The forms contain standardized wording specified in the Wisconsin Statutes. If these forms are filled out and signed correctly, the completed forms are presumed to be valid in the State of Wisconsin. Do not change the preprinted wording of the forms unless instructions for the forms indicate you may do so. Changing the forms, contrary to their instructions may invalidate the completed forms. If the forms are not sufficient for your needs, consult an attorney.

supported decision-making (sdm) is a recognized alternative to guardianship through which people with disabilities use friends, family members, and professionals to help them understand the situations and choices they face, so they may make their own decisions. Simply, it’s the way most of us make decision; by asking trusted friends and allies to help explain things we don’t understand.

SDM can help people understand information, issues and choices, weigh all their options, ensure that their personal preferences are considered and help them communicate those preferences and choices to the right people. It is a less restrictive alternative to full guardianship of the person.

emeRgency PRePARedness

get A kit of emeRgency suPPlies

The first step is to consider how an emergency might affect your individual needs. Plan to make it on your own for at least three days. It’s possible that you will not have access to a medical facility or even a drugstore. It is crucial that you and your family think about what kinds of resources you use on a daily basis and what you might do if those resources are limited or not available.

emeRgency suPPly kit items: Water, one gallon of water per person per day for at

least three days, for drinking and sanitation

Food, at least a three-day supply of nonperishable food

Battery-powered or hand crank radio and a NOAA Weather Radio with tone alert and extra batteries

First aid kit

Whistle to signal for help

Dust mask to help filter contaminated air and plastic sheeting and duct tape to shelter-in-place

Moist towelettes, garbage bags and plastic ties for personal sanitation

Wrench or pliers to turn off utilities

Can opener for food (if kit contains canned food)
