THE INSTITUTE Since its inception on December of 2006, the Instituto Paulista de Desenvolvimento Humano - IPDH (São Paulo Institute of Human Development, in Portuguese) aims to provide better quality of life to its surrounding communities, especially the Lins and São Vicente regions, both in the Sao Paulo state, southeastern Brazil. The IPDH focuses on the management of health institutions and social projects related to culture, sports and social welfare. All IPDH initiatives have as a basic principle the valuing of human life. Its resources are allocated to ensure the health and psychological care of the citizens, improving their quality of life. Humanization, ethics and transparency are cornerstones of all of the Institutes’s projects and its contributors are constantly reminded to put into practice its slogan: “Caring for Life with Excellence”.

3 LOCATION Our head offices are located at José Ariano Rodrigues Avenue, 303, in Lins, a city in the heart of Sao Paulo state. Other 10 cities make up the Lins sub-region, an strategic location for the development of our activities.

The IPDH menagement contract for the Lins’ Basic Health Services came after the Institute won a municipal procurement process that took place in 2013 and had bids by several competing organizations.

Basic Health Service Administration of the city of Lins This administration contract for the Basic Health Service for the city of Lins has been conquered through public tender of a Project presented on the year 2013, running against numerous other entities alike, having prevailing over them. The contract consists of the adminstating of the Health Unities listed below.

Psychosocial Care Center Drugs and Alcohol – CAPS AD “Green Space” CAPS AD is a health unity specialized on attending alcohol and drugs addicts within the guidelines determined by the Ministry of Health, which is based on treating the patient on a free environment, seeking their social reintegration. The multidisciplinary team is structured by 13 (thirteen) professionals composed of Social Assistant, psychologist, Nurse, Occupational Therapist, Clinical physician, Psychiatrist, Nursing technician, among others; being 09 (nine) directly linked to the IPDH, 02 (two) Service Providers and 02 (two) statutory professionals ceded by the City Hall. Psychosocial Care Center I – CAPS I “Dr. Alberto Prata junior” CAPS are institutions destined to take in patients suffering from mental disorders; stimulate their social and family integration, supporting them in every effort towards their own autonomy, offering them medical and psychosocial care. These patients are attended according to an individual Therapeutic Project, which consists of specific treatment to each patient prepared by the team. Aside from the clinical appointments, the Project can be made of several diversified other activities such as cultural and therapeutic workshops, group therapy, individual or group counseling, among others. The multidisciplinary team is structured by 09 (nine) professionals composed of Social Assistant, Psychologist, Nurse, Occupational Therapist, psychiatrist, Nursing Technicians, among others, being 08 (eight) directly linked to the IPDH and 01 (one) statutory professional ceded by the City Hall. Pediatric Psychosocial Care Center – Pediatric CAPS “sunflower” Pediatric CAPS is a daily care service destined to children compromised by some level of mental disorder. Pediatric CAPS is equipped with excellent physical

structure, where each detail was carefully designed to embrace each child suffering from any mental disorder from autism, to psychosis, severe neurosis and every one of those who, because of their mental condition, are impaired of keeping nor build social bonds. The multidisciplinary team is structured by 10 (ten) professionals composed of Social Assistant, Psychologist, Nurse, Occupational Therapist, Psychiatrist, Nursing Technician, among others, being 05 (five) directly linked to the IPDH, 01 (one) Service Provider and 04 (four) statutory Professionals ceded by City Hall City Pharmacy of Lins The Pharmacy has the purpose of decentralizing medications from Basic Health Care to Basic Health Unities and Family Health programs, and also having the intention to promote easy access for the people of Lins to City Pharmacy, supporting health actions towards accessibility of especial medicine and/or specialized components, supporting each Mental Health Unity through these actions and promoting rational use of such medications. The team is structured by 05 (five) professionals composed of Pharmacists, pharmacy technicians and administrative support, being 03 (three) directly linked to IPDH and 02 (two) statutory professionals ceded by City Hall. Home Care Medical Program (PROMID) Home Care is a type of health care which can be for replacement or additional to the already given medical care. Characterized by a joint of actions for improving, prevent and treat diseases. Providing rehabilitation given at patients´ home. For those who are in bed-rest, or dependent of a care taker to rely on for daily activities, also for those who suffer from severe disabilities as result from chronic diseases. Attempting to reduce the demand for hospital care and

also reduce the time of hospitalization, increasing humanization of health care and providing some autonomy for the patients. The multidisciplinary team is structured by 13(thirteen) professionals directly linked to the IPDH, composed of Social Assistant, Nurse, Physical Therapist, Occupational Therapist, Physicians, Nursing Technicians, among others Dentistry Specialized Center (CEO) The Dentistry Specialized Center, (CEO) are health establishments participating of National Health Establishments – CNES, classified as Specialized Clinic or Specialties Ambulatory. The Dentistry Specialized Centers are prepared to offer the following services to our population: a) Oral diagnostic, with an emphasis on diagnosing and detecting oral cancer. b) Specialized Periodontology c) Minor oral surgery of hard and soft tissue d) Endodontics e) Attendance to people with special needs. The multidisciplinary team is structured by 10 (ten) professionals composed of, oral health assistant, dental surgeon, endontist, stomatologist, periodontist and administrative support, being 06 (six) directly linked to IPDH and 04 (four) statutory professionals ceded by City Hall. Counseling and testing Center (CTA) Counseling and testing center (CTA) are health services that provide diagnostics and prevention to sexually transmitted diseases. Through these services it´s possible to test HIV, syphilis and hepatitis B and C all free of cost. Every test is performed according to the technical standards established by our Ministry of Health and using registered and controlled products from Health Surveillance (ANVISA).

The multidisciplinary team is structured by 08 (eight) professionals composed of social assistant, psychologist, nurse, clinical physician, infectologist, nursing technician, among others, being 06 (six) directly linked to IPDH and 02 (two) statutory professionals ceded by City Hall. Family Health Program (PSF) PSF prioritizes actions of prevention, promotion and recovering of people´s health completely and continuously. Attendance is provided on the Basic Health Unit or at patients´ home by our professionals (doctors, nurses, nursing help and community health agents) who compose the Family Health Program team. Thereby, these professionals and city population monitored by them establish shared responsibility bond, which helps identifying and treating health issues of each community. Currently the city of Lins holds 4 PSF units and 9 new others are being implemented, reaching a number of 13 units and thereby increasing city coverage from 16% to 50%. Current Unities are: PSF Pasetto “Dr Thiers Garcez Aguiar” Multidisciplinary team is structured by 15(fifteen) professionals composed of community health agents, general physician, dental surgeon, dental health assistant, nurses, nursing technician, administrative support, among others, being 11 (eleven) directly linked to IPDH and 04 (four) statutory professionals ceded by City Hall.

Multidisciplinary team is structured by 11 (eleven) professionals composed of community health agents, genral physicians, nurses, pharmacists, nursing

technicians, administrative support, among others, being 10 (ten) directly linked to IPDH and 01(one) statutory professional ceded by City Hall. PSF São João “ Dr. Heyde Coelho Santos”

The multidisciplinary team is structured by 12 (twelve) professionals directly linked to IPDH and composed by community health agents, general physician, dental surgeon, dental health assistant, nurses, pharmacist, nursing technician, administrative support. PSF Pasetto “Pastor Agenor Miranda de Campos” The Multidisciplinary team is structured by 16 (sixteen) professionals composed of community health agents, general physician, dental surgeon, nurses, pharmacist, nursing technician, administrative support, among others, being 14 (fourteen) directly linked to IPDH and 02 (two) statutory professionals ceded by City Hall. Basic Health Units (UBS) Basic Health Unities are sites where you can get basic health services at no charge in the areas of Pediatrics, gynecology, general clinic, nursing and dentistry. The main services offered by UBS are medical consults, inhalations, injections, first aid bandages, and vaccines, sample collection for lab tests, dental treatments, specialty referrals and basic medication provision. Currently, the city of Lins holds 05 (five) Basic Health Units that support PSF, these are: UBS CAIC The multidisciplinary team is structured by 32 (thirty two) professionals

composed of community health agents, pediatricians, gynecolostis and general physicians, also dental surgeons, dental health assistants, nurses, pharmacists, nursing technicians, administrative support, among others, being 14 (fourteen) directly linked to IPDH and 18 (eighteen) statutory professionals ceded by City Hall.

UBS “Dr. Mohanna Adas” – Centro The Multidisciplinary team is structured by 23 (twenty three) professionals composed of community health agents, pediatricians, gynecologists and general physicians, dental surgeons, dental health assistants, nurses, pharmacists, nursing technicians, administrative support, among others, being 06 (six) directly linked to IPDH and 17 (seventeen) statutory professionals ceded by City Hall. UBS Junqueira – “Dr. Alberto Ariano Crespo” The multidisciplinary team is structured by 39 (thirty-nine) professionals composed of community health agents, pediatricians, gynecologists and general physicians, dental surgeons, dental health assistants, nurses, pharmacists, nursing technicians, administrative support, among others, being 17 (seventeen) directly linked to IPDH and 22 (twenty-two) statutory professionals ceded by City Hall.

UBS Rebouças – “Dr. Nilton Nicolau Naufal” The multidisciplinary team is structured by 28 (twenty-eight) professionals composed of commuty health agents, pediatricians, gynecologists and general physicians, dental surgeons, dental health assistants, nurses, pharmacists, nursing technicians, administrative support, among others, being 14 (fourteen) directly linked to IPDH and 14 (fourteen) statutory professionals ceded by City Hall.

UBS Ribeiro The multidisciplinary team is structured by 49 (fourty-nine) professionals composed of community health agents, pediatricians, gynecologists and general physicians, dental surgeons, dental health assistants, nurses, pharmacists, nursing technicians, administrative support, among others, being 23 (twenty-three) directly linked to IPDH and 26 (twenty-six) statutory professionals ceded by City Hall. Endemic and Vector-Transmitted diseases control This service functions along with many other organizations participating of Unique System of Health, organizing vector transmitted diseases and intermediate hosts programs and they are: Malaria, Chagas disease, Schistosomiasis, Visceral Leishmaniasis, Tegumentary Leishmaniasis, Dengue, Yellow Fever and Spotted Fever and other vector transmitted diseases of epidemiological relevance. The service also develops activities in order to inform, educate and communicate the population, aiming to inspire them on participating of such activities of vectors and intermediate host control The multidisciplinary team is structured by 35 (thirty-five) professionals composed of epidemic control agents, veterinary and nursing technician, being 17 (seventeen) directly linked to IPDH and 18 (eighteen) statutory professionals ceded by City Hall.

The IPDH has 194 direct employees working exclusively in the Lins’ Basic Healthcare Services system. In addition, 208 of Lins’ municipal civil servants and 55 interns are under IPDH’s management. The monthly average attendance in the IPDH administered healthcare facilities in Lins is:

- 9.116 weekly appointments at Community Health Centers (UBS);- 1.284 weekly appointments within the Program for Family Medicine;- 41.500 weekly consultations in TOTAL- 4.040 Registered families in the Program for Family MedicineMental Health Patients currently enrolled:- 143 at the Center for Psychosocial Care of Alcohol and Drug Abuse - CAPS AD- 145 at the Center for Psychosocial Care - CAPS I- 124 at the Center for the Psychosocial Care of Infants - CAPS Infancy- 960 at the Mental Health Clinic- Totaling 1.372 patients

HEALTHCARE 2. Bilac Beneficent Association The Bilac Beneficent Association has been assisting the towns of Bilac, Piacatú, Gabriel Monteiro and Santópolis for the last 46 years. Presently it has 36 hospital beds and open demand for emergencies. The Bilac Beneficent Association has performed 192 obstetrics surgeries only on year 2014 and it´s been introducing new specialties such as Ophthalmology, Orthopedics and Endovascular surgery. In addition to its qualifications in Federal Public Benefit on State and City levels, it holds the Certification on Beneficent Entity of Social Assistance in the fields of Health (CEBAS-SAUDE). Through its professional administration team, its procedures and physical structure are being upgraded in order to reach the Qualification of Social Health Organization for the State of São Paulo, where it is already recognized and registered on State Entities Registration. For its commitment to human resources development for a better assistance, Bilac Benefit Association celebrated a cooperation agreement with the University of Marília on the internship field for Unique System of Health (SUS). Its mission is to reach an excellence level on SUS attendance, aiming to bring into reality what is described in written words on our Federal Constitution.

Instituion´s Characteristics

Bilac Beneficent Assosiation is a hospital of small/medium size, it has participation on SUS (Unique System of Health), having 36 hospital beds being in this way distributed: 10 for General Surgery, 17 for General Clinic, 3 for Neonatology, 4 for Clinical Pediatrics, 2 for rehabilitation. 92 professionals compose the personnel forces, among them are: Physicians, Intensive care Physicians, nurses and Nursing Technicians. In the year 2014 the Hospital attended 25.872 (twenty-five thousand, eight hundred and seventy-two) patients

3. Internship Program

Since December 2008, the IPDH has a Technical Cooperation Agreement with the University of Marília - UNIMAR that implemented an internship program at the Promissão General Hospital. With this internship program, every year, around 60 medical students on their way to graduation strengthen the hospital staff to provide, trough the Brazilian Public Health System (SUS), healthcare to the local population.

4. Program Queremos Ouvir Você

Queremos Ouvir Você (We Want to Hear You, In Portuguese) is a program for the assessment of patient satisfaction with the medical care provided in the clinics overseen by the IPDH. The qualitative research is carried out by nurses of the Unisalesiano Faculty of Nursing, on the basis of a Technical Cooperation Agreement signed between that institution and the IPDH. Such activities now serve as a mandatory internship for all of Unisalesiano’s training nurses.

SOCIAL PROJECTS (ONGOING)

1. Social Empowerment Trough Sports

Developed in the city of Penápolis, 20 miles west of Lins, the Social Sport Project promotes the social inclusion of disadvantaged children through the practice of sports. Around 100 children take part in its activities. There, they learn the basics of soccer and other sports, but also have classes on citizenship and hygiene.

2. Agricultural Surplus Redistribution

Since 2011, the IPDH, in partnership with small producers and farmers, collects their agricultural surplus for redistribution to several charities in the municipality of Promissão. Among the beneficiaries of this food redistribution program are the Lar da Esperança Orphanage, the Action Against Cancer Network, and the municipal nursing home.

3. Drug Use Prevention

In partnership with the Sao Paulo State Department for Prevention and Suppression of Narcotics (DENARC), the IPDH periodically promotes lectures at schools, associations and organizations to children and adolescents on the social and health effects of drug abuse. The program also aims to train teachers and

monitors to evaluate possible involvement with drugs of children and adolescents under their tutelage,

and act to counsel them according to their training under the program.

SOCIAL PROJECTS (IN DEVELOPMENT)

1. Ponto e Vírgula Project

Expanding the development of its social actions, the IPDH is implementing in the city of São Vicente, São Paulo State, the Ponto e Vírgula Project (Semicolon Project, in Portuguese), which provides alternative treatments for children and adolescents with Attention Deficit Hyperactivity Disorder ADHD.Although still in its implementation phase, the project already has 109 children with diagnosed ADHD registered for participation in the Ponto e Virgula Soccer Club (PVFC). Designed as an extra activity, soccer helps in the education and social inclusion processes of the children, promoting citizenship. The continuous practice of physical exercises like soccer demonstrably improve cognitive performance of children with ADHD. These activities are not restricted, of course, only to the developing of the child’s cognitive abilities, but also boosts their morale, and their families’ morale as well. The PVFC encourages children and adolescents with ADHD to the practice of collective activities stimulating both their physical and psychological development, making them feel welcomed to a group with increasing self-esteem.

2. Specialties Center for Rehabilitation (CER)

The CER promotes specialized outpatient care in rehabilitation. It performs diagnosis, assessment, counseling, early intervention and specialized care in rehabilitation, lease, maintenance and adaptation of technology in care, providing a reference to the local health care network. The CER will comprise two main divisions, the Physical Habilitation and Rehabilitation Service, and the Intelectual Habilitation and Rehabilitation Service for Autism Spectrum Disorders, to be implemented in the city of Lins, Sao Paulo state, Brazil. Both projects were approved by Brazil’s Ministry of Health. The construction of its facilities is currently under procurement process by the Lins municipality. Upon completion, the IPDH will manage all of its activities.
