DURABLE POWER of ATTORNEY

1. DESIGNATION OF AGENT. I, YOUR NAME HERE. residing at

YOUR ADDRESS HERE, within the City of San Diego, County of San Diego, State of California, do

hereby appoint the following as my Attorney-in-Fact, whom I trust with any and all my financial and

probate decision making power immediately upon the authorization of this form: YOUR POA NAME

AND ADDRESS, as my Attorney-In-Fact (herein sometimes referred to as Agent) to act for me and in

my name as authorized in this document.

2. CREATION OF DURABLE POWER OF ATTORNEY. By this document I intend to

create a general power of attorney under the laws of the State of California. Subject to the limitations in

this document, this power of attorney is a durable power of attorney and shall not be affected by my

subsequent incapacity or absence.

3. STATEMENT OF AUTHORITY GRANTED.

I hereby grant to my agent full power and authority to represent me in probate proceedings and/ or

intestate succession proceedings pertaining to the assets of Mr. JOHN DOE, in judicial and nonjudicial

proceedings, present documents on my behalf, accept or repudiate any inheritance rights that I may be

entitled to, sign allocation of corresponding assets; furthermore, agent shall have full power and authority

once the corresponding probate proceeding is completed; the assets granted shall become property of

grantor, and may be sold in favor of third parties and receive corresponding payment on my behalf.

I hereby grant to my agent full power and authority to act for me and in my name in any way which I

myself could act, if I were personally present and able to act, with respect to the following matters as each

of them is defined and construed by the laws of the state of California:

(1) Real estate and inheritance transactions.

(2) Tangible personal property transactions.

(3) Financial institution transactions.

(4) Claims and litigation.

(5) Tax and matters.

(6) Records, reports, and statements.

4. DURATION. This power of attorney shall exist for an indefinite period of time.
