Frequently Asked Questions

What is Post-Acute Care? Following a hospitalization or medical event for injury or illness, many patients require continued medical care, either at home or in a specialized facility. Post-Acute Care is a range of medical services that support the patient’s continued recovery from a recent injury or illness to ensure all issues are addressed and treated.

If you no longer need inpatient hospital care, we may discharge you to a Post-Acute Care setting to assure you receive appropriate transitional care for continued recovery.

What are the benefits of Post-Acute Care? Patients who are discharged to a Post-Acute Care Facility are less likely to be readmitted to the hospital. They are closely monitored for complications or condition changes. This improves the likelihood of catching new or recurring problems and treating them appropriately.
