CHWs motivate and support patients to achieve their health goals, such as helping them connect to a primary care provider, creating health goals, and help or assisting with finding affordable healthy food options. CHWs are assigned to work in a H+H facility, and also perform community-based work, such as conducting home visits within NYC areas. CHWs may be activated in future public health emergencies. CHWs will work with a specific patient population: Adult Primary Care; Pediatrics Primary Care; Behavioral Health; or Special Populations (individuals experiencing homelessness, foster families or formerly incarcerated people). What is a Community Health Worker (CHW)? Community Health Workers must meet these job qualifications: 1. Three years of full-time experience in a government agency or community organization in providing community services to the public or in assisting members of the community in obtaining community services; or 2. Education and/or experience equivalent to “1” above. Study at an accredited college in sociology, psychology or other behavioral science may be substituted on the basis of 30 semester credits for each year of the experience described above. However, all persons must have at least one year of the full-time experience described above. *All qualified individuals, including Contact Tracers, Optum Monitors, and Resource Navigators are encouraged to apply.
