Opalescence is recommended for whitening discolored teeth prior to placement of composite, veneers, and/or crowns. Opalescence iseffective in removing some or all of the internal tooth discolorations due to congenital, systemic, pharmacologic, traumatic, etc., factors andaging. Great success with fluorosis and even tetracycline staining.2,3 The Opalescence sticky formula does not leach from the tray like otherbleaching gels.2 Patients experience results quickly, which motivates them to pursue the treatment. Vital teeth whitened with Opalescenceremain lighter for years.2,3,4,5 Opalescence is available in a variety of concentrations, formulations, flavors and kit configurations to meet allyour whitening needs.

STAYS ACTIVE THROUGH THE NIGHT

Pour impression with fast set plaster ordental stone. Alginate must be pouredshortly after making impression to ensureaccuracy. Trimming is less work if quantityof stone is kept to a minimum.

With vacuum former, heat tray materialuntil it sags approximately 1.5 inches.Adapt plastic over mold. Cool andremove model.

For reservoir spaces, apply LC Block-Outapproximately 0.5mm thick onto the labialsurfaces. Stay about 1.5mm from gingivalline. Do not extend onto incisal edges andocclusal surfaces.

Use Ultra-Trim scalloping scissors toprecisely trim tray 0.25 to 0.33mmocclusal from gingival margin.

Return tray to model; check trayextensions. Gently flame polish edges,one quadrant at a time.

Remove excess gel with clean finger orsoft toothbrush. Follow dentist'sinstructions.

Load the tray by expressing one continu-ous bead of gel approximately half way upfrom the incisal edge on the facial side ofthe tray from molar to molar. This shoulduse about 1/2 to 1/3 of a syringe.

Monitor patient results with the shadeguide card. Keep one for patient recordsand send one home with the patient.

How does bleaching work?The whitening process is possible “due to the ability of the carbamide peroxide and hydrogen peroxide to freely pass through enamel and dentin and topermeate to all parts of the tooth.”1 These peroxides break down into oxygen radicals, which then migrate between the enamel prisms, breaking down anycolored molecules that result in tooth discoloration. The structure of the tooth is not altered; the internal tooth color is simply made lighter.

“Bleaching agents break down into tiny molecules and move in all directions so even if the entire tooth is not covered with bleach, the entire toothis whitened.”2

Numerous studies have proven the effectiveness of peroxides in whitening teeth. Enamel, dentin, existing fillings, and binding materials are not affectedby the whitening agents, nor are they harmed by the whitening materials.

Will bleaching affect bond strength?Even though bleaching agents release a great amount of oxygen into the tooth, existing bonds are notweakened.3

If one is planning to bleach first and then bond, a period of 7-10 days should elapse after thebleaching before bonding is attempted, due to the high concentration of oxygen in the tooth, whichcould significantly and adversely affect polymerization of the resins.3

How long does the bleaching last?Bleaching results are very stable. However, depending on the patient's nutrition and lifestyle habits,bleaching may need to be redone periodically. Due to the safety of the bleaching agents, this shouldnot cause concerns to the dentist or patient.

Will bleaching cause tooth sensitivity?Tooth sensitivity is a common side effect of bleaching. This sensitivity is transient and disappears afterthe completion of bleaching. However, Opalescence products are available with PF desensitizingagents already in them. Research has shown that “just as hydrogen peroxide penetrates through theenamel and dentin and to the pulp, so does potassium nitrate. Fluoride acts primarily as a tubularblocker, plugging the holes and slowing down the fluid flow that causes the sensitivity. Potassiumnitrate acts more like an analgesic or anesthetic by keeping the nerve from repolarizing after it hasdepolarized in the pain cycle. Therefore, there are two mechanisms of action—one affecting fluid flowand the other a direct effect on the nerve.”4

Will bleaching cause problems by weakening the tooth’s enamel?Findings on the Opalescence PF product line presented at the American Association of Dental Research confirm that whitening with Opalescence PF willactually minimize sensitivity, provide anti-caries benefits, increase enamel microhardness, and improve overall enamel health.5, 6, 7, 8, 9

Important: Whitening treatments should only be done under dental supervision!Tooth whitening treatments today are effective/efficacious and safe, if they are used appropriately and with the correct materials. This includes an initialdiagnosis, professional teeth cleaning, briefing on the whitening process to be used, and then monitoring of the patient during the treatment phase.Self-treatment by the patient with over-the-counter (OTC) products can pose risks for the teeth and gums, and often does not provide the results desired.Therefore, all products of the Opalescence system are medical products, and are dispensed or released only by dental practitioners.

There are many causes behind tooth staining. Certain medicines, tooth trauma, root fillings, and even foods and beveragescan cause tooth discoloration over time.
