•Are members of the communities they serve and understand the structural barriers.

Case Study

The immigrant family where the mother has hypertension, the father has diabetes, and the children are struggling with obesity (which will eventually shorten their lives and afflict them with diabetes, hypertension, heart disease, liver disease and more) I (the medical provider) can treat their illnesses when they occur, I (the medical provider) can even recommend better diets and more exercise, but if the family is poor and living in a crime-ridden neighborhood in a food desert, accessing those ingredients necessary for a healthy lifestyle may be impossible for them.

Community Health Programs @ LFMC

1. REACH/CENA programs addresses the critical problems at the roots of obesity in our community

2. A medical case manager is on call all the time to help patients address their social issues and connect them with community resources.

3. Integrated behavioral health program employs counselors, case managers and community health workers to help patients deal with their mental health problems holistically.

4. OB education department includes a case manager/counselors, an RN Manager and 3 community health workers to help new families.

Caring for the Whole Person

Community Health Clinics can point the way forward for health care providers looking for ways to help care for the whole person by employing these innovative programs

In 2010 alone, Community Clinics in the U.S. served nearly 19.5 million patients through over 8,100 locations in every state in the nation.

The Community Clinic movement evolved out of a recognition that for these marginalized populations, just providing “medical” care isn’t enough and primary care must extend beyond the illness being treated to include the patient’s family dynamics and whatever community factors might be affecting the patient’s health
