scandals in recent years were caused by the unethical practices of skillful, experienced and well-established professionals:

10 Major Accounting Scandals – BCCI in 1990s, ENRON CORP, WORLDCOM, KANEBO LTD, PARMALAT, AIG, SATYAM COMPUTER SERVICES, TYCO INT’L,WASTEMGT. INC., HEALTH SOUTH CORP.

They have Codes of Ethics and Professional Codes, but they still committed fraud, etc.
