BACKGROUND

The Little Harbor Sewer Expansion Project including the Little Harbor, Atlantic Avenue and Deep Run/Rust Way Sewer Expansion Districts will be completed in the fall of 2009, excluding various paving and landscaping elements which will continue through 2010. A Draft and then Final Betterment for the cost of the Town’s construction will be issued to serviced users in accordance with prior Town Meeting approvals for the project. The Cohasset Board of Sewer Commissioners is issuing this information document to advise residential property Owners regarding required steps, schedules and measures necessary in order to complete the connection process. The Board of Sewer Commissioners filed a Project–Wide Notice of Intent (NOI) with the Cohasset Conservation Commission. The Project-Wide NOI establishes the submission and review procedures to be followed by the sewer service contractor/drain layer and homeowner. This Project-Wide Notice of Intent will eliminate the need for a separate NOI’s for the private property sewer service construction. An Order of Conditions has been issued for the project and a copy is enclosed in the Appendix materials. Additional site specific conditions may also be required for your property based upon your service layout and a site inspection by the Conservation Commission Agent. In addition, the Board of Sewer Commissioners has been working with the Cohasset Board of Water Commissioners to establish cost effective and environmentally sound construction methods which will allow Owners to simultaneously upgrade their old and often corroded water supply services to their homes within the same excavation trench. This combined utility installation methodology has been approved by the MADEP Southeast Regional Office (SERO) and can significantly reduce site disruption and ledge excavation costs. Refer to the separate Water Commission package enclosed loose with this package. The Board of Sewer Commissioners has also been working with the Cohasset Board of Health to develop a process for septic system decommissioning as part of the overall sewer connection application process so that multiple Town Board applications will not be required. Please be advised that some limitations may be imposed for re-use of septic tanks as pump chambers. It is envisioned that each Owner will be given twelve (12) months to complete their connection from the Official Date of Notification listed on their authorization form unless there are extenuating circumstances. If an Owner cannot complete the connection within this time frame, they must petition the Board of Sewer Commissioners for an extension. Extension forms are provided in Appendix materials. In rare instances, a homeowner with a fully compliant septic system may be allowed to waive connection. A petition form for this action is also enclosed in Appendix materials. In order to initiate the process of connection, an Owner will need to select a contractor who is a Drain Layer licensed in the Town of Cohasset by the Board of Sewer Commissioners. A current list of licensed Drain Layers is on file at the Sewer Commission or can be found on the Sewer Commission web site http://www.townofcohasset.org/sewer/index.html. The Drain Layer will need to depict the sewer service route as well as other features on a plan provided with the application for that purpose and that plan will need to be resubmitted to the Sewer Commission together with a completed application (attached loose with package). Once the application is approved, the Owner can order their grinder pump and initiate construction. After suitable construction, inspection and testing of the new service installation, including the submittal of an as-built plan of the installation by the licensed drain layer, a Final Certification and Authorization to Discharge will be issued by the Sewer Commission. The following sections detail the application process in more detail.

APPLICATION PROCESS An Application for Sewer Service Connection (provided as an Attachment) will need to be completed by each Owner to obtain a sewer service construction permit. The standard application has been revised somewhat to specifically address the needs of the new low pressure sewer expansion connections. Of the over 460 sewer services connections to be installed under this expansion, the vast majority will require the installation of a low pressure sewer service connection and a grinder pump. There are a few conventional, gravity sewer services in the general proximity of the Town Common, and these services will be permitted under a “standard” application process. The application process can be summarized by the following major tasks:

A. Each property owner will be given a site plan of their property based upon available aerial

topographic mapping. (Two (2) copies provided with this information package.)

B. Each owner will need to contract with a Licensed Drain Layer. The drain layer will be required to delineate on a copy of the site plan, the location of the proposed pump, check valve, isolation ball valve, vent and control panel location, as well as, the low pressure sewer service route, any known underground utilities within the work area (including water service, electric, gas, telephone, cable, irrigation lines and drains) and the existing septic system to be decommissioned. Owner input is essential to ensure existing utility locations are depicted on the plan.

C. The drain layer will also be required to physically “stake out” the proposed pump location, sewer

route and existing septic location which he has shown on the submitted application plan. D. The owner will be required to complete and sign the application including a statement to allow site

access for purposes of inspection. This statement also requires compliance with all rules and regulations, including the applicable common Project-Wide Notice of Intent Order of Conditions (which was issued on a project-wide basis ONLY for work related to the sewer connection), plus any site specific requirements issued by the Conservation Commission Agent based upon a site inspection.

E. A fee of $1000 will be collected at the time of the application. This fee includes $150 for

application and plan review, $300 for Conservation Commission inspections, site assessment and erosion control barrier layout, $350 for sewer service, grinder pump and septic system decommissioning inspections and related administration costs and a $200 sewer service pressure testing fee. (Total Fee : $1000.00) An additional Water Commission fee of $200 will also be applicable if an owner chooses to upgrade their water service simultaneously with the sewer service construction. (Refer to Water Commission Information pamphlet attached to this package.)

F. Once the application is approved, a pre-payment for the Grinder Pump can be made and a Grinder

Pump Equipment Voucher will be issued for the Owner’s contractor/drain layer. The voucher will allow the drain layer to pick up the grinder pump once the ordered unit has been delivered to the designated warehouse facility in Rockland, MA. E-One Extreme Grinder Pumps have been chosen as the recommended pump by the Sewer Commission for installation in this expansion area. Alternate pumping units may only be approved on a case-by-case basis through submission of appropriate data and formwork by the owner for review and approval of the Sewer Commission. (Additional information on E-One grinder pump options is included in the Appendix materials.)

FINDING A LICENSED DRAIN LAYER All low pressure sewer service installations must be installed ONLY by Cohasset Licensed Drain Layers. These are the only contractors who are permitted to make your final sewer service connection. If you have a contractor you wish to use who is not currently licensed, they must apply to the Sewer Commission for licensure. A Performance Bond must be posted by the contractor, suitable professional references and experience history must be submitted for verification, a permit fee must be paid and the contractor must also show evidence that a minimum $2 Million General Liability Insurance Policy specifically naming the “Town of Cohasset” including bodily injury and property damage is in place before a License can be issued. The review process can take up to 30 days to complete, so such a delay should be anticipated. On the following page is a list of currently licensed Drain Layers. This list is constantly being updated, so if there are any questions relative to whether your intended contractor is licensed, please call the Sewer Commission Office at Town Hall at 781-383-4108 or go to www.townofcohasset.org/sewer/documents to obtain the latest information. The Board of Sewer Commissioner’s is working with the E-One Grinder Pump local service firm, F. R. Mahony of Rockland, MA to coordinate the distribution of the pre-paid grinder pumps and facilitate pump start-up and certification. F. R. Mahony is also providing Cohasset Licensed Drain Layers with training on the proper installation of the selected low pressure sewer grinder pump units (E-One). This training started early in 2009 and will continue on a quarterly basis. Please verify that your selected drain layer has been or will be attending the training session. Un-trained Drain Layers will not be allowed to pick up the grinder pumps at the F. R. Mahony distribution warehouse. In general terms, Owners are advised to obtain several quotes from various licensed drain layers and to check references. Specifically ask the drain layer what route they would recommend to make the connection. The sewer expansion area is characterized by difficult access conditions and often significant ledge removal issues. Ask the drain layer the size and type of the construction equipment that would be used and specifically the impacts that will result on your property. Ledge removal is often a highly variable cost and an Owner can often avoid conflicts if a unit cost for ledge (such as “cost per cubic yard removed”) or a lump sum cost can be pre-negotiated up front. Owners should also research their other site utilities and provide this information to their drain layer. DO NOT ASSUME that existing utility information is known by the utility providers, it is often not known within the private property limits. By identifying where the water, septic, drain, gas, telephone, cable, electric and irrigation lines and septic system are located, you will help expedite the application process, provide essential information for your drain layer and help avoid “extra work” claims and utility outage issues later during construction, if such utilities are damaged during excavation. Final connection costs can often be reduced through the use of re-plumbing methods. Older estate homes often have several septic lines exiting their homes. By combining these lines into a single discharge point and/or relaying the sanitary lines to exit closer to the roadway service stub, owners may save thousands of dollars by reduced excavation and ledge removal. Generally, it is almost always cheaper to re-route plumbing within the basement and/or crawl space than conducting exterior excavations. Many Licensed Drain Layers are already working with licensed plumbers, septage haulers and electricians to facilitate all elements of the installation and to provide a comprehensive package to the new users.

It is also important to understand the extent of the drain layers warranty or guarantee of his work. Most drain layers will provide at least a one year warranty. Prior to start-up of the pump system, you are required to have the Town pressure test your sewer service installation. One pressure test is provided as part of the application fee. If the pressure test fails and/or a leak occurs somewhere in the drain layers work, we recommend that the Owner get sufficient assurances up-front from the drain layer that they will bear the costs of any leak detection and repair and also cover the cost of the Town’s re-testing fee which will be billed at $95/hour. No connection will be allowed until an installation has passed the Town Pressure Test, which is a one-hour 150 psi hydraulic test.

ORDERING GRINDER PUMPS The Board of Sewer Commissioners has taken measures to secure, on behalf of the future users, the best possible grinder pump equipment package for the service area. A competitive public solicitation was conducted by the Board and the company chosen to supply the grinder pumps to Cohasset residential property Owners is the Environment One Corporation of Niskayuna, New York as locally represented by F.R. Mahony of Rockland, MA. The public solicitation process helped to acquire very competitive pricing for state-of–the-art grinder pumps with extended five (5) year warranties almost twice as long as what is commercially available at this time. In addition, the pumping units will be supplied with optional packages to facilitate both operation and increased performance. The manufacturer’s representative will provide services for the testing, start-up and certification of each installation to assure the unit is properly installed. All of these features have been combined into the solicitation to provide Cohasset users with an exceptionally affordable pump package. The units will be ordered by the Town in bulk and owner will be required to pre-pay their pump order as a direct assessment to the Town to secure the units. Delivery is generally anticipated to follow within three weeks of the direct assessment pre-payment. Everyone should include this delivery time in scheduling their installation. The pre-payment of the grinder pump equipment order (the direct assessment) will occur after the application is filed by the Owner and approved by the Sewer Commission’s agent. The Commission’s agent will review the application and notify the Owner of any corrections needed. As part of the application, the contracted drain layer, in consultation with the Owner, will need to depict the location of the proposed service connection route, the grinder pump, the control panel, the vent (if located in flood prone areas), the isolation ball valve and the redundant check valve for the Owner’s service on a plan provided by the Town. In many cases, the layout of the proposed service and the sizing of the equipment is the primary area where applications can often be deficient. An Owner’s involvement in the route and location selection is imperative. The information in the Appendix details the types of grinder pump units and their cost available through the Town’s solicitation. It should be stressed that the costs reflected in the solicitation include an extensive list of additional services and features not generally included with a direct pump purchase from the vendor. The Town’s package pricing includes several versions of the “Extreme” Series grinder pump, a redundant check valve, an alarm/disconnect panel with generator receptacle and transfer switch, control features including Brown-Out, Run-Dry and High Pressure Protection packages, Indoor Alarm Module, 5 year extended labor and materials warranty, start-up certification, installer training, and Performance Bonding. At the time of application approval and once the Owner pre-payment has been received, a Grinder Pump Equipment Voucher for the pumping equipment order will be issued to the Owner. The Owner will retain the voucher until notified that their unit has been shipped. At that stage the owner will give the voucher to their contracted drain layer. The drain layer will present the voucher to F. R. Mahony and the equipment will be released to the drain layer for installation. Once again, only individual drain layers who complete the F. R. Mahony sponsored training should be allowed to install the grinder pump equipment. F. R. Mahony will hold several training sessions each year to allow sufficient opportunity for Cohasset licensed drain layers to complete the training process.

SEQUENCE OF WORK The following application process flow-path depicts the anticipated sequence of a standard application. (Shaded box is an Owner action item and double boxed is a Drain Layer action item.)

Owner contracts with a licensed Drain Layer. Owner completes the application and files the application with the Sewer Commission, paying the $1000 fee.

Licensed Drain Layer compiles a quote for the sewer service construction work and related system decommissioning.

Licensed Drain Layer helps Owner complete the connection application form.

Application is reviewed and approved by Sewer Commission Agent.

Owner pre-pays the Sewer Commission for their pump equipment order and signs the acknowledgement section of the application.

The Owner gives the Voucher to the Drain Layer and the Drain Layer picks up the equipment.

Sign-offs from the Conservation Commission and Board of Health agents issued, the Sewer Commission issues a Final Certification and an Authorization to Discharge.

The septic system is pumped and decommissioned and the new pump is activated. The Drain Layer completes the site work and provides Decommissioning Certification.

An equipment voucher is issued to the Owner. The Town accumulates orders and processes periodic equipment orders for the E-One pumps. Once equipment has been received in Rockland, the homeowner is notified that the equipment is ready for pick-up.

The Drain Layer commences work, ensuring Conservation Comm. Order compliance and the necessary scheduling of all Town inspections 48 hours in advance. Drain Layer certifies that the installation is ready for pressure testing and schedules a pressure test. Once the system passes a pressure test, a pump start-up test is scheduled with the manufacturer’s representative (F.R. Mahony). Once the pump start-up test is complete, the Drain Layer submits an as-built plan of the new service components

PROJECT CONNECTION SEQUENCING SCHEDULE The new sewer system is being activated in phases to facilitate the gradual acclimation and adjustment of the treatment plant processes as flows begin to increase. The phasing also will help to reduce the potential for odor generation, common with low flow start-ups. Based upon this phasing, the map depicted below shows the alphabetical sequencing of areas relative to the anticipated authorization sequence for connection to the new sewer system.

OWNER CHECKLIST The following check list contains common major tasks related to sewer service construction. Your individual sewer connection may be more or less complex and actual items may vary.

PRELIMINARY STEPS

□ Review this application package, obtain a current list of Licensed Drain Layers and begin the selection process for a Licensed Drain Layer.

□ Research your existing utilities and buried structures on your property so that data can be transferred onto the application and allow any conflicts to be identified and resolved.

□ Determine what you will want to do with your old septic system (see Board of Health Decommissioning Regulations in the Appendix).

□ Determine if you also want to replace/upgrade your water service at the same time or if you want to install a second water meter for outdoor water use to reduce sewer fees (see Water Commission loose pamphlet attached to this package).

□ After obtaining Drain Layer quotes, select a licensed drain layer for your installation.

□ Work with your selected Drain Layer to devise a sewer service layout plan suitable and affordable for your site.

□ With your selected Drain Layer’s input, determine what size and type of pump unit is most suitable for your home. (Note: Indoor units and units with added storage are available.)

APPLICATION & EQUIPMENT ORDERING

□ Submit your Application for Connection along with the layout plan showing ALL the required information. Pay the $1000 application fee.

□ Resolve any discrepancies in the application, obtain application approval, order the pumping equipment, pre-pay the equipment costs and receive your equipment voucher.

□ Upon receiving a call from the Cohasset Sewer Commission (CSC) Office that the ordered equipment has arrived, give your voucher to your drain layer for pick-up and installation.

CONSTRUCTION

□ Have Drain Layer schedule and coordinate all permits and inspections for the system including: □ Inspection of piping and pump unit with CSC prior to backfilling

□ Trench permit and all permits for electrical work and any Building Code related plumbing (Separate Permitting required from Building Department)

□ Pressure testing and Start-up certification inspections through CSC

□ Complete the service installation including the grinder pump. SERVICE ACTIVATION

□ Have Drain Layer submit As-Built Plan within 14 days to the CSC and obtain a Final Certification and Authorization to discharge from the CSC.

□ Have your old septic system decommissioned and activate new sewer service.

INFORMATION FOR COHASSET LOW PRESSURE SEWER USERS

ARE THERE ANY STANDARDS TO FOLLOW IN THE CONSTRUCTION OF MY SEWER SERVICE? Yes, specifications are provided within the Town’s Sewer Regulations and are on the Sewer Commission’s web site http://www.cohassetmass.org/sewer/index.html, for easy reference. Additional information will also be periodically distributed to Licensed Drain Layers. The homeowner will be responsible for construction of all work on their property, including the low pressure sewer service, a redundant check valve, isolation valve, the grinder pump and the building sewer. The grinder pump will also require electrical power feed for its operation. For the Low Pressure Sewer System in Little Harbor, most services will be 1-1/2 inch, 200 PSI rated, SDR21 PVC Pipe with either gasketed joints or solvent welded (cemented) joints. Pipes should be installed at a minimum depth of cover of 48 inches to reduce freezing potential and could potentially be placed at a 36” cover depth if pre-insulated (2” thick) piping were utilized. (Note: Shallow burial pipe increases the chances of winter sewer service freezing, especially if sewer has minimal use.) The sewer should also be placed 10 feet away from any existing water service lines, but there are also some single trench options if a new water service is also installed simultaneously with the sewer service. (Refer to Water Commission Package Attached)

WHO CAN CONSTRUCT MY SEWER SERVICE? Only a Licensed Drain Layer authorized by the Town of Cohasset Board of Sewer Commissioners can work on the sewer system. The contractors are bonded and references are checked to better ensure the quality of work. Additional training is also provided to these licensed individuals by the grinder pump manufacturer, E/One Corporation. A list of licensed Drain Layers is on the web site. For the most up-to-date list, please contact the Board of Sewer Commissioner’s office at 781-383-4108.

WHAT IS THE SCHEDULE FOR THE SERVICE CONNECTION WORK? It is anticipated that the connection activity will commence as early as September 2009 and be in full swing in 2010. The Commission will authorize various areas to connect in a phased manner to control flow increases into the system. In preparation of this authorization, you should collect as much information as possible as to where your existing utilities are (water, drain, gas, electric, telephone, cable, irrigation and septic system) so that the service excavation can avoid these utilities. In addition homeowners should also begin their selection of a Licensed Drain Layer for their service work.

WHAT DO I NEED TO DO WITH MY OLD SEPTIC SYSTEM? After connecting to the municipal sewer system, your old septic system must be decommissioned. Generally this means that the old septic tank and any chambers must be pumped out, sanitized, have their tops crushed and then the tanks/chambers backfilled. This precludes any future issues relative to collapse of the tank. The Board of Health has also recently developed tank re-use criteria in certain instances. Tanks can be re-used as pump chamber, emergency wastewater storage during power outage and as stormwater storage tanks for irrigation purposes. CAN I DISCHARGE MY SUMP PUMP OR ROOF LEADERS TO THE SEWER? NO, this is considered clean water and should be discharged away from the house to the ground surface or to an approved sub-surface leaching chamber. No one is allowed to discharge their sump pump flow, garage or cellar drains or downspouts to the sewer because it consumes wastewater capacity and contributes to hydraulic overloading of the wastewater treatment facilities.

WHY DO I NEED A GRINDER PUMP? Several areas of the Town are serviced by what is referred to as a Low Pressure Sewer System. These Low Pressure Sewer Systems allow wastewater to flow uphill and over difficult terrain eliminating deep excavations through ledge which would significantly increase the cost of the system. The grinder pump grinds the solids and pumps your wastewater into this Low Pressure Sewer System.

HOW DO I GET A GRINDER PUMP? Homeowners are responsible for purchasing their own grinder pump through a Town sponsored program. The Town has solicited guaranteed pricing, warranty and product specifications from various vendors and selected E/One Corporation to furnish the grinder pump units. When a homeowner is starting their sewer service construction, they will prepay the Town for the pump (as a direct assessment) and it will be delivered to F.R. Mahony in Rockland (the E/One local representative and service firm) for pick-up by the homeowner’s contracted Drain Layer. E/One offers several units including an indoor version for homes with extreme ledge issue. There are also units with increased storage capabilities. E/One will also be training the licensed Drain Layers relative to their products to aid the Drain Layers in evaluating the best product for your site.

AFTER I AM NOTIFIED WHAT DO I NEED TO DO? The first step is to complete an Application for Connection and pay the $1000 application fee. This fee covers the application review, inspection by the Conservation Commission Agent to assess potential wetland issues, inspection by the Sewer Commission’s Agent at various stages of construction, pressure testing of the completed sewer service and final certification. You will need to contact a Licensed Drain Layer to complete the application process. You may also need a Plumber, an Electrician and potentially a landscaper to address all aspects of the construction, but many Drain Layers are already working with such individuals to better serve you in this construction effort. The Drain Layer must submit a preliminary routing plan for your service along with your Sewer Connection Application to the Sewer Commission. Once the application is approved, you can commence the work. The Commission must also be contacted to allow inspection of the installation prior to backfilling and also to allow the Town’s contractor to conduct a pressure test of the system. After the installation is approved, you will be given a final notice that you can begin to discharge to the system. Since many users will be contacting Drain Layers at that time, it is best to start the process now by soliciting quotes for the work.

WHAT HOUSEHOLD PRODUCTS SHOULDN’T I DISCHARGE TO THE SEWER? Very simply, only wastewater from toilets, wash sinks, showers, tubs and laundry should be discharged to the sewer. Solid food waste, chemical cleaners and disinfectant, toxic substances, painting products, feminine hygiene products, flammable materials and pesticides/herbicides should NOT be discharged to the sewer system. Homeowners have many options when it comes to selecting the proper grinder pump for their home. F. R. Mahony & Associates, Inc. can help with this selection. The following general guidance may help you make the proper selection.

Regions with basements are more likely to consider indoor pump stations. These stations can aide in “whole house plumbing” to collect drains from basement fixtures adjacent to family rooms or walk out basement uses.

There are a number of factors to be considered. The existing conditions of the home and the lot play a major role in this selection. The Environment One products, developed nearly 40 years ago, were initially intended for indoor use. Not all homes could modify plumbing or make the internal changes required in existing homes. This prompted the development of the outdoor accessway stations. Many E/One applications have been for connection of regions with failing septic systems with existing homes. This made the more common installation fit the outdoor stations.

The indoor stations can result in a savings of installation by avoiding excavation in difficult terrain. Interior plumbing and electrical connections are relatively easy to make. The station cost is reduced by eliminating the top accessway extension and associated excavation to place the station outdoors. Depending on the location one can expect a considerable savings on the total installed cost.

OUTDOOR PUMP Benefits and Options: The typical outdoor pump model for single family homes is the DH071-93 shown to the left. This station operates in the same manner as the indoor station. The pump package comes complete, ready for installation in the ground.

Standard height stations are provided to provide a gravity drain from the home. Deeper stations can be provided to accommodate deeper building drains. These stations are best used when the existing plumbing can not be changed and the best option is to connect existing building drains outside of the home. The outdoor unit may be more suited to customers who prefer that service and access be out of doors. Service providers can service these units without entering the home provided the alarm panel is also outside and accessible. Outdoor models come in a variety of sizes to accommodate larger flow demands.

Project-Wide Notice of Intent (NOI) & Order of Conditions The following text details the basis of approval under the Project-Wide Notice of Intent (NOI) submitted to the Cohasset Conservation Commission on behalf of all future users by the Cohasset Sewer Commission. The Order of Conditions enclosed is applicable to all new sewer connections. By submitting a Sewer Connection Application, the home owner agrees to accept all conditions under this Order as well as any site specific conditions which may be imposed by the Conservation Commission agent as a result of his review and inspection of the proposed sewer service route. Failure to fully accept said conditions will mean that the home owner waives his option to be included under the Project-Wide NOI and the home owner elects to be fully responsible for his own permitting under the Wetland Protection Act and under the local Wetland By-Laws. Through such an election, the home owner must first obtain any such applicable permitting and related Order of Conditions for his individual sewer service prior to commencing any field work.

Project-Wide NOI Sewer Service Completion

1) The Sewer Commission submits an NOI for the project-wide

construction of sewer services and system decommissioning. 2) Develop a Standard Order of Conditions for all services connection

within the 100 Buffer.

3) Develop an expanded set of conditions for work within the 50 buffer.

4) The Order of Conditions would specifically reference the Sewer Service Application and Review process to define all steps and measures to be implemented as part of the process.

The “Process” shall include the following:

APPLICATION

A. Each property owner would be given a site plan of their property based upon available aerial topographic mapping.

B. Each Homeowner would contract with a Licensed Drain Layer and the

Drain Layer would be required to delineate the proposed pump, check valve, isolation valve, vent and control panel location, forcemain route, any known underground utilities within the work area (including water service and drains) and the existing septic system to be decommissioned.

C. The Drain Layer would also be required to stake out the proposed pump

location, sewer route and existing septic location shown on the submitted plan.

D. The homeowner would be required to sign the application along with a

statement to allow site access for purposes of inspection and agreeing to comply with all rules and regulations including the applicable common Order of Conditions (which shall be attached to the application).

E. A fee would be collected at the time of the application including $150 for

application and plan review, $300 for a conservation agent site visit, site assessment and erosion control barrier layout (marking up copy of application plan), $350 for inspections (for sewer service, grinder pump and for septic system decommissioning) and $200 for sewer service pressure testing fee. (Total Fee ……$1,000.00)

REVIEW PROCESS

A. A copy of the application and plan will be made for the Conservation Commission and the Board of Health.

B. The Conservation Commission agent will inspect the site and approximate

the closest wetland boundary and depict on the copy of the plan the required placement of erosion control barriers. A copy of the marked-up plan would then be given back to the Sewer Commission including any “special” site specific conditions that may be required. If there are no wetland resource areas pertinent to the work, the agent will signify this ending Conservation Commission involvement.

C. The Sewer Commission agent would also review the proposed sewer service

layout and develop a list of Sewer Conditions relative to service design, materials and methods.

APPROVAL PROCESS AND PERMIT ISSUANCE

A. The Conservation Commission agent review comments and the Sewer Commission agent review comments would then be combined into an approved application.

B. The owner would then be required to sign a statement agreeing to

implement the approved application conditions or be subject to administrative penalty.

C. The final permit would then be issued for “pick-up” by the Drain Layer.

CONSTRUCTION PROCESS

A. The Drain Layer would be required to sign a statement agreeing to a stipulated inspection schedule, agreeing to abide by process provisions and agreeing to submit an as-built plan using the site plan and ties for the facilities within 14 days of work completion or be subject to administrative penalty and/or revocation of license.

B. The Drain Layer would call and schedule, 48 hours in advance, the

required inspections and testing once all components are ready for such. C. Once all work is complete and the as-built plan has been submitted, a

Certificate of Compliance “sign-off” would be executed by the Conservation Commission, a Septic System De-commissioning “sign-off” would be execurted by the Board of Health and a Final Certification and Authorization to Discharge would be issued by the Sewer Commission.

SEPTIC TANK RE-USE POLICY #1

PURPOSE: To establish procedures for using a decommission septic tank as a storage vessel for lawn and garden irrigation. POLICY:

1. The tank must be pumped; cleaned and inspected by a Cohasset license septage hauler/septic system installer. The tank must be structurally sound to assure it does not become a hazard to safety.

2. Once inspected the tank must be cut off from the soil absorption system. The end of the pipe connecting to the soil absorption system must be glued and capped.

3. A plan (not engineered) must be submitted to the Board of Health showing how stormwater will be collected and how it will be directed into the tank. The plan will be attached to an application form obtained from the Board of Health with an application / inspection fee of $75.00.

4. All work must be inspected before backfilling.

SEPTIC TANK RE-USE POLICY #2

PURPOSE: To establish procedures for using a decommission septic tank as a housing for the grinder pump unit which is connected to the municipal sewer system. POLICY:

1. Prior to placing the grinder pump unit in the septic tank, the tank must be pumped, cleaned and inspected for structural integrity by a licensed septage hauler or septic system installer. The tank must be considered structurally sound. The septic tank outlet pipe must have at least a two (2) foot section removed. After the force main from the grinder pump is installed, the end outlet pipe must be connected shut to preclude the entrance of water into the septic tank. The grinder pump unit must be placed below one of the Septic tank manholes.

2. All work must be inspected.

NOTE: The cover of the grinder pump should be at grade for easy maintenance. This may not be possible if the grinder pump is larger than septic tank manhole. It may be simpler to just crush the tank, remove the concrete and use the hole to put in the grinder pump.

PURPOSE: To establish procedures for using a decommissioned septic tank as overflow storage of sewage from the grinder pump unit. POLICY:

1. The Decommissioned septic tank can be used for the storage of overflow sewage provided the tank is made watertight. A test to assure water tightness must be conducted prior to use as an overflow storage tank.

Directive 01-6: The Title 5 Credit and State Mandated Sewer Connections

DIRECTIVE: Technical Information Release 99-5 allowed taxpayers who repair or replace a failed septic system pursuant to a federal court order, consent decree, or similar mandate from a federal court of competent jurisdiction to take the Title 5 personal income tax credit. This Directive extends the Title 5 personal income tax credit to taxpayers who repair or replace a failed septic system pursuant to an Administrative Consent Order from the Massachusetts Department of Environmental Protection, a Massachusetts state court order, consent decree, or similar mandate from a state court of competent jurisdiction. CLAIMING THE CREDIT: In order to claim the credit a taxpayer must obtain a verification letter from the city or town in lieu of the Certificate of Compliance. See 310 CMR 15.021; TIRs 97-12 and 99-5. The verification letter must state that the taxpayer is subject to an Administrative Order or state court mandate to connect to the city or town sewer system, the date the taxpayer's sewer connection was completed, and that the "abandonment" of the taxpayer's septic system was undertaken in accordance with the Title 5 regulation. See 310 CMR 15.354.(1) The credit is generally available to eligible taxpayers beginning in the tax year in which the work required to repair or replace a septic system is "completed." See G.L. c. 62, § 6(i); TIR 97-12. For purposes of this Directive, that year is the year stated in the verification letter. Taxpayers claiming the Title 5 credit pursuant to this Directive must attach a copy of the verification letter to Schedule SC when filing their Form 1 or Form 1-NR/PY.
