About CERG CERG Grants support CMSP counties in responding to the COVID-19

pandemic emergency through the provision of funding to expand the delivery of services that support local preparedness, containment, recovery and response activities in CMSP counties affected by the novel coronavirus.

Example of a Complete Target Population Response (Sections 2.B.)Shasta County citizens who are homeless are at a higher risk of the Covid-19 virus due to a higher likelihood of pre-existing health conditions. This population fits three of the above populations as they may be uninsured or underinsured, lack support services, are identified as higher risk, and often have transportation and struggles accessing health and housing resources.

Shasta Community Health Center’s HOPE van has seen 127 homeless patients with COPD-lung issues in the past 90 days, and 86 homeless patients over the age of 65. These numbers are indicative of the high need for hotel services for at-risk homeless individuals.

Shasta County HHSA’s HMIS reports show that over the past three years, 56% of our clients suffer from chronic health conditions. Our 2019 PIT counted 434 unsheltered homeless individuals. If we use the same formula, 243 of our unsheltered homeless have chronic health conditions and would be at-risk for Covid-19.

Example of a Complete Proposed Partner Organizations Response (Section 3)

Shasta County Housing & Community Action Programs (SCHCAP) is currently taking the lead on the Project Roomkey emergency funding for hotel vouchers for our homeless at-risk population. They will utilize CERG additional funds to increase hotel capacity for those in need. All those served by these funding sources will be input into the Housing Management Information System (HMIS) for data collection purposes.

Funds will be allocated to the Shasta County Health and Human Services Agency (HHSA) to work in collaboration with Shasta Community Health Center (SCHC) and SCHCAP to provide wrap-around services for our most vulnerable homeless citizens. Partial funds will be distributed to the SCHC to provide a full-time case manager, referral coordinator, part-time clerical assistance, and supervisory oversight. They will also provide hygiene items as needed. HHSA will receive funds to provide a driver, and a part-time data analyst to assist with reporting and data collection. Administrative expenses will be split between SCHC, SCHCAP, and HHSA.

Other funding and identified gaps example (Section 4.B) Aside from $309,000 Project Roomkey emergency funding, Shasta County homeless service providers have not received additional funding to address the COVID-19 pandemic for the vulnerable homeless in our community. Although those funds will assist with the costs of motels and food for those citizens, we expect a shortfall of costs. In addition, staffing to serve the homeless in hotels as well as provide accurate documentation is lacking. Some gaps we are experiencing are because many staff are either working from home due to health conditions or not working at all because of child care issues. CERG funding will fill a staffing gap for our community, and provide additional hotel funding to assure adequate coverage.

Section 4.C. - Explaining Costs It is important to justify any planned expenses in Section 4.C. of the

Request Form. All cost estimates should relate to the number of individuals you expect

to serve. Example: An applicant plans on serving 300 individuals through

supportive quarantine services, but only designates $2,000 for food, hygiene supplies, and rent costs. This number is not enough to serve that many individuals, and needs further explanation.
