Patient transition through the system to be fully integrated / seamless
Shift from episode of care approach to a holistic lifetime care perspective
Managed transition of patients through the primary, community and continuing care services and between these and the hospital / programme sectors
Central dependence on pre- hospital care - enhanced roles ANPs, Therapists, etc
Shared Workforce appointments – clinical staff following the patient from one location / jurisdiction
Hospital based services in accordance with casemix severity with clinical staff working in multiple settings
Shift from institution based services to community based settings
Patient/client access to services via a health professional responsiblefor ensuring appropriate access to healthcare service in both acuteand non-acute settings
Implications for ICT Strategy
Key themes for ICT from the Transformation Programme are:
Shift the paradigm of care from acute Hospitals to PCCC coupled with greater emphasis on prevention / promotion. In essence, to achieve significant reduction in dependence on Hospitals
Integrate services around the patient. The patient becomes the central element of the service, not institutions or professions
Implement standards based performance measurement and management throughout the health service
Implications for ICT
The key information focus must be the patient
Information must follow the patient through the system
It must be possible for health professionals to:
access and add all pertinent information on the patient at point of consultation, diagnosis, treatment and / or care
communicate and receive information, in respect of patients from all appropriate sources
It must be possible for:
patients to access information on their personal health and schedule appointments
pre-hospital care professionals to contact and transmit patient physiological information from remote locations wirelessly
access and merge patient, financial and HR information for planning, control, decision making, performance, quality/safety/RM etc. purposes
consolidate and analyse data for population health planning and for corporate & service planning
transmit information across the entire system
assemble/merge data from multiple locations and systems
assemble data on a specific patient from multiple locations/systems, each of which identifies the patient by multiple reference criteria

NIMIS Objectives
The installation of PACS/RIS solution to deliver a filmless solution for Radiology & Cardiology imaging in each hospital that currently does not have such
To deliver a paperless solution for Radiology including ordering and result reporting
To integrate all new and existing sites to a Central Data Repository to allow the controlled and rapid movement of patient image data throughout the health service
To deliver on a large range of identified benefits including the ability for remote reporting, removal of film-based risk factors, cost savings related to image management, and more rapid reporting of diagnostic imaging procedures
To promote and act as a facilitator to the HSE Transformation Programme.
