It is the responsibility of the alumna to obtain her sorority form, complete and submit it according to her group’s requirements. What information should I include? Personal Information Legal name preferred name Date of Birth Home phone Cell phone Email Address, city, state, zip Parents or Guardians name and address High School, City, State (if you have moved during High School, list your other school as well and the dates attended) Year graduated Number in graduating class and your class rank GPA and GPA scale (example: 3.5 on a 4.0 scale) ACT or SAT scores Current College information (are your currently enrolled and attending college) College Classification, major, hours completed, GPA College history information (institutions where you have college hours) College name, hours completed, GPA Sorority Legacy information : relation to you, their name, organization and college attended Activities in high school, work experience, leadership positions, honors, awards, hobbies, church Activities in college Pictures - We suggest a head shot and one full length photo. Check with your college panhellenic to see if they suggest something else! Some schools require that you send a copy of your transcripts with your information
