President Truman’s Health Programme

THE PRESIDENT’S message to Congress on Nov. 19is a landmark in America’s progress.in social reform.It begins with a reminder that up to last April nearly5 million men between the ages of 18 and 37 registeredfor service in the Armed Forces of the United Stateswere reckoned medically unfit for military service,the number so rejected forming some 30% of allthose examined. In addition, about I 2 million menhad after induction to be discharged from the Forcesfor physical or mental disability, exclusive of wounds,and an equal number have required treatment whilein the Forces for defects existing before induction.-Mr. TRUMAN refers also to a previous message toCongress proposing an Economic Bill of Rights towhich he thinks every United States citizen is entitled,including" the right to adequate medical care andthe opportunity to achieve and enjoy good health,"and " the right to adequate protection from economicfears of sickness." He points out that millions ofcitizens do not enjoy these rights, and thence heproceeds to a masterly analysis of present defects andthe means by which they can be removed.Of his five " basic problems," the first concerns the
number and distribution of doctors and hospitals.Though the USA has more medical practitioners inproportion to population than any other country,their distribution is " grossly uneven." In the lastcensus year, 1940, there were 31 American countiesin which there was no practising doctor. Thedistribution of hospitals was similarly unsatisfactory.There are 1200 counties, with an aggregate populationof 15 million, which have either no hospital at all ornone that reaches the minimum standards of nationalprofessional associations. Mr. TRUMAN emphasises’that mere building is not enough ; the hospitals mustbe staffed, and the staffs properly paid. The secondand third problems arise from the need for the
development of public-health services (especiallymaternal and child care), and of research and profes-sional education. About 40 million American citizenslive in communities without full-time local public-health services : and, to illustrate the need foradvances in research and medical education, the
PRESIDENT points out that the annual number ofrecorded deaths from cancer is over 160,000 ; thatthere are at least 2 million persons suffering frommental disease ; that about 10 million will probablyneed hospital treatment for mental disease at sometime of their life ; and that mental cases fill morethan half of all hospital beds at a cost of about500 million dollars a vear. He maintains thatAmerica needs more mental hospitals, more out-
patient clinics, more services for early diagnosis, and"
especially much more research and many moredoctors." Fourthly and fifthly Mr. TRUMAN remindsCongress that the chief reason why the people do notreceive sufficient medical care is that ’’ they cannotafford to pay for it on an individual basis at the time
they need it," and that this is true of a large pro-portion of normally self-supporting persons."
" Sickness," he says, "not only brings doctors’bills ; it also cuts off -income. On an average day,there are about 7 million persons so disabled bvsickness or injury that they cannot go about theirnormal tasks. Every year, 400-500 million workingdays are lost from productive employment becauseof illness or accident among those working or looking
for work-about -40 times the number of days lostbecause of strikes on an average during the-ten yearsbefore the war."
To solve these problems Congress is recommended toadopt a comprehensive national health programme.This should include Federal government grants toState or other authorities for provision of hospitalsand related facilities, for expanded public-healthservices, and for the advancement of research andprofessional education. It should also include theestablishment of a system of " required " prepaymentof the costs of medical care-in other words, a systemof health insurance on a compulsory basis. It shouldprovide, inter alia, for free choice of doctor, freedomto the doctor to decide what services his patientshould receive, and adequate medical remuneration,the rates and methods of which should be adjustedlocally. Finally, protection against loss of wagesfrom sickness and disability should"be aiforded by anextension of the present system of unemploymentinsurance.The PRESIDENT’S message will be variously received
by his fellow country-men. The proposals relatingto expansion - of existing services will probably bewidely accepted, though there may well be differencesof opinion on methods of allocation. His recom-mendation of health insurance, however, is likely toencounter much opposition. After the introductionof the British system in 1912, efforts were made byAmerican social workers to establish compulsoryhealth insurance in their country; but with no
success. The question was again brought before thepublic by the appearance, in 1932, of the report ofthe Committee on the Costs of Medical Care, whichwas followed by a series of volumes embodying theobservations of the committee’s field workers-aremarkable staff of trained investigators. These
reports furnished an impressive body of evidence
showing that the American people, even before thedepression, were, from economic causes, far fromgetting the medical attention they needed, and thatit was unlikely they would get it without govern-mental assistance. After the issue of these docu-ments, the movement towards health insurance
gained ground, but it was still opposed by the schoolof thought represented in the official utterances ofthe American Medical Association. Compulsoryhealth insurance, said its opponents, savoured of
paternalism and was therefore’incompatible with therugged individualism of the American character.British experience of health insurance was quotedby the opposition as a warning example of " socialisedmedicine," which had turned British clinicians intogovernment servants, lowered the standards of
professional efficiency, impaired the confidentialrelation of doctor and patient, and generally wroughthavoc in the medical profession. These strangemisconceptions, once prevalent among Americandoctors, are now giving way to more instructedviews. President TRUMAN, however, knowing the

kind of opposition likely to be evoked by his healthinsurance scheme-which has much in common withours in this country-goes out to meet the critics.
" The American people," he argues, " are the mostinsurance-minded people in the world. They willnot be frightened off from health insurance becausesome people have misnamed it socialised medicine.’What I am recommending is not socialised medicine.Socialised medicine means that all doctors work as
" employees of the government.... No such systemis here proposed."What effect the message will have upon American
legislation is a matter of conjecture. But as a State
paper it will take an honoured place in the long seriesof historic documents prepared by the Chief Executiveof the American people for the consideration of theAmerican Legislature.
Antidote to ArsenicalsWHEN so much is being made of the destructiveness
of science, it is refreshing to know of a case wherescience has reversed its capacity for damage, and inso doing has produced a valuable therapeutic agent.It was a well-kept secret that a new anti-arsenicalsubstance was on trial, but Prof. R. A. PETERS, FRS,Dr. L. A. STOCKEN, and Dr. R. H. S. THOMPSON,1 ofthe biochemistry, department at Oxford, have nowdescribed the results of their research in the field ofchemical warfare which led to the discovery of BAL(British-anti-lewisite). This antidote should makethe arsenical war gases obsolete-in itself no meanachievement-but it is also applicable to the treat-ment of some of the rare but none the less grave and
distressing complications of arsenical therapy, suchas dermatitis and encephalitis ; and the facts disclosedsuggest that it will also be a useful antidote to someother metallic poisons. The new substance is 2,3-dimercaptopropanol (CH2SH.CHSH.CH20H), whichforms with lewisite (and other trivalent arsenicals) astably combined ring compound :HzC-SH HC—SBI I "A8.CH : CHClH C-SH + C12 As.CH : CHC1 == H C-s/ -I- 2HCl.
I BH2C.OH HzC.OHWhen BAL is - applied to tissues containing a
trivalent arsenical compound, combined arsenic istransferred from the tissues to the BAL and is soexcreted ; thereby the toxic agent is removed. Thisdiscovery was made in the Oxford biochemistrydepartment towards the end of 1940, but had its originin earlier work done in that department over manyyears. Its final successful development has been afine example of the cooperation of British, Canadian,and United States investigators and clinicians, withfull support from their government organisations.Both the Oxford and the United States investigatorsearly appreciated the possible value of BAL againstarsenical dermatitis and encephalitis, and they startedcautious treatment by inunction early in 1943. The
larger resources in the USA led to quicker progress,and an ampoule " using as a vehicle 10% benzylbenzoate in arachis oil " was developed there for theintramuscular injection of BAL ; this supersededinunction methods, and has been used both in theUSA and in Britain. The favourable results reportedfrom America are in agreement with the work alreadydone here for the Medical Research Council; in a
preliminary report to the Council, half of the 30 casestreated were said to have responded favourably.If this success is maintained, BAL is certainly betterthan any other remedy so far described. (Authoritiessuch as EARLE MOORE maintain that injections of
thiosulphate, though widely used, are of no clinicalvalue.) Evidently, however, it is not free from
toxicity. Its LD50 for rats is about 110 mg. per kg.,and it produced erythema and transient lacrimationin a human volunteer who had 1 c.cm. rubbed into hisforearm. Until the stage of clinical trial is over itsuse should be well controlled. The initial dosagenow recommended, based on American experience, isup to 4 mg. per kg. body-weight at intervals of fourhours. With Mapharside ’ (arsphenoxide), as withlewisite, there is evidence that excretion of arsenic isincreased by the drug, though in cases where sometime has elapsed since the arsenical was administeredthe increased excretion is not always detectable.
Kidney damage is not a contra-indication to theuse of BAL, but it should be withheld in cases ofliver damage.Two ideas guided the group of workers in Oxford
who early in the war set out to find an antidote tolewisite, under the direction of Professor PETERS forthe Ministry of Supply ; one came from research onthe biochemical lesion in aneurine (vitamin B1)deficiency and the other from a much earlier plannedresearch on the action of chemical warfare agents.In the years before the war it had been established

that the opisthotonos of Bi-deiicient.pigeons was dueto a partial loss of aneurine pyrophosphate in thebrain, producing a block in carbohydrate metabolismby failure of the " pyruvate oxidase enzyme systemthis led to an accumulation of pyruvate in the blood.It had been suggested further that vesication mightbe due to a selective poisoning of this system, suchas is known to occur with iodoacetic acid and dichlor-diethylsulphone ; arsenite also inhibits it.1 This wasthe basis of the use of the pyruvate oxidase system as abiochemical test for arsenicals-a simple enzyme testwhich has proved invaluable throughout the research.The other guiding idea, that arsenicals primarilyattack an -SH group in the enzyme, originated withwork on chemical warfare agents done in the sameOxford laboratory by E. WALKER under PETERS’Sdirection and begun in 1923. It was found that thesearsenicals react with thiol groups in the tissues; theidea owed its inspiration to HoPxns’s discovery ofglutathione. Simultaneous work by VoEGTMN andhis colleagues in the USA 5 on the action of therapeuticarsenicals led them to attribute the toxic action ofarsenic on living cells to its reaction with essentialthiol compounds in protoplasm. Other importantwork over the intervening years reinforced the ideathat arsenicals attack -SH groups ; and it was highlysignificant that COHEN, KING, and STRANGEWAYS 6showed in 1931 that compounds of arsenicals withthiols of type R.As(SRl)2 (thioarsenites) were dissoci-able in alkaline solution ; this suggested that thepoisoning might be reversible in vivo.
