Treatment strategies, including the benefits and application of the Passy-Muir ® Valve as part of the evaluation and management of tracheostomized and/or ventilator adult will be given. How the multidisciplinary team approach and early intervention may reduce the physiologic complications of tracheostomy and length of stay. Topics covered will include: Anatomy and physiology review Physiologic and clinical complications of tracheostomy Impact of tracheostomy on speech and swallow Application of the Passy-Muir Valve including advanced ventilator application Airway assessment techniques Rehabilitation strategies: including strength training and mobility; and weaning. 1. Explain why the timing of the tracheotomy and tube selection criteria plays a key role in successful use of speaking valves, rehabilitation and decannulation.

2. Understand physiologic changes to the airway secondary to tracheostomy.

3. State the role and importance of upper airway airflow and subglottic pressure for swallowing function.

4. Identify the different Passy-Muir® Valves and understand the unique benefits of the “no leak” design.

5. Recognize several assessment, placement, and multi- disciplinary treatment strategies involved in the utilization of the Passy-Muir® Valve.

6. Describe how the treatment plan may be impacted by the placement of a tracheostomy tube and Passy-Muir® Valve.

7. Describe successful strategies to perform advanced air way assessment and placement of the Passy-Muir® Valve in the mechanically ventilated adult patient.

8. Describe some common ventilator adjustments that allow successful ventilation with the cuff deflated, and learn appropriate alarm settings that promote safe application.

9. Specify a minimum of two significant physiological effects of tracheostomy and the positive impact of the PMV®

valve on functions of the upper airway.

10. Learn how a comprehensive team approach and pulmonary rehabilitation with early mobilization, communication and swallow exercises can impact length of stay, functional outcomes and quality of life for tracheostomy and ventilator patients.

11. Recognize the role of the Passy-Muir Valve as it relates to the restoration of a closed respiratory system and positive intrathoracic pressure for speech, postural control, extremity force and utilization of treatment strategies that require expiratory airflow through the mouth (e.g. expiratory muscle trainer).
