A.1 SEAT BELTS

Contractors, subcontractors, and grantees, are encouraged toadopt and enforce on-the-job seat belt policies and programs fortheir employees when operating company-owned, rented, or personallyowned vehicles.

A.2 COMPLIANCE WITH U.S. IMMIGRATION LAWS AND REGULATIONS

NRC contractors are responsible to ensure that their alien personnelare not in violation of United States Immigration and Naturalization(INS) laws and regulations, including employment authorizationdocuments and visa requirements. Each alien employee of the Contractormust be lawfully admitted for permanent residence as evidenced by AlienRegistration Receipt Card Form 1-151 or must present other evidencefrom the Immigration and Naturalization Services that employment willnot affect his/her immigration status. The INS Office of BusinessLiaison (OBL) provides information to contractors to help themunderstand the employment eligibility verification process for non-UScitizens. This information can be found on the INS website,http://www.ins.usdoj.gov/graphics/services/employerinfo/index.htm#obl.

The NRC reserves the right to deny or withdraw Contractor use or accessto NRC facilities or its equipment/services, and/or take any number ofcontract administrative actions (e.g., disallow costs, terminate forcause) should the Contractor violate the Contractor's responsibilityunder this clause.

STATEMENT OF WORK

1.1 Background

Based upon emergency preparedness policies set forth by the NuclearRegulatory Commission's (NRC) Incident Response Program, NRCheadquarters and regional offices are required to be equipped with indoorsatellite telephones to use during emergencies when-standard methods ofcommunication become unavailable. Although, NRC regional offices arecurrently using Kyocera equipment to comply with this requirement, during 2003,Kyocera publicly announced that the hardware for many of the satellite phonesthey offer, including the ones that NRC use, would no longer be supported. Thatannouncement from Kyocera has prompted NRC to replace the indoor satellitephones they have currently installed at their headquarters and regional officeswith new Iridium ICHU1000 single-unit satellite phones. NRC has alreadyreplaced the Kyocera satellite phone in Region I, and will be replacing theKyocera satellite phones at Headquarters on March 30, 2004, and plans are nowunderway to replace the Kyocera satellite phone at Region Ill by the end of April2004. NRC is also considering whether to replace the Kyocera satellite phones atRegion II and Region IV.

It is paramount for NRC to quickly replace the Kyocera indoor satellite phoneinstalled at their Region IlIl office in Lisle, Illinois because that office is scheduledto move into a new facility in April 2004. Therefore, NRC is requestingimmediate contractor support to acquire and install a new Iridium ICHU1000single-unit satellite phone with handset, and wall mount locking enclosure at thenew Region Ill facility.

A site survey was previously conducted at both the existing and new Region IlIlfacilities in December 2003 to document the present conditions and to identifythe technical requirements associated with installing the new Iridium ICHU1000single-unit satellite phone. A complete site survey report resulting from that visitis enclosed (See Attachment 1), and shall be used to formulate proposals for thiseffort.

Finally, this statement includes two contract options that are referred to asUcontract option 1" and 'contract option 2". Both of these optionsrequestcontractor support to conduct a site survey and install a new Iridium ICHU1 000single-unit satellite phone with handset, and wall mount locking enclosure at theNRC Region II and NRC Region IV offices.

1.2 Contract Objectives

The primary objectives of this contract are to acquire contractor services toreplace the existing indoor satellite phone installed at NRC Region III, which islocated in Lisle, Illinois, and to provide a deliverable that can technically describethe results of the completed installation clearly and precisely.

2.0 SCOPE OF WORK

Using the NRC Region IlIl Final Site Survey Report (See Attachment 1) as an aid,the contractor shall, (1) procure and install a new Iridium ICHUM000 single-unitsatellite phone with a handset and wall mount locking enclosure at the new NRCRegion IlIl facility, (2) activate the newiridium ICHU1000 single-unit satellitephone - NOTE: Satellite airtime service on the new IridiumICHU1000 single-unitsatellite phone is provided under Delivery Order No. DR-07-03-416, (3) ensurethat the PSTN (public switched telephone network) telephone currently used byNRC Region IlIl is properly integrated with the new Iridium satellite phoneconfiguration and fully operational, and (4) provide a final installation summaryreport that includes a detailed technical description and technical drawings of thecompleted installation.

3.0 TASKS

3.1 Procure a new satellite phone and satellite phone service forNRC Region Ill

* The contractor shall procure an Iridium ICHU1000 single-unit satellitephone that is modular consisting of an antenna, transceiver/receiver,handset, and power supply.

* The contractor shall procure cables, connectors, and any othermiscellaneous hardware needed to successfully complete theinstallation.

* The contractor shall procure a Wall-mount locking enclosure tosecurely store the new Iridium ICHU1000 single unit telephone.

* The contractor shall activate the new Iridium ICHU1000 single-unitsatellite phone.

NOTE: Satellite airtime service on the new Iridium ICHU1000 single-unitsatellite phone is provided under Delivery Order No. DR-07-03-416.

3.2 Install a new satellite phone at new NRC Region Ill office

* The contractor shall install one new Iridium ICHU1000 single-unitsatellite phone with handset and wall mount locking enclosure at thenew NRC Region IlIl facility.

3.3 Deactivate existing satellite phone at the current NRC RegionIll office

* The contractor shall ensure that the PSTN (public switched telephonenetwork) telephone currently used at NRC Region IlIl is properlyintegrated with the new Iridium satellite phone configuration and fullyoperational.

3.4 Qualify Installation at new NRC Region Ill office

* The contractor shall demonstrate to NRC the features andfunctionality of the new Iridium satellite phone by placing several testcalls.

* The contractor shall provide a final installation summary report toNRC that includes a detailed technical description and technicaldrawings of the completed installation.

4.0 DELIVERABLE/SCHEDULE/REPORTING

Deliverables/Tasks Schedule/Reporting RequirementsKick-off meeting Within 2 working days after contract award,

the contractor and NRC shall hold a kick-off meeting telephonically to introduceNRC staff at HQ and Region IlIl withcontractor personnel, and to review projectand deliverable schedules.

Procure a new satellite phone and satellite Within 1 working day after the kickoffphone service for NRC Region IlIl meeting, the contractor shall procure the

following items:

- Iridium ICHU1000 single-unit satellitephone with handset

- Cables, connectors, and any othermiscellaneous hardware needed tocomplete the installation

- Activation for the Iridium ICHU1000single-unit satellite phone

Install a new satellite phone at the new Within 12 working days after the kick-offNRC Region IlIl office meeting, the contractor shall install one

new Iridium ICHU1000 single-unit satellitephone with handset, and wall mountlocking enclosure at the new NRC RegionIlIl facility.

Qualify Installation at new NRC Region III On the day that the installation is completeoffice the contractor shall complete the following

Deliverables/Tasks Schedule/Reporting Requirements- Properly integrate the PSTN (publicswitched telephone network) telephoneused in the current configuration with thenew Iridium satellite phone configurationand ensure that it is fully operational.

- Demonstrate to NRC the features andfunctionality of the new Iridium satellitephone by placing several test callsbetween the region and headquarters.

Draft Installation Summary Report Within 2 working days after the installationis completed, the contractor shall providethe NRC project officer with an electroniccopy of the draft Installation SummaryReport. The report shall include detailedtechnical descriptions, digital photos, andengineering drawings of the completedinstallation. The engineering drawings shallbe created using Visio software or someother software package comparable toVisio and shall be provided to NRC in bothraw format as well as ADOBE PDF format.The contractor will not be required toprovide engineering drawings with the draftreport.

Draft Installation Summary Report Review Within 2 working days after NRC receivesthe draft Installation Summary Report,NRC will provide their comments about the

__ report electronically to the contractor.Final Installation Summary Report Within 2 working days after the contractor

receives comments from NRC about thedraft Installation Summary Report, a finalInstallation'Summary Report will beprovided electronically to the NRC projectofficer.

Final Installation Summary Report Review Within 2 working days after NRC receivesand Approval the final Installation Summary Report, NRC

will provide their approval electronically tothe contractor.

Distribute the Final Installation Summary Within 1 working day after the contractorReport receives an approval notice from NRC, the

contractor shall mail two hard copies of thefinal Installation Summary Report to theNRC project officer.

6.4 Travel

All travel expenses will be reimbursed in accordance with the FederalTravel Regulations. Estimated travel expenses are for one trip to Lisle,Illinois to conduct the installation. Per Diem shall be reimbursed at a dailyrate not to exceed $86.00 per day. The per diem is comprised of lodgingexpense of $55.00 plus $31.00 for meals and incidental expenses, thetotal of which shall not exceed the daily rate. All common carrier travelreimbursed hereunder shall be via economy class rates when available.First class air travel is not authorized. Miscellaneous cost for rental car,taxi, parking etc., shall be reimbursed at a rate of $50.00 per day.Receipts are required for common carrier transportation, lodging andmiscellaneous charges. For offerors located in Lisle, Illinois, the localmileage rate is 37.5 cents per mile for private automobile, if private ownedautomobile is used.

6.0 ORDER TERMS, CONDITIONS, AND REQUIREMENTS

6.1 The new NRC Region III facility, located in lisle, Illinois, is a union facilityand only union certified electricians are permitted to do electrical work inthe building in any capacity. This includes any work associated withconduit or wire pulling.

6.2 Since the new NRC Region III facility, located in lisle, Illinois is a unionfacility, the contractor selected to perform the installation must be a unionmember certified to work on low voltage cabling.

6.3 A 20amp duplex power outlet connected to the NRC region 3 backupgenerator was run from a transformer on the 2nd floor to the buildingphone closet on the 6th floor. An open ended 3/4 inch conduit was run andmarked from the 2nd floor LAN closet to the 6th floor building phonecloset. An 18" square junction box was installed on the south wall of the6th floor building phone closet. A % inch conduit was run from the 6thfloor building phone closet to a junction box on the exterior of thepenthouse location on the west side of the roof.

No wire has been installed in the conduit and the penthouse conduit wasnot extended nor secured to the exterior of the penthouse since the finalheight for the antenna still needs to be determined.

The contractor shall install all low-voltage cabling in the open conduitprovided. The contractor shall also extend and secure the existingpenthouse conduit once the height of the external satellite antenna hasbeen determined.

8.3 Tasks

8.3.1 Conduct a Site Survey

V. Existing NRC Location:

A. Summary: The current Kyocera system, consisting of antenna, transceiver andconnection for a standard telephone is deployed in the following fashion. Theantenna is mounted to and extends above a wall on the roof, and is connected viaapproximately 30 feet of coax cable to the transceiver. The transceiver unit ismounted to a column within the penthouse structure on the roof, and providesconnection points for both the coax cable to the antenna, and the standard Telcocable, which runs from the transceiver to the telephone located in the NRC's IncidentResponse Center on the 2nd floor.

B. Electrical: Electricity (120 VAC) for the Kyocera system isoutlet located within a few feet of the transceiver unit.

As shown the cable comes out the microflect plate up and around the vent, and then up theconduit to the antenna. The phone line (brown cable) coming off of the Transceiver goes upthe wall and travels along the conduit. After leaving the penthouse level the phone cablegoes through the electrical closets down to the 2nd floor, where it is plugged into one pushbutton phone. Conduit is not used for the phone line through the building.

VI. Survey of NRC Future Location:

A. Summary: The new Iridium system consists of the same basic components as theexisting Kyocera system, those being antenna, transceiver and connection for astandard telephone. As shown in the illustrations that follow, the antenna will bewall-mounted using a seven-foot piece of conduit, allowing the antenna to extendfour feet above the top of the wall. Due to manufacturer distance limitations (3 dBmax. loss, or "'8 meter cable length with RG-214 coax), the COAX cable from theantenna cannot be run to an electrical closet located on the 2 nd floor as NRC hadexpected. Coax cable will be run from the antenna down to an electrical closet on the6th floor, where the transceiver will be mounted. However, the new NRC Region IIIfacility requires licensed and union qualified electricians for any electrical work to beperformed in the building. This includes any work associated with conduit or wirepulling. Inside the 6th floor electrical closet, the transceiver box (measuring H 12.5" xW 6.0" x D 3.5") will be mounted on a wall currently occupied by an existing wiringbox. C. Hausman is currently checking with GSA and the building landlord todetermine if the existing wiring box can be removed. If the existing wiring boxcannot be removed, the transceiver would have to be mounted either above or belowthe existing box, or at a location in the room with other available wall space. Thereare no special clearance or distance requirements with respect to the mounting of thetransceiver box.

From the transceiver, standard Telco cable (4 conductor, 24 gauge solid, withpolyethylene jacket) enclosed in 1" conduit will be run down to the 2nd floor electrical

closet and then into the room specified by C. Hausman as the new location forstandard telephone. The phone line will be terminated at a patch panel, wheresite can use it on either 1 of 3 phones. The method of termination for the phonewill be determined once a patch panel has been provided by the Government.

B. Electrical: A standard electrical outlet (120 VAC) is present in the 6th floor electricalcloset and is situated approximately 10 feet away from the proposed location of thetransceiver. There is no other space available to mount the transceiver closer to theexisting outlet. If this distance proves to be too far away from the transceiver, perthe building landlord, the electrical outlet can be relocated. However, the new NRCRegion III facility is a union facility and only union certified electricians are permittedto do electrical work in the building.in any capacity. Also, the transceiver does notcome with any type of battery back-up.

D. Illustrated Description:

The new Antenna will be mounted using unistrut and concrete anchors to the brick wall witha 7' ft Long 2" diameter conduit and span 4' above the roofline. The picture on the rightshows the entry point of the conduit going through the brick into the building. After comingInto the penthouse the conduit goes to the left 25'.

After the 25' the conduit goes through the brick wall on the left. It continues on for 8' andthen straight down 12' through the floor that is shown on the picture to the right. Then afterthe conduit goes through the floor it moves across the ceiling into the electrical closet.

The new Transceiver can mount to the wall in the electrical closet were an existing box hasbeen abandoned (this per the building landlord). The conduit will travel through theelectrical closets from the 6t' floor down to the 2nd floor.

When the conduit gets to the 2nd floor It will leave the electrical closet over the door way onthe left. It will come out to the hallway straight down the hallway and turn right at the exitsign. After turning right at the exit sign it will continue another 15' to the room shown in thepicture. The phone line will then be terminated to a patch panel, where the site can use it oneither I of 3 phones.
