    Before you begin:

    • Click ‘Save Information’ periodically and before leaving the online portal; it will time out after 30 minutes of inactivity.

    • When uploading documents to the online portal, the combined file size may not exceed 15 MB (megabytes) total.

    Step 3: The online portal will then prompt you to Validate OTP (One-Time Passcode).• Check your email and enter your one-time passcode in the entry box.

    • If you did not receive your one-time passcode, please check your spam folder.

    • After entering your passcode click Verify.

    Step 4: Review the following to determine whether you are eligible for the SBA Form 3508 or 3508EZ.

    You can apply for forgiveness of your PPP loan using the SBA Form 3508EZ if you can check at least one of the three boxes below. If you cannot check one of these boxes, use the SBA Form 3508.

    n You are a self-employed individual, independent contractor, or sole proprietor who had no employees at the time of the PPP loan application and did not include any employee salaries in the computation of average monthly payroll in the Borrower Application Form (SBA Form 2483).

    OR

    n You did not reduce annual salary or hourly wages of any employee by more than 25 percent during the Covered Period or the Alternative Payroll Covered Period compared to the period between January 1, 2020 and March 31, 2020.

    - “Employees” means only those employees that did not receive, during any single period during 2019, wages or salary at an annualized rate of pay in an amount more than $100,000.

    AND

    You did not reduce the number of employees or the average paid hours of employees between January 1, 2020 and the end of the Covered Period.

    - Ignore reductions that arose from an inability to rehire individuals who were employees on February 15, 2020 if the Borrower was unable to hire similarly qualified employees for unfilled positions on or before December 31, 2020.

    - Also ignore reductions in an employee’s hours that the Borrower offered to restore and the employee refused.

    OR

    n You did not reduce annual salary or hourly wages of any employee by more than 25 percent during the Covered Period or the Alternative Payroll Covered Period compared to the period between January 1, 2020 and March 31, 2020.

    - “Employees” means only those employees that did not receive, during any single period during 2019, wages or salary at an annualized rate of pay in an amount more than $100,000.

    AND

    You were unable to operate during the Covered Period at the same level of business activity as before February 15, 2020, due to compliance with requirements established or guidance issued between March 1, 2020 and December 31, 2020 by the Secretary of Health and Human Services, the Director of the Centers for Disease Control and Prevention, or the Occupational Safety and Health Administration, related to the maintenance of standards of sanitation, social distancing, or any other work or customer safety requirement related to COVID-19.

    Step 5: You will then enter the Loan Application Stage and Select Form.

    Step 6: Begin by entering Loan Forgiveness Information under Loan Identification.

    If you are submitting the SBA Form 3508EZ, proceed with Step 6. If you are submitting the SBA Form 3508, skip to Step 12.

    Step 8: Enter Payroll Schedule, Covered Period, and Alternative Payroll Covered Period, if applicable.• At the bottom of the page, it will ask if you received PPP loans in excess of $2 million.

    • Select Yes or No.

    • According to SBA guidance, the loan will be fully forgiven if the funds are used for payroll costs, and eligible non-payroll costs including:

    o Interest on mortgages

    o Business rent or lease payments

    o Utilities

    Reminder: At least 60% of the forgiven amount must have been used for payroll.

    Step 10: Potential Forgiveness Amounts will be auto-populated based upon your above payroll and non- payroll cost entries.

    Step 11: Estimated Loan Forgiveness.• An Estimated Loan Forgiveness amount will populate in line 8.

    • As it notes in gray print, if you have applied for an EIDL Loan, the SBA will deduct advance amounts from the total eligible forgiveness amount.

    If you are submitting the SBA Form 3508EZ, skip to Step 15.

    Step 12: Begin the SBA Form 3508 by entering Loan Forgiveness Information.

    screen under User Uploaded Documents.
