Beauty and Dentistry

In the world of medicine and dentistry, the relationship with patients is changing radically. Patients are beco-ming more like clients, paying great attention to the tre-atments and services offered, as well as to quality and costs. Their knowledge is increasing thanks to the internet and the many communication and information tools now available. Dentistry now needs to improve communica-tion with patients, and offer more clarity and a closer re-

lationship. Research shows that the dentist has become a figure linked with the concept of “pain” - a syndrome passed down through the generations and alive even to-day. Dentistry has always been viewed as a “high added value” profession focussed mainly on the actual dentistry work and this view does not include the social aspects which surround people in their daily lives such as “well-being”, and enjoying their quest for health and beauty. This is one of the aspects of the Tecno-Gaz plan which

links the high added value of professional dentistry with cosmetic medicine and beauty treatments. So, several aspects of the dentistry profession must evolve radically:

• Rationalize the organisation of the surgery, with the aim of increasing expertise, optimizing management costs and properly managing the assistants’ work.

• Increase the services on offer, that is, not only sim-ple treatments but also complementary services such as dental hygiene and product sales. View beauty in a general sense, extending the focus from the mouth to include the face, as well as consultations on gene-ral prevention.

• Introduce client loyalty policies and use telemar-keting to measure satisfaction levels and bring the surgery to patients’ notice.

• Increase communication for higher visibility; adver-tise services, treatments and medical expertise.

• Diversify services on offer while maintaining the required medical image. Services should be extended within the sphere of beauty and well-being - aspects that are very current in our modern society.

Like all businesses, dentistry must change with the times and plan to offer less “traditional” services. Many surgeries are moving towards extending the pro-fessional services they offer. It is important to remember that a smile is everyone’s calling card and lips, teeth and wrinkles around the mouth form part of a person’s overall image. Teeth are the most important reflection of a person’s well-being and personal care. And so, it is clear that dentistry must be at the heart of this new and important business.

Making the most of new opportunities

Anti-ageing treatment for their smile is an important objective for everyone who cares about their looks. Dentists offering this service not only expand their business but also create client loyalty particularly through “pampering” treatments. In this way, a den-tist is not only the professional who treats a patient’s mouth concentrating exclusively on cosmetic denti-stry and teeth whitening, but is also able to revitali-

se the lips, the skin around the mouth and the face. These are non-invasive treatments that complement a dentist’s work. This trend is emerging in patients’ who want to achieve maximum balance in the be-auty of their smile as well as their face after dental work such as implants, gum surgery and ceramic caps – the face also needs to reflect a patient’s overall well-being and deserves a little attention. Modern

dentistry and also cosmetic dentistry are in harmony with the beauty of a patient’s Face and Smile. Until yesterday, a dentist could work on the beauty of a smile using treatments focussed mainly on the teeth. Now Tecno-Gaz can help dentists to include cosmetic medicine techniques in their work to achieve unex-pected results, bringing this profession towards new concepts of “Wellbeing of the Face and the Smile”.

A revoLUtionAry trend

weLL-Being As An oBjective

Dentistry has always focussed exclusively on the area inside the mouth and avoided involvement in the treatment of surrounding areas such as lips, face and neck. This allowed other professionals to exploit services linked to cosmetic medicine - an area which is still growing rapidly. Maintaining health and beauty through specific and personalized treatments, and the prevention and treatment of skin ageing, is one of the largest areas of intervention.

The great opportunity today is being able to use cosmetic fillers, equipment and products that give great results without invasive appli-cations, thereby minimizing responsibility and eliminating problems. New regulations and governmental guidelines limit the use of the-se devices to medical or specialized profes-sionals but soon new opportunities will arise where dentistry can also play a part.

Choosing your partnerto stArt this BUsiness

To undertake a new project, we must have clear ideas and cooperate with specialized companies that, above all, have extensive knowledge of the dentistry market. Knowing the sector is extremely important as the plan must be placed into the correct medical-dentistry context and be maintained at a highly professional level.

After preparing the work space, thought needs to be given to training. Tecno-Gaz SpA can set up a well-defined programme for learning the basic princi-ples and obtaining all the information required to work correctly and successfully. Another interesting aspect is communication: in fact, the success of the

FACE&SMILE project in dentistry is also linked to the ability to communicate. Planning communication is an integral part of the project. Good communica-tion can achieve excellent returns, not only in relation to the beauty project but for the surgery overall.

Organising the business

Planning the beauty project within a dental surgery is a very important moment as a new operational and communications structure needs to be organised.

Advertising this business

In a modern society, communication is at the heart of the success of every business, even for professional and medical businesses. Recent deregulation now allows any professional or medical structure to advertise its business within normal ethics rules. This freedom allows these businesses to make their own structures and the services offered more visible.

After preparing the work space, thought needs to be given to training. Tecno-Gaz S.p.A. can set up a well-defined programme for learning the basic princi-ples and obtaining all the information required to work correctly and successfully. Another interesting aspect is communication: in fact, the success of the FACE&SMILE

project in dentistry is also linked to the ability to communicate. Planning communi-cation is an integral part of the project. Good communication can achieve excellent returns, not only in relation to the beauty project but for the surgery overall.

Ideas, tools, experience Planning the beauty project within a dental surgery is a very important moment as a new operational and communications structure needs to be organised.

Medical advertising should, in our opinion, be informati-ve and socially aware but retaining a medical slant, easy to understand, highlighting the advantages and the re-sults and giving useful advice. Traditional dentistry is dif-ficult to advertise, as it is seen as an indispensable tre-atment but particularly traumatic. Advertising white and

healthy teeth, a smile, the beauty of the face and mouth is, on the other hand, a positive message, easily received by the public and brings a large number of people to the surgery. A dental surgery then assumes a less “traumatic” and more positive image and consequently also dental care and prevention treatment can be accepted more

calmly and with more understanding by the patient. The Face&Smile project also becomes an added value that goes beyond the merely practical and visual to become a strategic point in the visibility and development of eve-ry dental surgery that wants to grow and adapt to future market dynamics.

Hyperbaric oxygen at appropriate pressure levels can be used in cosmetic medicine in beauty treatments for the skin. The Oxis range of products uses hyaluronic acid with a fractioned molecular weight for higher effectiveness and, as a leader

in marine cosmetology research, also uses Pelvetia Canaliculata (an excellent active ingredient for combating skin ageing). This synergy leads to non-surgical treatments, without the use of injections and painless.

Numerous studies have looked into the problem of the skin’s permeability. In transdermal transfer, high density molecules (of high molecular weight) are transported leading to many benefits:

• Changes in permeability

• Activation of the protein channels

• Definition of the wave types for transferring water soluble substances

ERA optimizes the methods for transferring active ingredients overcoming the limits of past techniques: these were their low concentration, the limited depth reached in the tissues, and tissue damage caused by the intense currents induced. For these reasons, this method is state-of-the art in skin care and treatment, particularly in the treatment of cellulite: New, recent studies carried out in various countries have shown excellent results, highlighting the future of this new technique which eliminates the use of needles.

THE AGEING PROCESSAs age advances, it is this turnover that inevitably slows down, and the skin’s quality and speed of renewal tends to progressively deteriorate: at the age of 20, the skin renews completely every three weeks but at the age of 50, this process can take up to nine weeks. This is where oxygen therapy comes into play: cells tend naturally to lose their fight against ageing as the years go by, through stress, illness, excessive exposure to sun and pollutions. These factors lead to deterioration in the micro-capillaries that feed the skin, thus reducing the quantity of oxygen that the blood carries to the tissues. Topical oxygen treatment significantly increases the amount of oxygen transported to the tissues and compensates this deficiency, revitalizing the skin’s reconstruction processes through the natural action of the oxygen.

The technology applied in the cosmetics field is evolving rapidly with new offers constantly appearing on the market. The general trend seems to be towards techniques that are as non-invasive as possible. In the sphere of skin rejuvenation, the latest innovation is the use of hyperbaric oxygen, widely advertised by famous international stars that have used it intensively with very appreciable results.

Hyperbaric oxygen therapy

HYPERBARIC GASThis can produce efficient metabolic acceleration at tissue level and efficiently transfer specific molecules through the skin’s layers. This is, therefore, a non-invasive technique which on the one hand, naturally stimulates tissues and, on the other, facilitates absorption of the anti-ageing products prescribed by doctors in the individual cases diagnosed.

PERCUTANEOUS INFUSIONHyperbaric oxygen technology works through so-called percutaneous infusion. The machine can extract oxygen from the surrounding air, concentrate it and bring it to a pressure higher than atmospheric. The gas produced in this way comes into direct contact with the skin through a special apparatus that allows the operator to regulate the intensity of the therapy. Hyperbaric oxygen is of great benefit to all skin pathologies where the reconstruction process plays an important role.

Oxygen technology

• Oxygenates tissues improving their functionality

• Perfects skin tone and thickness and illuminates the skin

• Reduces dynamic and static wrinkles

• Improves firmness and elasticity

After BeforeTHE RESULTSThe most striking result of hyperbaric oxygen therapy is the significant reduction of wrinkles giving more tone, elasticity and thickness to the skin (stimulation of collagen).

HYALURONIC ACIDHyaluronic acid is one of the basic components of human connective tissues. Its concentration in body tissue tends to diminish with age, thus weakening the skin and causing the formation of imperfections. The use of hyaluronic acid in cosmetics is important as it gives the skin resistance and helps it maintain its shape, eliminating wrinkles and preventing skin ageing.

PELVETIA CANALICULATA

Pelvetia Canaliculata is an algae found in Brittany. Because of its small size, it must be harvested by hand and is processed when fresh. Its active ingredients – fucans - improve microcirculation and skin elasticity. Extremely hydrating, Pelvetia Canaliculata improves vascularisation and micro-circulation and protects capillary walls.

ELECTROPORATIONThis method is used in medical therapy exploiting its invasive ability to increases the permeability of skin tissue. Consequently the lipid layer of the cellular membrane changes: it forms protein channels called “electropores”.

These electropores form after induction of the electrical impulse. This usually takes a few minutes thus allowing molecules, even larger ones, to permeate through all the tissue targeted by the impulse. These channels remain for a period of time directly proportional to the wave length of the impulse itself: this time period generally stretches from a few seconds to ten or more minutes. In particular, what occurs has been demonstrated recently in the chemical/biological field: the famous research on “Water Transport in Cells”, which won the Nobel prize for Chemistry in 2003 for two American scientists, R. MacKinnon and Peter Agre. In fact, this research discovered the presence of molecular channels that allow or block access through the cellular membrane of indispensable substances such as water and salts (membrane proteins).

The control for the exchange of ions or molecules through these structures is regulated by chemical signals that allow communication between various cells. These signals are ions or small molecules that open a series of “cascade reactions” inside the cell, and lead to macroscopic manifestations such as, for example, muscle tension and biochemical or metabolic reactions in our body or brain. On the basis of the above, with the methodology perfected in ERA - ISOFORESIS - water-soluble molecules can be transferred through the epidermis, a barrier that has a very low permeability for water soluble substances due to its extra-cellular lipid matrix (ceramides, cholesterol, fatty acids) allowing active pharmaceutical agents to be introduced at different depths.

Era EraTHE TRANSDERMAL TRANSPORTER

The electronic transdermal method has now been in use for several decades, starting with the first medical uses for administration of pain relief and in rehabilitation therapies, and has recently been applied in the field of dermatology and in professional cosmetic techniques. The main barrier against administering substances through human skin is its most external layer - the corneous layer - a tenacious barrier against the transfer of substances through the skin.

Alteration in the skin’s impedance under electrical impulse and consequent variations in its permeability;

Creation of a wave shape generator;

Definition of the wave shape that can be used for transportation.

ELECTROPORATION

ERA uses a pulsating and modulated current: an elastomodulated wave shape is emitted at low frequency and the modulated intensity can be altered by the operator. Its shape and particular modulations activates molecules of both low and high molecular weight electronically, letting them pass into the extra-cellular compartment at depths never before achieved: Up to 9-10 cm and introducing 90% of the active substances required in the treatment.

A DOUBLE OBJECTIVE IS ACHIEVED IN THIS WAY:

Permeability of skin tissue is increased, even in the deeper layers of the derma

Water-soluble active substances of high molecular weight (e.g. hyaluronic acid, collagen, Vitamin C, etc.) are introduced.

Techniques used Techniques used

ERA IS DIFFERENT FROM OTHER SYSTEMS FOR TWO IMPORTANT REASONS:

• Wave modulation: Non-continuous, negative/positive or alternated, allowing the product to be transferred without heat damage up to several centimetres. Wave modulation overcomes the maximum absorption limit of human tissue (0.1 mA/cm2) given by the heat recovery time in the off phase. The systems allows the automatic adjustment of the wave frequency in relation to the set penetration depth.

trAnsportAtion eLectroporAtionioniZAtion

After about 12-13 minutes electropores are created: The molecules transfer substances by osmosis from one cell to the other.

The ionized molecule is transferred to the optimal depth.

The elastopulsated wave meets the molecule which is then ionized and rendered “active”, ready to be transported.

• The apparatus is equipped with an ionization chamber that polarizes and electrically arranges molecules and no longer ions, so that any water soluble substance can be transported through the opening of the so-called “electropores” and assisting intracellular penetration; the “undissociated” molecules actively carried into the cells modulate the metabolism according to their characteristics and concentration and move, through electro-osmosis, towards the dermal matrix. In summary, during ISOFORESIS, the transportation process occurs through a synergy of progressive processes:

Face&Smile Beauty of the face: a new market in dentistry

FASE 1Peeling/exfoliation using Bright Skin

ERA combines another method that exploits the value of the isoforesis treatments to the full: Ultrasound and galvanic current for Bright Skin. This equipment is patented and the protocols registered.

Bright skin is an ultrasound device that rejuvenates the skin and prevents ageing of the tissues through the use of ultrasonic waves: It consists of a thin metal spatula with a vibration frequency of 25,000 KHertz, bent at a 45° angle to distribute ultrasonic waves on the surface under the principle of cavitation.

This phenomenon can be seen in a liquid when it undergoes high depression. When the absolute pressure become lower than the tension of the liquid, an energy field is produced with abundant transformation into vapour of the gas contained in the liquid in the form of micro-bubbles that favour the detachment of the dead cells “by propulsion” and also absorb residues of cosmetics and impurities deposited in the sebaceous follicles. The Bright Skin spatula also emits, depending on the selected programme, galvanic micro-currents to transport substances or for electronic peeling.

This allows the synergy between ultrasound (sonic propulsion) to be combined with galvanic micro-currents (ion propulsion) to best advantage.

Bright Skin

Bright Skin, used as a pre-treatment before ERA, prepares the skin rendering it clean and aseptic and ready to receive the active substances in specific treatments.

The Bright Skin software includes numerous treatment programmes based on the type of skin (3 types) matching the ultrasound as required to the galvanic currents.

Bright Skin

Rebalancing of the metabolism, clarifying action: Micro-circulation is activated with an increase in tissue oxygen, stimulating the production of collagen and elastin through micro-massage by the elastopulsating wave. Moreover, when used with a specific product, melanin discolorations are reduced and anti-oxidant effects are enhanced.

Firming of the skin tissue and cellular stimulation using electronic peeling, protocols and galvanic currents combined with ultrasound.

Exogenous and endogenous moisturisation: The active ingredients are micronized and transferred in depth using ultrasound, also through osmosis activated by the compression and decompression of the interstitial liquids, leading to excellent and deep hydration of the skin.

Peeling/exfoliation of dead cells from the skin’s surface and deep cleansing of the skin to remove various substances and pollutants (smog, cosmetics, bacteria, etc.): Sonic cavitation removes the horny layer and upper layer of dead cells, making the younger corneous layer more visible (cellular turnover) and, together with the emission of galvanic micro-currents that have an important ionic action, making the skin more toned and luminous.

The apparatus and ionization chamberThe ionization chamber is located inside the apparatus and is a real innovation:here, the processes of osmosis and conductivity of the substances gradually take place. Firstly, the active moisturizing substances (in powder form) are mixed in the apparatus with a conductive gel prepared specifically for ERA. It is soluble and transports the ionisable active ingredients. It does not contain any preservatives. In the ionization chamber, built with fixed metal frames, the mixture undergoes fixed and constant induction by the elasto-modulated current. In fact, the apparatus is structured so that the ionization chamber frames are fixed and constantly in contact with the mixture ensuring continuous ionization of the active ingredients to be transported.

Apparatus and treatments

ERA offers mesotherapy treatments without the use of needles, painless and without bruising, using low level currents with elasto-modulated waves. The ERA apparatus is electrically charged and, moving across the area to be treated, pushes the ions to a deep level, without damaging skin tissue. The ingredients actively penetrate at the moment of change in the cutaneous lipids created by the specific electric impulse and reach the target cells. The cells exchange ionized ingredients through channels known as aquaporins.

FASE 2Transportation of the active ingredients using the special apparatus

The ERA apparatus has been designed for

• A gradual and non-chaotic transportation of activated substances. Its metal rollers create electroporation in the skin, constantly releasing the mixture, adhering perfectly and always at the same distance.

• No dispersion and nor spikes in concentration, always ensuring a constant level of electric energy and therefore of ionized substances.

• Moreover, the apparatus guarantees the high level of hygiene required by health protocols as its stainless steel components can be removed and are all washable and sterilizable.

Finally, ERA treatments combined with ultrasound are normally carried out as follows:

• In the first phase, ultrasound/Bright Skin for skin cleansing treatments.

• In the second phase, modulated waves (Isoforesis)/ERA for skin penetration, where sequential wave shapes can penetrate 90% of the active substances.

IMMEDIATE AND DEEP REJUVENATION

Electroporation treatments are immediately effective and visible. The skin is more hydrated and relaxed, wrinkles and lines less deep, and the improvement in the skin’s tone gives a more youthful and fresh appearance. Electroporation improves the skin’s functions and leads to rapid reduction in imperfections.
