Dear collegues,we report a pure endoscopic biopsy for two astrocytoma grade II in adult population of the floor of the fourth ventricule. We chose the medial trans cysternal approach through the foramen of Magendie.

There are different routes to reach the brainstem by endoscopy. first: the ventricule - acqueduct route , second via the stomy of the third ventriculostomy and two inferior approaches: lateral via the foramen of Luska and the medial trans cisternal way through the foramen of Magendie. We report two cases with this approach. Patient in prone position head in flexion and a ventriculoscop was navigated.
