Document the DOJ Report by clickingon the sign under “Open ExistingCross report to “Create New Cross Report.”

- Enter Date and time this report was filed.- Click on the “Cross Report Type” drop-down menu and choose “Child Abuse Investigation Report.”

Under the “Sent To” grid, click on the checkbox for “Department of Justice.”The drop-down below this checkbox becomes a yellow/mandatory field.

Choose “Department of Justice” from the drop-down menu.

If a referral that meets DOJ reporting criteria is not sent to the DOJ, explain why in the Narrative Description box. Do not check the Sent To box.

To print the report, from the top drop-down menu, click on “file” and select “Print Report.”

- In the pop-up menu, the area of interest defaults to “Referral Reports” *- Under “Report Name”, Choose “Child Abuse Investigation Report (SS8583)”- Click “Print Preview” (NOT “print”) * You can change the Area of

Interest for this drop-down menu if itdoes not default to Referral Reports.

Choose appropriate name (if multiple names available) and Click OK.

NOTE: You will not be able to click Ok (grayed out) unless certain criteria are met. There needs to be at least one completed contact with the child, in person, and for the “Contact Purpose” of “Investigate Referral.”There must also be at least one allegation conclusion of “Substantiated” or“Inconclusive.”

- Enter Date and Time of the incident and any other known information.- Choose victim or victims from the “Victims” box and enter any additional information- Choose suspect or suspects from the “Suspects” box and enter any known information- Click OK

- Click “Yes” for Active Investigation conducted per PC11169(A). If Not, Explain.- Click “Yes” or “No” for victim(s) contacted as well as Suspect(s) contacted.- Click on the suspect(s) name, the click “Yes” for “Suspect given written notice per PC11169B?”- If notice not given, explain why in the comment box. Otherwise, enter date Notice was sent.- Click OK

Print two copies of the report by by using File in the broswer. All Child Abuse Investigation Reports regarding this referral must show the same form identification number.
