What should I expect as an Employee?

Before you are eligible to provide services on your new job you must:

Pass a statewide criminal history check

Who is responsible for submitting timesheets to PPL?

It is the responsibility of the Employee to call the TIPS telephonic timesheet system to log in and out. If the Employee is submitting paper timesheets, it is the responsibility of the Employer to collect all timesheets from their Personal Attendants for a paycheck cycle and submit them to PPL together. If Personal Attendant timesheets for a single Employer are not submitted together, payments may be delayed or even denied.

What is the US Citizenship and Immigration Service (USCIS) Form I-9?

The USCIS Form I-9 is your employment eligibility verification. You must bring this form, and the documents listed on page 3 of the I-9, to your Employer. Your Employer will review the documents, confirm your identity and verify your identity under penalty of perjury by signing this form. Federal law requires that all Employers and Employees complete this form.

What taxes will be withheld and how will my paycheck stub change?

PPL will withhold Social Security, Medicare (FICA), and state and federal income taxes from your paycheck. A summary of all tax withholdings will appear on your paycheck stub throughout the calendar year. PPL also will mail you a W-2 form in January. You will need this W-2 form to file your individual tax return by April. Your Employer will receive regular reports from PPL about your total year to date wages and taxes paid.

When do I need to complete and return tax forms to Public Partnerships?

You must complete and return the required forms to PPL once you are hired by an Employer. Delays which go beyond the scheduled timeframe may result in payment delay, or inaccurate tax withholding for services you provide to your Employer.

Are there other forms I need to review?

Yes. PPL has enclosed the following forms for your review.

PPL Timesheet and Payment Calendar. A signed timesheet will authorize PPL to issue a paycheck. A timesheet can be completed online, faxed or mailed, but must to be legible, signed and for authorized service dates and hours. Paychecks are issued every other week. See the enclosed Payment Schedule for more information about processing days and deadlines. Please feel free to make copies of the Timesheet form to record Employee hours.

EMPLOYMENT TERMS AND CONDITIONS AGREEMENT

Certain unemployment tax exemptions exist for employers who hire family members. Is there a

WHEREAS, the State of Indiana, Family and Social Services Administration (FSSA) has designated Public

Partnerships, LLC (PPL) as a Fiscal Intermediary (FI) to provide agent of the employer services on behalf of the

employer stated above, which shall include processing paychecks, performing state and federal withholdings and

reporting, and procuring unemployment insurance, and

WHEREAS, the employer has selected the employee to provide certain services and supports consistent with the

above named Employer’s Service Authorization, and

WHEREAS, the employer will 1) direct the employee on how to deliver services, 2) utilize the individual’s FSSA

funding allocation to support all aspects of the employee’s service, and 3) ensure compliance with the State of Indiana

Program Rules

THEREFORE, the employer and employee hereby agree as follows:

Offer: The employer is pleased to offer the employee a position to provide personal

attendant services to the waiver program participant. The employer believes

there is a good fit between the employee’s skills and interests, and the

individual’s needs.

Expected Start Date:

agreeing to an annual, statewide criminal background check and submitting the

required documents to PPL. Once this information is received, PPL will

conduct the background check and report results to FSSA and the Employer.

Employee must not begin work until informed by PPL that they have been

certified to start.

Wage: $9.80 per hour. The employee will be paid on a bi-weekly schedule per

submission (telephonic, mail or fax) of timesheets to Public Partnerships, LLC,

agent of the employer. A valid timesheet must be signed and dated by the

employee and employer. PPL will withhold appropriate taxes and issue tax

statements based on tax forms filed by the employee and employer, respectively.

The employee is not eligible to receive benefits under this agreement, or

participate in any state pension or retirement plan.

Continued employment will be determined by the FSSA, Division of Aging and

the employer. Employment eligibility is based on satisfactory employee

performance, the employee remaining in good standing with their criminal

background, the employer’s needs, and the availability of State Program funding

for the waiver program participant.

The employee will be listed in a web-based provider registry as an approved

provider available for hire by other waiver program participants. If you do not want to be include in the provider registry please check the opt-out box below:

By signing this agreement, the employee signals his or her understanding that he

or she will be subject to a statewide limited criminal history and the result of the

history may determine employment eligibility.

Also, by signing this agreement, you acknowledge the following federal

regulation that may affect employment: The United States Department of Health

and Human Services-Office of the Inspector General (HHS-OIG) maintains a

List of Excluded Individuals/Entities (LEIE), which includes persons with

convictions for program-related fraud, patient abuse, or licensing board actions.

Medicaid payments cannot be made for services furnished by an excluded person

who is on the LEIE. PPL, in coordination with EverCare Select, is screening

prospective and current attendants monthly to assure compliance with this

federal law.

If the employee is providing transportation to the employer, the employee must

provide a copy of his or her valid driver’s license and proof of insurance. If

there are any changes to an employee’s driving history, the employee is

obligated to update the employer and Fiscal Intermediary.

Either party may terminate this agreement by notifying the other party and PPL,

in writing by submitting the “Notice of Termination” form 5 days prior to formal

separation of employment.

Employer is responsible for proper execution of USCIS Form I-9, as defined in

Instructions for Employment Eligibility Verification, Department of Homeland

Security. Employer must retain original Form I-9. PPL will only provide Form

I-9 in employment packets and retain a forwarded copy in PPL maintained

employee files.

Confidentiality

Upon receipt of information relating to the waiver program participant, the employee will become

a holder of confidential data. The employee agrees to use confidential data solely for carrying out

his/her responsibilities under this agreement.

Indemnification The employee agrees to indemnify and hold PPL and PPL’s principals, agents, employees and

subcontractors harmless for all claims, losses, expenses, fees, including attorney fees, costs and

judgments that may be asserted against PPL based on any acts or omissions of the employee and/or

employer in carrying out their individual responsibilities under this agreement.

Guide to Tax Exemptions Based on Age, Student Status, and Family Relationship Employee Copy – Keep for your records

Employees providing domestic services such as personal assistance may be exempt from paying certain federal and state taxes based on the employee’s age, student status or family relationship to the employer. In some cases, the employer may also be exempt from paying certain taxes based on the employee’s status. IMPORTANT: Please see IRS Publication: #926 – Household Employer’s Tax Guide, and IRS website article: “Foreign Student Liability for Social Security and Medicare Taxes” for additional information.

IMPORTANT:

These exemptions are not optional. If the employee and employer qualify for these tax exemptions they mustbe taken.

If the employee’s earnings are exempt from these taxes, the employee may not qualify for the related benefits,such as retirement benefits and unemployment compensation.

The questions regarding family relationship refer to the relationship between the employee and the employer ofrecord (common law employer). In some cases, the program participant is the employer of record. In othercases, the employer of record may be someone other than the program participant. Check program rules.

Program rules may prohibit some types of employees. For example, most Medicaid-funded programs do notpermit a spouse to be paid as an employee for providing services to a spouse. Check program rules.

PCG Public Partnerships will determine the tax exemptions that apply to the employee and employer based onthe information provided by the employee. PCG Public Partnerships cannot provide tax advice.

Tax Exemptions for Non-Resident Students

For a non-resident student in the United States on an F-1, J-1, M-1, or Q-1 visa admitted to the US for the purpose of providing domestic services, the employer and employee are exempt from paying FICA (Social Security and Medicare taxes) and the employer is exempt from paying FUTA (Federal Unemployment Tax) on wages paid to this employee. The employer may also be exempt from paying State Unemployment Insurance, depending on the rules in the state.

Tax Exemptions for Children Employed by Parent

For a child under 21 employed by his or her parent, the employer and employee are exempt from paying FICA (Social Security and Medicare taxes) and the employer is exempt from paying FUTA (Federal Unemployment Tax) on wages paid to this employee until the child (employee) turns 21 years of age. The employer may also be exempt from paying State Unemployment Insurance, depending on the rules in the state.

Tax Exemptions for Spouses Employed Spouses

For a spouse (husband, wife, or domestic partner in some states) employed by his or her spouse, the employer and employee are exempt from paying FICA (Social Security and Medicare taxes) and the employer is exempt from paying FUTA (Federal Unemployment Tax) on wages paid to this employee. The employer may also be exempt from paying State Unemployment Insurance, depending on the rules in the state.

Tax Exemptions for Parents Employed by Children

For a parent employed by his or her child and answering “No” to any of the additional questions under Question #6 regarding caring for a grandchild or step grandchild, the employer and employee are exempt from paying FICA (Social Security and Medicare taxes) and the employer is exempt from paying FUTA (Federal Unemployment Tax) on wages paid to this employee. The employer may also be exempt from paying Stat e Unemployment Insurance, depending on the rules in the state.

For a parent employed by his or her child and answering “Yes” to all of the additional questions regarding caring for a grandchild or step grandchild, the employer is exempt from paying Federal Unemployment Tax (FUTA) on wages paid to this employee. The employer may also be exempt from paying State Unemployment Insurance, depending on the rules in the state.

What is it for?

Why does the employer need to sign this?

The IRS holds both the employer and the employee responsible for an individual’s legal status and eligibility for employment.

I have a temporary work authorization that needs to be renewed at a later date. Can I still work?

Yes. If you are eligible to work now, you can complete this form. It is important that you keep your work authorizations up-to-date. Your employer will need to verify your status again once your temporary authorization has expired.

Do I need to do anything more than sign the I-9?

Yes. You need to show proof that you are legally eligible to work. There are 10 types of documents that verify work eligibility listed on page 2. Show these documents to your employer when he/she certifies the I-9.

I what Is the Purpose of This Form?

Employers must complete Form 1-9 to document verification of the identity and employment authorization of each new employee (both citizen and noncitizen) hired after November 6, 1986, to work in the United States. In the Commonwealth of the Northern Mariana Islands (CNMI), employers must complete Form 1-9 to document verification of the identity and employment authorization of each new employee (both citizen and noncitizen) hired after November 27, 2011. Employers should have used Form 1-9 CNMI between November 28, 2009 and November 27, 2011.

I General Instructions

Employers are responsible for completing and retaining Form I-9. For the purpose of completing this form, the term "employer" means all employers, including those recruiters and referrers for a fee who are agricultural associations, agricultural employers, or farm labor contractors. Complete, sign, and submit to PPL.d

Form l-9 is made up of three sections. Employers may be fined if the form is not complete. Employers are responsible for retaining completed forms. Do not mail completed forms to U.S. Citizenship and Immigration Services (USCIS) or Immigration and Customs Enforcement (ICE).

I Section 1. Employee Information and Attestation - Instructions for Employees

Newly hired employees must complete and sign Section 1 of Form Is Section 1 should never be completed before the employee has accepted a job offer. This must be completed before you can start work.Provide the following information to complete Section 1:

Name: Provide your full legal last name, first name, and middle initial. Your last name is your family name or surname. If you have two last names or a hyphenated last name, include both names in the last name field. Your first name is your given name. Your middle initial is the first letter of your second given name, or the first letter of your middle name, if any.

Other names used: Provide all other names used, if any (including maiden name). If you have had no other legal names, write "N/ A." Address: Provide the address where you currently live, including Street Number and Name, Apartment Number (if applicable), (l.ity, State, and Zip Code. Do not provide a post office box address (P.O. JJox). Only border commuters from Canada or Mexico may use an international address in this field.

Date of Birth: Provide your date of birth in the mm/dd/yyyy format. For example, January 23 , 1950, should be written as 01123/1950.

U.S. Social Security Number: Provide your 9-digit Social Security number. Providing your Social Security number is voluntary. However, if your employer participates in E-Verify, you must provide your Social Security number. E-mail Address and Telephone Number (Optional): You may provide your e-mflil address and telephone number. All employees must attest in Section I, under penalty of perjury, to their citizenship or immigration status by checking one of the fo11owing four boxes provided on the form:

2. A noncitizen national of the United States: Noncitizen nationals of the United States are persons born in American Samoa, certain former citizens of the former Trust Territory of the Pacific Islands, and certain children of noncitizen nationals born abroad.

3. A lawful permanent resident: A lawful permanent resident is any person who is not a U.S. citizen and who resides in the United States under legally recognized and lawfully recorded permanent residence as an immigrant. The term "lawful permanent resident" includes conditional residents. If you check this box, write either your Alien Registration Number (A-Number) or USCIS Number in the field next to your selection. You can find your Admission Number on Form 1-94, "Arrival-Departure Record," or as directed by USCIS or U.S. Customs and Border Protection (CPB).

(1) If you obtained your admission number from CBP in connection with your arrival in the United States, then also record information about the foreign passport you used to enter the United States (number and country of issuance).

By signing and dating this form, you attest that the citizenship or immigration status you selected is correct and that you are aware that you may be imprisoned and/or fined for making false statements or using false documentation when completing this form. To fully complete this form, you must present to your employer documentation that establishes your identity and employment authorization. Choose which documents to present from the Lists of Acceptable Documents, found on the last page of this form. You must present this documentation no later than the third day after beginning employment, although you may present the required documentation before this date.

Minors and Certain Employees with Disabilities (Special Placement)

Parents or legal guardians assisting minors (individuals under 18) and certain employees with disabilities should review the guidelines in the Handbookfor Employers: Instructions for Completing Form 1-9 (M-274) on www.uscis.gov/ I-9Central before completing Section 1. these individuals have special procedures for establishing identity if they cannot present an identity document for Form 1-9. The special procedures include (1) the parent or legal guardian filling out Section 1 and writing "mi.r10r under age 18" or "special placement," whichever applies, in the employee signature block; and (2) the employer writi~g "minor under age 18" or "special placement" under List B in Section ·2:'

Section 2. Employer or Authorized Representative Review and Verification

Before completing Section 2, employers must ensure that Section 1 is completed properly and on time. Employers may not ask an individual to complete Section 1 before he or she has accepted a job offer.

Employers or their authorized representative must complete Section 2 by examining evidence of identity and employment authorization within 3 business days of the employee1S first day of employment. For example, if an employee begins employment on Monday, the employer must complete Section 2 by Thursday of that week. However, if an employer hires an individual for less than 3 business days, Section 2 must be completed no later than the first day of employment. An employer may complete Form I-9 before the first day of employment if the employer has offered the individual a job and the individual has accepted. This must be completed prior to employment.

Employers cannot specify which document(s) employees may present from the Lists of Acceptable Documents, found on the last page of Form I-9, to establish identity and employment authorization. Employees must present one selection from List A OR a combination of one selection from List Band one selection from List C. List A contains documents that show both identity and employment authorization. Some List A documents are combination documents. The employee must present combination documents together to be considered a List A document. For example, a foreign passport and a Form I-94 containing an endorsement of the alien1s nonimmigrant status must be presented together to be considered a List A document. List B contains documents that show identity only, and List C contains documents that show employment authorization only. If an employee presents a List A document, he or she should not present a List Band List C document, and vice versa. If an employer participates in E-Verify, the List B document must include a photograph.

In the field below the Section 2 introduction, employers must enter the last name, first name and middle initial, if any, that the employee entered in Section 1. This will help to identify the pages of the form should they get separated.

Employers or their authorized representative must:

1. Physically examine each original document the employee presents to determine if it reasonably appears to be genuine and to relate to the person presenting it. The person who examines the documents must be the same person who signs Section 2. The examiner of the documents and the employee must both be physically present during the examination of the employee1s documents.

2. Record the document title shown on the Lists of Acceptable Documents, issuing authority, document number and expiration date (if any) from the original document(s) the employee presents. You may write 11N/A 11 in any unused fields. lfthe employee is a student or exchange visitor who presented a foreign passport with a Form I-94, the employer should also enter in Section 2:

a. The student1s Form I-20 or DS-2019 number (Student and Exchange Visitor Information System-SEVIS Number); and the program end date from Form 1-20 or DS-2019.

3. Under Certification, enter the employee1s first day of employment. Temporary staffing agencies may enter the first day the employee was placed in a job pool. Recruiters and recruiters for a fee do not enter the employee1s first day of employment.

4. Provide the name and title of the person completing Section 2 in the Signature of Employer or Authorized Representative field.

5. Sign and date the attestation on the date Section 2 is completed.

6. Record the employer1s business name and address.

7. Return the employee1S documentation.

Employers may, but are not required to, photocopy the document(s) presented. If photocopies are made, they should be made for ALL new hires or reverifications. Photocopies must be retained and presented with Form 1-9 in case of an inspection J:ty DHS or other federal government agency. Employers must always coll?'Blete Section 2 even if they photocopy an employee1s document(s). Making photocopies of an employee1s docume·nt(s) cannot take the place of completing Form I-9. Employers are still responsible for completing and retaining Form I-9.

Unexpired Documents

Generally, only unexpired, original documentation is acceptable. The only exception is that an employee may present a certified copy of a birth certificate. Additionally, in some instances, a document that appears to be expired may be acceptable if the expiration date shown on the face of the document has been extended, such as for individuals with temporary protected status. Refer to the Handbook for Employers: 1nstructionsfor Completing Form 1-9 (M-274) or 1-9 Central (www.uscis.gov/I-9Central) for examples.

Receipts

If an employee is unable to present a required document (or documents), the employee can present an acceptable receipt in lieu of a document from the Lists of Acceptable Documents on the last page of this form. Receipts showing that a person has applied for an initial grant of employment authorization, or for renewal of employment authorization, are not acceptable. Employers cannot accept receipts if employment will last less than 3 days. Receipts are acceptable when completing Form I-9 for a new hire or when reverification is required.

Employees must present receipts within 3 business days of their first day of employment, or in the case of reverification, by the date that reverification is required, and must present valid replacement documents within the time frames described below.

There are three types of acceptable receipts:

1. A receipt showing that the employee has applied to replace a document that was lost, stolen or damaged. The employee must present the actual document within 90 days from the date of hire.

2. The arrival portion of Form I-94/I-94A with a temporary 1-551 stamp and a photograph of the individual. The employee must present the actual Permanent Resident Card (Form I-551) by the expiration date of the temporary 1-551 stamp, or, if there is no expiration date, within 1 year from the date of issue.

3. The departure portion of Form I-94/I-94A with a refugee admission stamp. The employee must present an unexpired Employment Authorization Document (Form 1-766) or a combination of a List B document and an unrestricted Social Security card within 90 days.

When the employee provides an acceptable receipt, the employer should:

1. Record the document title in Section 2 under the sections titled List A, List B, or List C, as applicable.

2. Write the word "receipt" and its document number in the "Document Number" field. Record the last day that the receipt is valid in the "Expiration Date" field.

By the end of the receipt validity period, the employer should:

1. Cross out the word "receipt" and any accompanying document number and expiration date.

2. Record the number and other required document information from the actual document presented.

3. Initial and date the change.

I Section 3. Reverification and Rehires Complete this section ONLY if rehiring within 3 years.

Employers or their authorized representatives should complete Section 3 when reverifying that an employee is authorized to work. When rehiring an employee within 3 years of the date Form I-9 was originally completed, employers have the option to complete a new Form l-9 or complete Section 3. When completing Section 3 in either a reverification or rehire situation, ifthe employee's name has changed, record the name change in Block A.

Some employees may write "N/A" in the space provided for the expiration date in Section 1 if they are aliens whose employment authorization does not expire (e.g., asylees, refugees, certain citizens of the Federated States ofM.icronesia, the Republic of the Marshall Islands, or Palau). Reverification does not apply for such employees unless they chose to present evidence of employment authorization in Section 2 that contains an expiration date and requires reverification, such as Form I-766, Employment Authorization Document.

Reverification applies if evidence of employment authorization (List A or List C document) presented in Section 2 expires. However, employers should not reverify: 1. U.S. citizens and noncitizen nationals; or

2. Lawful permanent residents who presented a Permanent Resident Card (Form I-SS 1) for Section 2.

Reverification does not apply to List B documents.

If both Section 1 and Section 2 indicate expiration dates triggering the reverification requirement, the employer should reverify by the earlier date. For reverification, an employee must present unexpired documentation from either List A or List C showing he or she is still authorized to work. Employers CANNOT require the employee to present a particular document from List A or List C. The employee may choose which document to present.

To complete Section 3, employers should follow these instructions:

1. Complete Block A if an employee's name has changed at the time you complete Section 3.

2. Complete Block B with the date of rehire if you rehire an employee within 3 years of the date this form was originally completed, and the employee is still authorized to be employed on the same basis as previously indicated on this form. Also complete the "Signature of Employer or Authorized Representative" block.

3. Complete Block "C if:

a. The employment authorization or employment authorization document of a current employee is about to expire and requires reverification; or

b. You rehire an employee within 3 years of the date this form was originally completed and his or her employment authorization or employment authorization document has expired. (Complete Block B for this employee as well.)

To complete Block C:

a. Examine either a List A or List C document the employee presents that shows that the employee is currently authorized to work in the United States; and

b. Record the document title, document number, and expiration date (if any).

4. After completing block A, B or C, complete the "Signature of Employer or Authorized Representative" block, including the date.

For reverification purposes, employers may either complete Section 3 of a new Form 1-9 or Section 3 of the previously completed Form I-9. Any new pages of Form I-9 completed during reverification must be attached to the employee's original Form I-9. If you choose to complete Section 3 of a new Form l-9, you may attach just the page containing Section 3, with the employee's name entered at the top of the page, to the employee's original Form 1-9. If there is a more current version ofForm l-9 at the time of reverification, you must complete Section 3 of that version ofthe form.

I What Is the Filing Fee? This will be retained by PPL.

There is no fee for completing Form I-9. This form is not filed with USCIS or any government agency. Form I-9 must be retained by the employer and made available for inspection by U.S. Government officials as specified in the "USCIS Privacy Act Statement" below.

l Photocopying and Retaining Form 1-9

A blank Form I-9 may be reproduced, provided all sides are copied. The instructions and Lists of Acceptable Documents must be available to all employees completing this form. Employers must retain each employee's completed Form I-9 for as long as the individual works for the employer. Employers are required to retain the pages of the form on which the employee and employer enter data. If copies of documentation presented by the employee are made, those copies must also be kept with the form. Once the individual's employment ends, the employer must retain this form for either 3 years after the date of hire or 1 year after the date employment ended, whichever is later.

AUTHORITIES: The authority for collecting this information is the Immigration Reform and Control Act of 1986, Public Law 99-603 (8 USC 1324a).

PURPOSE: This information is collected by employers to comply with the requirements of the Immigration Reform and Control Act of 1986. What is it for?

This form tells the IRS about the withholding allowances for which the employee is eligible.

Who needs to sign?

Every employee working with an employer who is using the FI.

What if an employee does not want to sign this tax form?

The FI needs this form completed and signed in order to withhold taxes with the employee’s desired allowances. In order to pay an employee, the FI must have a completed and signed form on file.

How should I complete the W-4 worksheet?

The FI cannot give advice about what allowances you should claim. If you have questions about what allowances you should claim, contact your tax professional.

General Instructions

If you aren’t exempt, follow the rest of these instructions to determine the number of withholding allowances you should claim for withholding for 2019 and any additional amount of tax to have withheld. For regular wages, withholding must be based on allowances you claimed and may not be a flat amount or percentage of wages.

You can also use the calculator at www.irs.gov/W4App to determine your tax withholding more accurately. Consider

using this calculator if you have a more complicated tax situation, such as if you have a working spouse, more than one job, or a large amount of nonwage income not subject to withholding outside of your job. After your Form W-4 takes effect, you can also use this calculator to see how the amount of tax you’re having withheld compares to your projected total tax for 2019. If you use the calculator, you don’t need to complete any of the worksheets for Form W-4.

Note that if you have too much tax withheld, you will receive a refund when you file your tax return. Figure the total number of allowances you’re entitled to claim and any additional amount of tax to withhold on all jobs using worksheets from only one Form W-4. Claim all allowances on the W-4 that you or your spouse file for the highest paying job in your family and claim zero allowances on Forms W-4 filed for all other jobs. For example, if you earn $60,000 per year and your spouse earns $20,000, you should complete the worksheets to determine what to enter on lines 5 and 6 of your Form W-4, and your spouse should enter zero (“-0-”) on lines 5 and 6 of his or her Form W-4. See Pub. 505 for details.

Instructions for Completing Form WH-4This form should be completed by all resident and nonresident employees having income subject to Indiana state and/or county income tax.

Print or type your full name, Social Security number or ITIN and home address. Enter your Indiana county of residence and county of principal employment as of January 1 of the current year. If you neither lived nor worked in Indiana on January 1 of the current year, enter ‘not applicable’ on the line(s). If you move to (or work in) another county after January 1, your county status will not change until the next calendar tax year.

Nonresident alien limitation. A nonresident alien is allowed to claim only one exemption for withholding tax purposes. If you are a nonresident alien, enter “1” on line 1, then skip to line 7. You are considered to be a nonresident alien if you are not a citizen of the United States and do not meet the green card test and the substantial presence test (get Publication 519 from www.irs.gov for information about these tests).

All other employees should complete lines 1 through 7.

Lines 1 & 2 - You are allowed to claim one exemption for yourself and one for your spouse (if he/she does not claim the exemption for him/herself). If a parent or legal guardian claims you on their federal tax return, you may still claim an exemption for yourself for Indiana purposes. You cannot claim more than the correct number of exemptions; however, you are permitted to claim a lesser number of exemptions if you wish additional withholding to be deducted.

Line 3 - Dependent Exemptions: You are allowed one exemption for each of your dependents based on state guidelines. To qualify as your dependent, a person must receive more than one-half of his/her support from you for the tax year and must have less than $4,150 gross income during the tax year (unless the person is your child and is under age 19 or under age 24 and a full-time student at least during 5 months of the tax year at a qualified educational institution).

NOTE: An entry on this line does not obligate your employer to withhold the amount. You are still liable for any additional taxes due at the end of the tax year. You may file a new Form WH-4 at any time if the number of exemptions increases. Penalties are imposed for willingly supplying false information or information which would reduce the withholding exemption.

Direct Deposit, also known as Electronic Funds Transmission (EFT), is the fastest and safest way to receive your paycheck from PPL on behalf of your employer. Your payment can be deposited directly into your checking account, savings account, or to a pay card of your choice. To sign up, review the steps below and complete the Direct Deposit application. 

1. Meet Direct Deposit RequirementsComplete the Direct Deposit Application.Agree to immediately notify PPL in writing if you change your bank, account number, accounttype, ABA routing number, or contact information. You may need to submit a new Direct DepositApplication form. Failure to comply with this may result in delay of payment.

2. Submit Direct Deposit Application to PPLOnce you have completed the Direct Deposit application, you must gather and submit account verification documents to PPL. This differs depending on where you want your funds to go: • Checking account: Submit a voided check or a letter from your bank that states the checking

account number where your funds should be deposited.

• Savings account: Submit a letter from your bank that states your savings account number whereyour funds should be deposited.

• Pay card/debit card: Submit documentation from the pay card’s enrollment process or the paycard’s financial entity that verifies the account and the routing numbers.

NOTE: If you choose this option, please note that PPL does not support any particular pay card/debit card financial institution and is not responsible for any fees established by the financial institution. PPL recommends you review all pertaining to your pay card prior to enrolling and activating it.  

3. Await confirmation from PPLYour Direct Deposit account will become active after PPL verifies your account number with your bank or pay card. The whole process will take 1 to 2 pay cycles from the time we receive your completed and signed application. 

If there is a change in bank account information, your PPL payment account will be taken off Direct Deposit status until the new bank account information is verified. Verification may take a few weeks. You will receive paper checks in the interim period. 

The Direct Deposit payment is sent on the check date (see Payroll Schedule) and should be in your bank account 24‐48 hours afterwards. Please note that bank holidays may delay posting. After considering bank holidays, contact PPL if you don’t receive your payment on time. 

That’s it! Once your Direct Deposit becomes active, you will receive a summary of your gross wages, tax withholding, etc. on a document called a “Remittance Advice” that is mailed to you. Thank you for signing up – we hope you enjoy having faster access to your payments!

1. Do I need a bank account to obtain a pay card?No, you do not need a bank account to obtain a pay card.

2. What is a pay card?A pay card is an easy way to have your money deposited each payday without having a bankaccount. You are able to use this pay card at any ATM or it can be used just like a debit card tomake purchases directly.

3. Where can I obtain a pay card?A pay card can be obtained at a local merchant store (Wal‐Mart, CVS, Walgreens, etc) for anominal fee or via the pay card company’s website.

4. Is a new pay card needed for each pay cycle?No, the same pay card is used every pay cycle and the pay amount is directly transferred ontothe card.

5. What if all the money on the pay card is not used before the next pay cycle?The remaining balance on your pay card will carry over with your next deposit.

6. How do I know the balance on my pay card?For many pay cards, there will be a customer service number for the financial institution on theback of your pay card that you can call to obtain your card balance. Simply follow the directionsprovided to obtain your balance. If there is not a phone number for balances indicated on yourcard, refer back to your pay card enrollment paperwork for more information.

7. Are there any fees with the pay card?Your particular pay card may have transaction fees. PPL is not responsible for any feesestablished by the pay card’s financial institution.

8. Can I use my pay card at an ATM and will there be surcharges?Pay cards are usually accepted at any ATM. If the ATM charges a surcharge, you will be notifiedbefore the transaction is completed. You can accept the charge or you will have the option ofcanceling the transaction if you do not want to pay the fee.

9. Can I use my pay card to make online purchases?Usually, your pay card can be used to make online purchases. Refer to your pay card enrollmentpaperwork for additional information.

What You Need to Know About Incident Reporting

It is a requirement for anyone with direct care responsibilities of a participant including employers of records, providers and case managers with knowledge of an incident that affects, or potentially affects, this participant’s health, safety, or welfare to submit an initial Incident Report. An incident involving:

A) abuse, neglect, or exploitation of a participant or the death of a participantmust be submitted within 24 hours of the incident

OR

B) an “unusual occurrence” affecting the participant’s health, safety and welfareare must be submitted within 48 hours of the time of the incident

Spend-Down Policy

EFFECTIVE DATE: MAY 5, 2010

Individuals receiving services through the Aged and Disabled Waiver’s Self-Directed Attendant Care program may have a spend-down. Having a spend-down means that an individual must pay his or her provider(s) directly for a portion of the services he or she receives each month.

If the individual you provide services to has a spend-down, and that amount has not already been met through other incurred medical charges for a particular month, that amount will be deducted from your wages.

If PPL deducts money from your wages for spend-down, PPL will send you a statement with your paycheck listing the amount deducted for the spend-down. You will then need to collect the amount of the spend-down directly from your employer, who is responsible for paying you the amount of the spend-down.

Attention Provider

Legislation signed by the President on August 10, 2010 repeals the Advanced Earned Income

Tax Credit. Recipients will not be able to claim Advance EITC after December 31, 2010.

Due to this legislation, we are unable to process any requests for advanced EIC. Please note

that the Earned Income Credit still exists. It is just that you can no longer request this credit

in advance in your paycheck. It can be claimed on Form 1040 when you file your 2011

income tax paperwork.

IRS Publication 596 explains the Earned Income Credit in great detail. Please refer to this

document for additional guidance. If further clarification on this legislation is needed, please

contact a tax professional.
