Complete both sides of the form. Incomplete applications will be returned to you for completion. We can only accept documents in English or French. ABOUT THE PROCESS: The Office of the CSR Counsellor is new. It was set up as part of a Government of Canada initiative to enhance the social and environmental performance of Canadian mining, oil and gas companies operating abroad. This Office delivers a new option for constructive resolution of disputes between overseas Canadian mining, oil and gas companies and project-affected communities. Our problem-solving tool, called the Review Process, is about dialogue and creative problem-solving. It is about people with different views and interests working together to find mutually acceptable solutions to resolve disputes or issues. If this voluntary approach is of interest to you, bring your Request to the Office by filling out this form and sending it to us. Personal information collected for the Review Process will be used to establish contact between the Office of the Extractive Sector Corporate Social Responsibility (CSR) Counsellor and the Requester and the Responding Party, to assess the eligibility of the Request for Review (written submission), and to carry out the review to completion. The identity of the Requester and the Responding Party will be made public in the online Request for Review Registry. A Requester can ask that their identity not be included in the online Registry. The identity of the Requester will be provided to the Responding Party, subject to a request for confidentiality. The identity of parties to the Review Process will be shared with Canada’s National Contact Point. This information will be protected in accordance with the federal Privacy Act and may be accessed referring to the personal information bank described in InfoSource as DFAIT PPU 906.
