About the ASCP Board of CertificationFounded in 1928 by the American Society for Clinical Pathology

(ASCP), the Board of Certification is widely accepted as the most

influential leader in the field of certification of medical laboratory

professionals. To date over 490,000 individuals have been

certified. The Board of Governors of the ASCP Board of

Certification has 23 members:

• Five pathologists (nominated by the ASCP); including onemember of the American Society of Hematology

• Five laboratory professionals (nominated by the ASCP); includingone member of the American Association for Clinical Chemistry

• Four representatives from the American Society for ClinicalLaboratory Science (ASCLS)

• Two representatives from the Association of GeneticTechnologists (AGT)

• One representative from each of the following six additionalparticipating specialty societies:

• AABB; American Association of Pathologists’ Assistants;American College of Microbiology; American Society ofCytopathology; Clinical Laboratory Management Association;National Society for Histotechnology

QualificationWhat are the advantages of completing a Qualification?

• Visible recognition of specific skills in a technical area with theQualification initials after the ASCP credentials

• Recognition by state licensure—the state of Florida recognizesthose individuals certified and qualified as HT(ASCP)QIHCtoward meeting the specialist licensure requirements forhistology

• More job possibilities—career mobility throughout the country

• Professional growth—keeping up with state of the arttechnology

To receive this credential, candidates must meet the eligibilityrequirements and successfully complete a work sample project(QLI) or examination (QCYM, QIHC, QLS). Candidates whocomplete the Qualification process, will receive a Certificate ofQualification which is valid for three years.

The Qualification may be revalidated every three years uponpayment of a fee and completion of acceptable continuingeducation related to the area of Qualification. The Qualification willnot, in itself, entitle the individual to ASCP membership.Application forms are located at www.ascp.org/qualification.

Qualification Process Step 1: Meet the eligibility requirements for the appropriate

Qualification before submitting your application.

Step 2: Complete the appropriate application form (see page 9) andsubmit the following:

• Completed application form

• Appropriate application fee (check or credit card) or PayPalApplication fees are non-refundable

• Necessary documentation required to verify your eligibility(see pages 7-8).

Step 3: Receive notification by email to take the Qualificationexamination upon determination of your examinationeligibility.

Step 4: QCYM, QIHC, and QLS: Schedule and successfullycomplete your self-administered examination at the dateand time of your choice within the 60 day time periodindicated on your Admission Notification.

QLI: Upon eligibility you will receive electronically the QLIProject Booklet. Your QLI Project must be electronicallyreturned to the BOC office within 90 days.

Step 5: Receive your examination scores. QCYM, QIHC, and QLS:receive your final examination score and certificate withinten (10) business days of the date of your examination. QLI: Allow three (3) months for scoring, reporting andcertificate delivery.

Qualification Categories and Application Fees Application fees are non-refundable. Be sure you meet theeligi bil ity requirements as stated and are able to provide theappropriate documentation before submitting your applicationform and fee.

The Board of Certification does not establish eligibility of anycandidate from information that is supplied via e-mail,correspondence, fax, telephone calls or online chats alone. Ouroffice must base all decisions on a review and verification ofinformation supplied through formal application for examination.

Qualification in Cytometry, QCYMTo qualify, you must meet one of the following requirements:

Route 1: ASCP technologist/scientist or specialist certification,AND six (6) months full time acceptable experience in cytometryin the U.S., Canada or an accredited laboratory* within the lastfive years; OR

Route 2: ASCP technician certification (HT, MLT), AND twelve(12) months full time acceptable experience in cytometry in theU.S., Canada or an accredited laboratory* within the last fiveyears; OR

Route 3: Baccalaureate degree or higher from a regionallyaccredited college/university, AND eighteen (18) months full timeacceptable experience in cytometry in the U.S., Canada or anaccredited laboratory* within the last five years.

* CMS CLIA certificate of registration, compliance, accreditation; AND/ORCAP, AABB, Joint Commission accreditation; ORJCI accreditation; ORAccreditation under ISO 15189

To fulfill the experience requirements for the Qualification inCytometry, you must have experience in immunophenotypingand other applications, cytometric analysis, and qualityassurance as described below.

Immunophenotyping and Other Applications Experience is required in at least two of the following:

• Immunodeficiencies (i.e., CD4, HIV)• Immunoproliferative disorders (non-neoplastic and

neoplastic conditions)• Transplant• DNA Ploidy Analysis• Red Blood Cell Analysis (i.e., PNH, fetal hemoglobin)• Progenitor cells• Other applications; (to be specified by applicant)

Cytometry AnalysisExperience is required in all of the following:

• Specimen processing• Instrument set-up• Specimen analysis• Data management• Interpretation of results

Quality AssuranceExperience is required in all of the following:

• Specimen collection, processing, storage• Reagent selection, preparation, storage, disposal• Assay selection, validation, documentation• Instrument operation and maintenance• Quality control and proficiency testing• Safety

Successful completion of an online examination is required.

Qualification inImmunohistochemistry, QIHCTo qualify, you must meet one of the following requirements:

• Quality Assurance

The applicant should have participated in Quality Assurancerelated to ALL of the following:

• Method selection, validation, documentation• Quality control• Reagent selection, preparation, storage, disposal• Safety• Specimen fixation, processing, microtomy

Direct performance, training and/or management/supervision ofIHC procedures are acceptable in meeting the experiencerequirements.

Successful completion of an online examination is required.

Qualification in LaboratoryInformatics, QLITo qualify, you must meet one of the following requirements:

Route 1: ASCP technologist/scientist or specialist certification, ANDa baccalaureate degree from a regionally accredited college/university, AND one year* of full time acceptable experience inLaboratory Informatics in the U.S., Canada or an accreditedlaboratory** within the last four years to fulfill requirements listed; OR

Route 2: MLT(ASCP) or HT(ASCP) certification, AND one year* fulltime acceptable experience in Laboratory informatics in the U.S.Canada or an accredited laboratory** within the last four years, ANDtwo years of full time acceptable clinical laboratory experience in theU.S., Canada or an accredited laboratory**; OR

Route 3: Baccalaureate degree or higher from a regionallyaccredited college/university including 16 semester (24 quarter)hours of biological science with one semester of microbiology, 16semester (24 quarter) hours of chemistry with one semester inorganic or biochemistry, one semester (one quarter) of mathematics,AND one year* full time acceptable experience in LaboratoryInformatics in the U.S., Canada or an accredited laboratory** withinthe last four years, AND two years of full time acceptable clinicallaboratory experience in the U.S., Canada or an accreditedlaboratory**.

* The year experience may include a three (3) month clinical practicum offeredthrough an accredited college/university, AND nine (9) months ofexperience.

** CMS CLIA certificate of registration, compliance, accreditation; AND/ORCAP, AABB, Joint Commission accreditation; ORJCI accreditation; ORAccreditation under ISO 15189

To fulfill experience for the Qualification in Laboratory Informatics, thecandidate must document experience within the last four years inseven of the nine categories below:

• Bidirectional Interfaces: system to instrument (e.g., analysis ofinterface messages)

• Bidirectional Interfaces: system to system (e.g., analysis ofinterface messages)

• Coding and nomenclature systems• Computer system and data quality assurance• Hardware/software change management and quality control

• Informatics system disaster recovery• Policy and procedure development• Security: physical data and administrative control• Software installation, validation and maintenance

Experience is also required in six of the fourteen categories below:

• Autoverification support• Billing and charge capture• Budgeting and cost/benefit analysis• Data mining• Database administration• Hardware installation and maintenance• Information system selection and procurement• LIS user teaching/training• Management Reports• Middleware• Network and data communications• Regulatory compliance for clinical information systems and data• Rule writing and calculations• System analysis

A work project is required.

Qualification in Laboratory Safety,QLSTo qualify, you must meet one of the following requirements:

Route 1: ASCP Board of Certification technologist/scientist orspecialist certification, AND at least six (6) months acceptableexperience in laboratory safety in the U.S., Canada or anaccredited laboratory* within the last five years; OR

Route 2: MLT(ASCP) or HT(ASCP) certification, AND twelve (12)months acceptable experience in laboratory safety in the U.S.,Canada or an accredited laboratory* within the last five years; OR

Route 3: Baccalaureate degree from a regionally accreditedcollege/university, AND eighteen (18) months of acceptableexperience in the U.S., Canada or an accredited laboratory* withinthe last five years.

* CMS CLIA certificate of registration, compliance, accreditation; AND/ORCAP, AABB, Joint Commission accreditation; ORJCI accreditation; ORAccreditation under ISO 15189

To fulfill the experience requirements for the Qualification inLaboratory Safety, the applicant must have experience, within thelast five years, in 4 of the 8 following areas:

• Biohazard control (i.e., blood borne pathogens, infectioncontrol)

• Chemical safety• Ergonomics• Fire safety• First aid• Physical and environmental (i.e., electrical, equipment, spills,

waste management, shipping)• Safety management (i.e., risk assessment, monitoring, safety

committee)• Training and education

Successful completion of an online examination is required.

Official Documents Required

Academic work completed at colleges and universities outside ofthe United States and Canada—the official transcript must beevaluated by an evaluation agency listed in Appendix A.

Only evaluations from these listed agencies will be accepted.Contact the agency directly for information on obtaining anevaluation verifying U.S. degree equivalency. The original ornotarized copy of the evaluation showing degree equivalency shouldbe submitted with the application form.

Verification of Experience Experience Documentation Forms

Required experience must be completed and documented beforeyou will be determined eligible for examination. Only experienceobtained in an accredited laboratory in the United States, in anaccredited laboratory in Canada, or in an accredited laboratory*outside of the US or Canada will be acceptable.

To document your experience:

Step 1: Experience documentation forms (for the appropriateQualification category and route number) must be downloadedand printed from the ASCP website atwww.ascp.org/qualification. If you are unable to download theseforms, please go to www.ascp.org/bocfeedback to request aprinted copy or call 312-541-4999 and ask for Board ofCertification to request printed copies be mailed to you.

Step 2: The experience documentation forms must becompleted by your immediate supervisor or laboratory directorand attached to a letter of authenticity signed by this individualverifying the authenticity of the form. This letter must be printedon original letterhead and state that the experiencedocumentation form was completed by the employer, with thedate and signature. Experience documentation forms will beaudited to verify authenticity.

Step 3: Include the experience documentation form(s) andletter(s) with your application form and fee. A percentage ofthese forms will be audited and verified with your employer.

Documentation Required Eligibility to take a certification examination is established by:

• Meeting the stated minimum requirements for the particularQualification

• Submission of the appropriate application form

• Payment of the appropriate application fee

• Submission of all appropriate documentation including officialtranscripts indicating the appropriate degree

Before your eligibility can be determined, you must submit theappropriate documents verifying that you meet the current ASCPBoard of Certification requirements.

Verification of Academic Education If a college degree is required for the Qualification route of eligibility,you must submit an official transcript or transcript evaluation asdescribed below:

Continuing Education courses are not acceptable in fulfilling theacademic requirements.

Academic work completed at a regionally accredited UnitedStates college/university or an accredited Canadian university—official transcript from the regionally accredited college/university,bearing the embossed seal of the college/university, the signature ofthe Registrar and the date the degree was conferred, must besubmitted to our office in order to determine eligibility.

Regionally accredited colleges or universities are accredited by oneof the following associations:

MSA—Middle States Association of Colleges and Schools

NWCCU—Northwest Commission on Colleges and Universities

NCA-HLC—North Central Association of Colleges and Schools

NEASC-CIHE—New England Association of Schools and Colleges, Inc.

SACS/CC—Southern Association of Colleges andSchools/Commission on Colleges

WASC-ACCJR or ACSCU—Western Association of Schools and Colleges.

Note: A three year baccalaureate degree from a Canadian University is notacceptable. A minimum four year baccalaureate degree is required forALL Qualification categories.

Full and Part-Time WorkExperience Defined Full-time experience is defined as aminimum of thirty-five (35) hours per week.Individuals who have part-time experiencemay be permitted to utilize prorated part-time experience to meet the workexperience requirements.

For example, if you are employed 20hours per week for one year, yourexperience would be computed as 20divided by 35 multiplied by 52 weeks, orthe equivalent of 29.7 weeks of full-timeemployment.

Please contact the ASCP Board ofCertification office prior to applying if youhave any questions concerning part-timeexperience. The number of years ofexperience required must be completedwithin the time frame, specified in therequirements, calculated from the date ofapplication (e.g., application submitted3/1/2013; experience acceptable from3/2003 for a ten-year time frame).

ASCP CertifiedApplicantsAlthough you may be ASCP certified, youmust resubmit all required documentationnecessary to establish your eligibility for theQualification examination. Officialtranscripts must be submitted each timeyou apply.

Step 1: Make sure you have the appropriate application form. Theapplication form is valid for only one category ofqualification. A separate application form must besubmitted for each Qualification for which you apply. Thereare no deadline dates for applications. Applications areprocessed as they are received.

Step 2: Carefully and completely review the eligibility requirementsof the Qualification you have chosen (pages 4-6) to ensurethat you meet all the requirements as specified.Application fees are non-refundable.

Step 3: Carefully follow the instructions on the application form,completing all sections as indicated.

Step 4: If applying by mail, an application fee must be includedwith your application form. All fees must be submitted bycheck or money order in U.S. currency (DO NOT SENDCASH). Make checks payable to “Board of Certification.”The ASCP Board of Certification will not accept post-datedchecks.

If applying by mail, your application will be returned to youif it is not accompanied by a fee. Purchase Orders orVouchers will not be accepted for application fees. A checkor money order must accompany the application form.Faxed applications will not be accepted.

If applying online, payment must be completed by creditcard or PayPal.

Step 5: Download the appropriate experience documentationforms, have your supervisor complete them and attach aletter of authenticity. Include the experiencedocumentation form(s) and attached letter(s) with yourapplication form and fee. Faxed experiencedocumentation forms and letters of authenticity will notbe accepted.

Step 6: Contact your college/university and request that an officialtranscript be mailed to the ASCP Board of Certification atthe following address:

Credit Card Payments for OnlineApplicationsIndividuals submitting online applications for U.S. qualificationexaminations must complete the payment process by credit card orPayPal. Individuals wishing to submit checks or money orders forpayment may download the qualification application form, completethe form and mail it in with a check or money order.

Mailing Addresses Applications and application fees MUST be mailed using theUNITED STATES POSTAL SERVICE REGULAR MAIL ONLY. DONOT send applications and fees by Fax, Federal Express, UPS,Express Mail, Certified or Registered Mail or any overnight courierservice or any other express mail service. Applications andapplication fees using express mail service WILL NOT reach theBOC office.

Application Processing

ACKNOWLEDGEMENT OF APPL ICAT ION

Upon receipt in the ASCP Board of Certification office, yourapplication form and fee will be acknowledged via email. Do notcontact the Board of Certification office regarding receipt of yourapplication and fee until 30 business days following submission.Online applications paid by credit card or PayPal will beacknowledged within one business day.

If you do not receive an acknowledgement within 6 weeks from thedate you submitted your application please notify our office at:www.ascp.org/bocfeedback OR fax 312-541-4845.

Review of Documents All required documentation must be submitted to our office withinforty-five (45) days of submission of your application or you willbe determined ineligible for examination. In order to determineyour eligibility for examination, docu men ta tion verifying completion ofall eligibility requirements must be received in our office. You will benotified within 6 weeks of receipt of your application if anydocumentation is missing.

Determination of Eligibility Upon review of all required documentation, you will be notified ofyour eligibility status. If you are determined eligible for examination,you will receive an email notification with instructions for schedulingan appointment to take the examination.

Ineligibility If you are not determined eligible for examination, you will receive anemail notification.

Application fees are non-refundable. Be sure you meet theeligibility requirements as stated and are able to provide theappropriate documentation before submitting your application formand fee.

Study MaterialsStudy materials and recommended reading lists are available toprepare for examination. Visit the ASCP website atwww.ascp.org/qualification for details.

Scheduling Your ExaminationQCYM, QIHC, and QLS: Once your application and eligibilitydocumentation has been approved by BOC, you will receive anAdmission Notification email containing an authorization number tothe examination. The examination consists of a 50 question multiplechoice timed test that must be completed within a 90-minute timeperiod. The test is self-administered on your own computer at thedate and time of your choice within the 60 day time period indicatedon your Admission Notification.

QLI: Once your application and eligibility documentation has beenapproved by BOC, you will receive electronically the QLI Project

Booklet. Your QLI Project must be electronically returned to theBOC office within 90 days.

Cancellation PolicyYou will not receive any refund of your application fee nor can it betransferred to another examination period if you do not completeyour examination (QCYM, QIHC and QLS) within a 60 day timeperiod indicated on your Admission Notification. You will not receiveany refund of your application fee nor can it be transferred toanother examination period if you do not electronically return to theBOC office your work project (QLI) within 90 days. A full fee will berequired to reschedule the examination.

Reapplying for ExaminationQCYM, QIHC, QLS candidates if you cancel, do not schedule yourself-administered examination within the 60 day time period or aredetermined ineligible, you must submit a full application fee toreapply.

QLI applicants if you cancel, do not submit your QLI Project or aredetermined ineligible, you must submit a full application fee toreapply.

Your application in a particular category will remain valid for a periodof three years from the date of submission. RevalidationIn order to maintain your qualification, you must revalidate everythree years by completing 6 contact hours of acceptable continuingeducation in the area of qualification OR 3 contact hours ofacceptable continuing education in the area of qualification and 3contact hours of other activities related to the qualification.

Individuals who allow their qualification to expire will be permitted toreinstate their expired qualification by completing the RevalidationForm documenting all points earned as required by the qualificationcategory, submitting the current application fee, as well as areinstatement fee of $50. If the qualification is expired for more thanten years, the individual will be required to retake the onlineexamination for: QCYM, QIHC, and QLS. For QLI, the individual willbe required to submit a work project.

Instructions for Completing theQualification Revalidation FormStep 1: Complete the paper Qualification Revalidation Form by

documenting the 6 required contact hours of continuingeducation for your qualification category. List the CE provider, Course or activity, number of contact hours/CEsassigned and date of completion. Step 2: Submit your completed Revalidation Form and the $50application fee (by check/money order or credit card) onemonth prior to the date your current qualification is due toexpire.
