Criteria which might be included include: (i) disease including histological or cytological confirmation and stage; (ii) allowable type and amount of prior therapy; time since last treatment; (iii) age restriction, if any; (iv) performance status and scale; (v) life expectancy; (vi) any organ or marrow function requirements; (vii) any laboratory parameter requirements; (viii) willingness to use contraception as required; (ix) any additional inclusion criteria that are appropriate to the study.

4.2 Exclusion Criteria

A potential subject who meets any of the following exclusion criteria is ineligible to participate in the study.

Criteria which might be included include: (i) receiving any other investigational agents; (ii) brain metastases, if a factor; (iii) history of allergic reactions to any of the required agents on the study; (iv) concomitant medications or substances that have the potential to affect the activity or pharmacokinetics of the study agent; (v) uncontrolled or intercurrent illness that would interfere with achieving the study objectives;(vi) pregnant; (vii) HIV-positive; (viii) any additional exclusion criteria that are appropriate to the study.

5 STUDY ENTRY AND WITHDRAWAL PROCEDURES

Subject withdrawal criteria (i.e. terminating investigational product treatment/trial treatment) and procedures for specifying: (a) when and how to withdraw subjects from the trial/investigational product treatment; (b) the type and timing of the data to be collected for withdrawn subjects; (c) whether and how subjects are to be replaced; and (d) the follow-up for subjects withdrawn from investigational product treatment/trial treatment. 5.1 Study Entry Procedures

5.1.1 Required Pre-Registration Screening Tests and Procedures

Refer to section 11, STUDY CALENDAR, for the screening tests and procedures that are required prior to registration, and for the timing of these events relative to the start of treatment. (sample language)

5.1.2 Registration Process

Describe the process to be used. Who should be contacted and how? What information is required (ICF, HIPAA authorization form, copy of required laboratory tests, eligibility screening worksheet, registration form, etc.)? How soon after registration must the subject begin treatment? What are the procedures to be followed if this is a multicenter trial?

5.2 Study Withdrawal Procedures

5.2.1 A patient may decide to withdraw from the study at any time.

5.2.2 A patient may be removed from treatment for one of the following criteria: (The 2 broad categories are as follows, although the first is frequently made more specific as indicated below.)

5.2.2.1 If in the opinion of the treating physician, it is in the best interest of the patient to do so. (This is a generic statement that some investigators prefer.

You may want to include more specific criteria such as (i) unwillingness or inability of the patient to comply with the protocol requirements; (ii) disease progression; (iii) intercurrent illness that prevents further administration of treatment; (iv) unacceptable adverse events (AE[s]); and (v) general or specific changes in the patient's condition render the patient unacceptable for further treatment in the judgment of the investigator.)

5.2.2.2 Sponsor’s decision to discontinue the study.

5.2.3 The reason for withdraw from the study and the date the patient was removed from the study must be documented in the case report form (eCRF/CRF [choose one]).

6 TREATMENT PLAN

A description of the trial treatment(s) and the dosage and dosage regimen of the investigational product(s). Also include a description of the dosage form, packing, and labeling of the investigational product(s). [GCP 6.4.4] (See also section 9, PHARMACEUTICAL INFORMATION.)

The expected duration of the subject participation, and a description of the sequence and duration of all trial periods, including follow-up, if any. [GCP 6.4.5]

The treatment(s) to be administered, including the name(s) of all the product(s), the dose(s), the dosing schedule(s), the route/mode(s) of administration, and the treatment period(s), including the follow-up period(s) for subjects for each investigational product treatment/trial treatment group/arm of the trial. Indicate any tests or procedures that are necessary to confirm that a subject still meets the eligibility requirements, or that are required at baseline for later outcome comparison after study intervention. Alternatively, indicate that this information is located in section 11, STUDY CALENDAR.

6.2 Investigational Agent Administration (or other appropriate title; “Radiation Therapy” might be appropriate for a radiation oncology protocol)

**’*PHASE 1 STUDIES***

State the starting dose of the investigational agent and describe the dose escalation scheme and treatment regimen (dose, route, duration of infusion for intravenous drugs, and schedule). Use exact dose rather than percentages. State any special precautions or warnings relevant for agent administration (e.g., incompatibility of agent with commonly used intravenous solutions, necessity of administering agent with food, pre-medications, etc.). A table may be used to describe the regimen.

***PHASE 2 STUDIES***

Describe the regimen (agent, dose, route, duration of infusion for intravenous drugs, and schedule) and state any special precautions or warnings relevant for investigational study agent administration (e.g., incompatibility of the agent with commonly used intravenous solutions, necessity of administering agent with food, pre-medications, etc.). Inclusion of a treatment table may be useful in clarifying the regimen.

***INTERVENTIONAL STUDIES INVOLVING OTHER THAN A STUDY DRUG***

If the primary intervention involves something other than a study drug, a different header will most likely be more appropriate for this section. As an example, Radiation Therapy” may more accurately reflect the intervention. The “Additional Treatment Modalities” section below might then be used to describe the concomitant use of other treatment modalities.

6.3 Definition of DLT and MTD (applicable only for phase 1 trials)

Please provide explicit definitions of the types, grades, and durations of all AEs that will be considered dose-limiting, or provide definitions of other endpoints that will be used to determine dose escalations. A classical 3+3 design is illustrated below, but other designs may be used at the discretion of the Investigator. What follows is sample language that can be included in addition to that indicated in this paragraph.

Management and dose modifications associated with the above AEs are outlined in section 7, DOSING DELAYS/DOSE MODIFICATIONS.

Dose escalation will proceed within each cohort according to the following scheme. DLT is defined above.

This is generally the recommended phase 2 dose. At least 6 patients must be entered at the recommended phase 2 dose.

6.4 Additional Treatment Modalities (if applicable)

Provide a detailed description of any other modalities (e.g., surgery, chemotherapy, radiotherapy) or procedures (e.g., hematopoietic stem cell transplantation) used in the protocol treatment.

6.5 General Concomitant Medication and Supportive Care Guidelines (if applicable)

Describe in detail any prophylactic or supportive care regimens required for investigational study agent(s) administration and state any special precautions or relevant warnings (e.g., incompatibility of agent with commonly used intravenous solutions, necessity of administering agent(s) with food, pre-medications, etc.). Provide the same information for any other agent used in the study.

State guidelines for use of concomitant medications or any additional appropriate supportive care medications or treatments. The potential for interaction with the cytochrome P450 system should be addressed if applicable.

6.6 Duration of Therapy

Describe the number and duration of cycles of therapy. Describe if and how subjects are replaced, if applicable. Reference to the study calendar may be made.

Describe the procedures to be used to monitor subject compliance such as the use of a study diary.

6.8 Follow-Up Period

8.1 Definitions

8.1.1 Adverse Event (AE)

AE means any untoward medical occurrence associated with the use of a drug in humans, whether or not considered drug related.

8.1.2 Suspected Adverse Reaction (SAR)

Any AE for which there is a reasonable possibility that the drug caused the AE. “Reasonable possibility” means that there is evidence to suggest a causal relationship between the drug and the AE.

An AE with an attribution of possible, probable, or definite (see section 8.3.3 below) is a SAR.

8.1.3 Serious AE (SAE) or Serious SAR (SSAR)

An AE or SAR is considered “serious” if, in the view of either the investigator or sponsor, it results in any of the following outcomes:

death,

a life-threatening AE (An AE or SAR is consider “life-threatening” if, in the view of either the investigator or sponsor, its occurrence places the patient or subject at immediate risk of death. It does not include an AE or SAR that, had it occurred in a more severe form, might have caused death.),

inpatient hospitalization or prolongation of existing hospitalization,

a persistent or significant incapacity or substantial disruption of the ability to conduct normal life functions, or

a congenital anomaly/birth defect.

Important medical events that may not result in death, be life-threatening, or require hospitalization may be considered serious when, based upon appropriate medical judgment, they may jeopardize the patient or subject and may require medical or surgical intervention to prevent one of the outcomes listed in this definition.

8.1.4 Unexpected SAR

A SAR is considered “unexpected” if

it is not listed in the investigator brochure or is not listed at the specificity or severity that has been observed;

or, if an investigator brochure is not required or available, is not consistent with the risk information described in the general investigational plan or elsewhere in the current application, as amended.

“Unexpected” as used in this definition, also refers to SARs that are mentioned in the investigator brochure as occurring with a class of drugs or as anticipated from the pharmacological properties of the drug, but are not specifically mentioned as occurring with the particular drug under investigation.

If the investigator/research nurse/CRA do not plan to reference the investigator brochure or general investigation plan to determine if a SAR is unexpected, then a comprehensive listing of SARs needs to be placed within the protocol so that the determination can be made without reference to the external documents.

8.1.5 Unanticipated Problem (UP)

Unanticipated problems include any incident, experience, or outcome that meets all of the following criteria:

unexpected (in terms of nature, severity, frequency) given (a) the research procedures that are described in the protocol-related documents, such as the IRB-approved research protocol and informed consent document; and (b) the characteristics of the subject population being studied;

related or possibly related to participation in the research (possibly related means there is a reasonable possibility that the incident, experience, or outcome may have been caused by the procedures involved in the research); and

suggests that the research places subjects or others at a greater risk of harm (including physical, psychological, economic, or social harm) than was previously known or recognized.

8.1.6 AE Description and Grade

The descriptions and grading scales found in the revised Common Terminology Criteria for Adverse Events (CTCAE) version 4.0 will be utilized for AE reporting.

8.1.7 AE Expectedness

The time period over which AEs will be captured should be specified, as well as the grade of the adverse events to be captured. The period of capture is often defined as running from the beginning of any study procedures through 30 days following the end of the study treatment.

Describe the procedures by which AEs, SAEs, and UPs will be recorded including the time points and method of recording (such as using CRFs).

14.1 Study Team

This section should be included for all studies. The following language is tailored for a study that involves more than minimal risk. It should be modified as necessary to meet the needs of a particular study.

The study team minimally consists of the principal investigator, the research nurse, the clinical research associate, and the study biostatistician. While subjects are on treatment, the principal investigator, the research nurse, and the clinical research associate will meet at least monthly, and will meet at least quarterly with the study biostatistician, to review study status (attendees and time periods should be modified so as to make sense within the context of the study). This review will include, but not be limited to, reportable AEs and UPs, and an update of the ongoing study summary that describes study progress in terms of the study schema. The appropriateness of further subject enrollment and the specific intervention for a next subject enrollment are addressed. All meetings including attendance are documented.

14.2 Audit Committee

This trial will be audited by the MCC Audit Committee.

15.4 Informed Consent Process

Informed consent is a process that is initiated prior to the individual’s agreeing to participate in the study and continues throughout the individual’s study participation. Extensive discussion of risks and possible benefits of this therapy will be provided to the subjects and their families. Consent forms describing in detail the study interventions/ products, study procedures, and risks are given to the subject and written documentation of informed consent is required prior to starting intervention/administering study product. Consent forms will be IRB-approved and the subject will be asked to read and review the document. Upon reviewing the document, the investigator will explain the research study to the subject and answer any questions that may arise. The subject will sign the informed consent document prior to any procedures being done specifically for the study. The subjects should have the opportunity to discuss the study with their surrogates or think about it prior to agreeing to participate. The subjects may withdraw consent at any time throughout the course of the trial. A copy of the informed consent document will be given to the subjects for their records. The rights and welfare of the subjects will be protected by emphasizing to them that the quality of their medical care will not be adversely affected if they decline to participate in this study.

15.5 Subject Confidentiality and Access to Source Documents/Data

Subject confidentiality is strictly held in trust by the sponsor/sponsor-investigator (choose one as appropriate), participating investigators, and any staff, [and the sponsor(s) and their agents] (include bracketed portion if applicable). This confidentiality includes the clinical information relating to participating subjects, as well as any genetic or biological testing.

16.1 Data Management Responsibilities

Describe responsibilities for data handling and record keeping as they specifically relate to the sponsor, clinical site, laboratory, and data coordinating center. Information should include the role in data collection, review of data, trial materials, and reports. Sample text for a multi-site study involving an IND is given below (modify accordingly to meet the needs of a particular study):

The sponsor/sponsor-investigator is responsible for: (i) reviewing SAE reports; (ii) determining if SARs need to be reported to the FDA, and if so, filing the report; (iii) filing annual IND reports; and (iv) filing IND amendments.

The responsible investigator at each site is responsible for: (i) the data management at his or her site; and (ii) reporting SAEs, UPs, and DLTs as required in section 8.

Any laboratory conducting correlative studies must maintain the laboratory records and documentation (laboratory notebooks, laboratory protocols, print-outs, recordings, photographs, etc.).

16.2 Source Documents

Provide a description of the source documents which include all information, original records of clinical findings, or other activities in a clinical trial necessary for the reconstruction and evaluation of the trial. Sample text below:

Source documents for clinical information (patient history, diagnosis, clinical and diagnostic test reports, etc.) are maintained in the patient’s clinical file. Source documents for the correlative studies are maintained in laboratory conducting the study.

16.3 Case Report Forms (or other appropriate title to reflect the study-specific manner in which protocol-specific data will be recorded)

MCC Oncore data management will provide standard electronic case report forms (eCRFs) and create study-specific eCRFs to capture all the information required by the protocol. The eCRFs will be approved by the study team to ensure the most effective data acquisition. All information on eCRFs will be traceable to the source documents which are generally maintained in the subject’s file. All eCRFs should be completed and available for collection within a timely manner, preferably no more than 14 days after the subject’s visit.

If the study does not involve eCRFs, a description of the manner in which the data will be recorded and maintained should be provided. If handwritten documents will be maintained, the following text can serve as a model for data entry.

All entries on the case report forms (CRFs) should be made legibly in black ink. Errors, when made, should be corrected by drawing a single line through the incorrect entry (do not erase, white-out, or tape over errors) and then entering the correct data above the original entry. Entry corrections should be initialed and dated. Explain missing data with “ND” for “not determined” and “NA” used for “not applicable.”

16.4 Study Record Retention

Specify the length of time for the investigator to maintain the records pertaining to this study. The 5 year period in the following paragraph is based on the “VCU Research Data Ownership, Retention, and Access Policy.” Research data collected for product application to the FDA may be subject to additional data retention requirements as specified by the sponsor and/or the FDA.

Medical treatment is available at the Virginia Commonwealth University Health System (VCU Health System). Fees for such treatment may be billed to you or to appropriate third party insurance. Your health insurance company may or may not pay for treatment of injuries or illness as a result of your participation in this study.

To help avoid research-related injury or illness it is very important to follow all study directions.
