PRE-SURGERY CHECK LIST There's a lot to remember when traveling for surgery and we recognise that when you're traveling to another country, you might need a little extra help. By now your appointment should be confirmed and your flights and hotel accommodation booked. Below is a check list to make sure that you have covered all the details of your upcoming surgery. You may have already received some of this information in other documentation, but we ask that you take this time to read this list to make sure you are ready and fit for your surgery. Make sure to inform your primary care physician before stopping any medication. A complete list of medications is available on request from a PPSI coordinator. Take off any body piercings. Stop taking supplements, such as Multivitamins, Vit. E., Fish oil tablets, Omega 3, Ginseng- Ginko, Evening Primrose oil, and herbs. Stop taking non-steroidal anti-inflammatory medications (such as Ibuprofen, Advil, Motrin, Nuprin, Aleve, Relafen, Naprosyn, Diclofenac, Naproxen, Voltaren, Daypro, Feldene, Clinoril, Lodine, Indocin, Orudis and others). For a complete list of medications and supplements, please contact PPSI. Wash hair and take shower, using no cosmetics or lotions. Remove acrylic nails and nail polish. No food or drink Drug affects wound healing, such as bad scarring, or cause severe capsular contracture. Smoking may cause blood supply compromise and delay wound healing. Anticoagulants will lead to increased bleeding or hemorrhage after surgery. Remove at least 2 weeks before surgery in order to detect and treat any underlying infection. PRE-SURGERY CHECK LIST

There's a lot to remember when traveling for surgery and we recognise that when you're traveling to another country, you might need a little extra help. By now your appointment should be confirmed and your flights and hotel accommodation booked. Below is a check list to make sure that you have covered all the details of your upcoming surgery. You may have already received some of this information in other documentation, but we ask that you take this time to read this list to make sure you are ready and fit for your surgery.

• You have a valid passport and have checked your visa requirementsIt is your responsibility to ensure that your travel documents are valid and to obtain a visa if required. Most clients enter the Kingdom of Thailand as tourists. For a list of nationals of countries which are exempted for visa requirements for stays up to 30 days, please see www.mfa.go.th

• You have checked the details in your Treatment PlanComplimentary transportation is available for clients having procedures that costs over 100,000 Thai Baht. If you have a confirmedtransport booking, please double check your treatment plan to make sure that all transport details are correct and inform us if thereare any changes in your itinerary.

• You understand payment policySomnio generally requires full pre-payment of your procedure 1 month prior to departure. After your face-to-face consultation andbefore your admission and surgery, the cost may change, if the costs are slightly more, You can pay this small amount direct withmost major credit cards or some debit cards, however please make sure to check with your bank if you have a maximum daily limit and to inform them that you plan to make a n international transaction in Thailand. This is to prevent your bank’s security system from blocking the transaction. Both Somnio and PPSI do not accept travellers or personal cheques. Please contact us should you require further information or if you have any questions regarding payment options.If a refund is necessary this will come direct from PPSI Accounting department. Please allow a minimum of 4 weeks for this to be reimbursed.

• You have advised Somnio of any underlying medical conditionsPlease notify us should you have pre-existing conditions such as diabetes, HIV, hypertension, asthma, etc. Certain underlying medical conditions or medications may affect your risks for surgery, and your surgeon may require your physician to confirm in writing that youare fit to undergo a general anesthesia. Please bring along your most recent test results and a medical report from your attending physician. If you are unsure please contact your Somnio coordinator. Note that you would be liable for any additional costs of any testing, prolonged hospital stay, etc which may occur due to your pre-existing condition.

• You have informed Somnio if you think you may have menstruation on your surgery dateIf you have menstruation on the day of your surgery, the surgeon may delay or cancel your procedure, especially if you are having multiple or lengthy procedures. Please notify us if you think you will be having menstruation on the day of surgery.

Arrival in Phuket check list

If you are arriving by plane at Phuket International Airport and you have a confirmed transport booking, please follow the guidelines in your treatment plan to meet up with the PPSI concierge staff and driver.

Ifyou are unable to locate their concierge staff or driver, your flight has been significantly delayed or you are concerned about your transport arrangements, please call either your Somnio Coordinator or PPSI concierge staff on +66 (0) 98-701-5952 in the first instance or public relations department on 076 249400 extension 1338 in the second instance. Public phones are available within the airport.

Pre-Appointment check list

• Fasting before surgeryYou are required to have nothing to eat or drink after midnight the night before your surgery, unless otherwise specified by yourSomnio coordinator. Please contact us if you are unsure of your fasting time.

• Confirm pickup time of driver for your PPSI appointmentOn your final treatment plan you will see the date and appointment time with PPSI. Should you qualify for our complimentary transport you will find the details of your pickup time from your hotel. Please be ready in the lobby of the hotel at the correct time.

• What to bring to the hospital 1. Passport or a valid ID for registration2. Any medication that you are currently taking3. Personal toiletries (a basic toiletry kit is provided by the hospital)4. Reading material5. Laptop, tablet or mobile phone. Complimentary Wi-Fi is available to all PPSI clients6. An adapter (to convert your electrical plug to fit into the Thai electrical sockets).7. Loose fitting clothing (to wear for your discharge from hospital). During your stay in the hospital you will be required to wear

the hospital garments8. Compression garment (these are also available in Phuket with Somnio Staff - should you forget to purchase prior)

Unless essential, leave your jewellry and valuables at your hotel or home as you will not be allowed to wear it during your surgical procedure. Safe deposit boxes are available in your private hospital room, which you access with your own password. Note that Somnio and PPSI will not be responsible for any loss or damage.

• Make sure you have your preferred method of payment on hand with youPlease note that no procedure or price is confirmed until after the face-to-face consultation. Payment is required in full before your surgery and additional payment options include: 1. AMEX, MasterCard and VISA credit/debit cards are welcomed.2. Cash in Thai Baht currency or your home currency (note that fees apply to payment in foreign currency).3. Pre-arrival bank transfers to PPSI or any of our a�liated agencies.

Underlying asthma

Consult Cardiologist for surgery clearance.

Consult with Internal Medicine Specialist.

Digital mammogram is strongly recommended. Please bring the results if you had this done within 6 months in your home country. Alternatively, you can have a mammogram at PPSI (charges apply). Clients choosing not to have a mammogram are asked to sign a Speci�c Risk Consent Form.

Digital Mammogram is required. The operative procedure will leave scars inthe breast tissue, which can mimic abnormal �ndings in future mammograms.Please bring results if you has this done within 6 months in your home country,or you may have it done at PPSI before surgery (extra charges apply).

Exercise Stress Test will be required.

ENT consultation with nasal Telescope will be required.

Blood test for HbA1c; Fasting blood sugar. Consultation with internal Medicine specialist.

Requires a Medical Report/Certi�cate from attending physician in home country with Hypertension history and current medications taken.

Thyroid function tests and Internal Medicine specialist consultation.

Treatments for 7 days and then re-assess for clearance to undergo the planned procedure. A medical certi�cate/medical report is recommended if treatments isreceived in your country. Please bring medications taken.

Generally we do not accept clients with these underlying diseases. In some cases the surgeon will determine the client’s eligibility for surgery on a case by casebasis. Note that there may be a surcharge for your surgery and you may be asked to sign a Speci�c Risk Form.

Well controlled asthma with no asthmatic attack within the last 12 months: Nofurther tests are required. Others: Consultation with Pulmonologist with Pulmonary Function Testing may be required. Please bring along a Medical Report/Certi�catefrom your physician and your current asthma medication/inhaler.

Cost of extra pre-surgery tests:

1. Digital mammogram: 3,000 baht2. Exercise stress test: 4,000-4,800 baht3. Thyroid function test (TSH ,FT3, FT4): 2,500 baht,4. Diabetes test (HbA1C and Fasting blood sugar): 1,000 baht5. Nasal Telescope: 2,500 baht6. Consultation Fee with specialist doctor (ENT, internal medicine, cardiologist, etc): 1-2,000 baht7. Support garment (body contouring surgery): 8,000-10,000 baht8. Facial band (face lift): 2,000-3,000 baht

Notes: The medical history is used for primary screening/evaluation before con�rming your appointment and surgery. PPSI surgeons and coordinators strive to con�rm your �tness for surgery before your departure from your home country, however in rare circumstances, a client may be considered to be un�t or not a good candidate for surgery upon consultation/physical examination at PPSI. The hospital/doctor has the right/authority to refuse such cases for patient safety reasons.

In case the plastic surgeons advises the client to consult with another doctor prior to surgery, and the client refuses to do so, the attending plastic surgeon has the right to cancel the surgery. The hospital/doctor will not be liable for the client’s �ight/travel and accommodation expenses.

You should not cease your medication without the advise of your physician. Please contact your Somnio coordinator for clarification if you have any concerns regarding your medication or supplements. For minor headaches, aches, pains and fever, you may take Paracetamol, Panadol or Tylenol.
