We recently received a violation for failure to notify the NRC of a name change. We have responded to the office in Washington D.C. per requirements to the violation. Technically, our facility did not have a name change and we should not have requested one on our most recent renewal. There was a merger at the corporate level which does require submittal of a standard questionnaire. However, the facility did in fact remain Frontier Refining LLC. Therefore our license is in error as it gives the parent company's name. We would like to correct that error and have then revert back to what it was, Frontier Refining LLC. As this is an error and not actually a name change we are not submitting that questionnaire, however, please let us know if you need additional information.
